Two Cases of Prurigo Nodularis in Hematologic Malignancies: Hodgkin Lymphoma and Waldenström Macroglobulinemia.

Prurigo nodularis (PN) is a chronic inflammatory skin disorder marked by intense itching and firm, pruritic nodules, often linked to dermatologic, autoimmune, or systemic conditions, including malignancies such as hematologic cancers. We present two cases illustrating PN as a potential cutaneous manifestation of hematologic malignancies: a 48-year-old male with a history of psoriasis and persistent pruritus misdiagnosed as scabies, later found to have Hodgkin lymphoma, and an 86-year-old female with fatigue, anemia, and erythematous lesions diagnosed with Waldenström macroglobulinemia. These c

Translating tissue expression of STAT 1, 3 and 6 in prurigo nodularis to clinical efficacy of oral tofacitinib - A prospective single-arm investigational study.

Background Interleukin (IL)-4, IL-13, IL-17, IL-22 and IL-3 are overexpressed in prurigo nodularis (PN). They mediate their action via the Janus Kinase (JAK) Signal transducer and activator of transcription (STAT) pathway. Objectives Our aim was to study the expression of tissue STAT1, STAT3, and STAT6, as well as the efficacy of the JAK-STAT inhibitor, tofacitinib, in PN. Methods A prospective study was conducted in a tertiary care hospital. Patients with PN were recruited after excluding secondary causes. Pruritus was graded using Pruritus Grading System Score (PGSS). All cases underwent his

Thymus and activation-regulated chemokine (CCL17) as a clinical biomarker in atopic dermatitis: significance and limitations in the new treatment era.

Thymus and activation-regulated chemokine (TARC; CCL17) is a T-helper-2 chemokine that reflects atopic dermatitis (AD) disease activity. Since 2008, serum TARC levels have been commercially measured in Japan, and clinical experience has shown the usefulness of TARC. The fallacy that eczema is always visible often hinders successful treatment, when there is subclinical inflammation which is inferable from the TARC level. AD treatment has entered a new era with higher therapeutic efficacy. TARC has a different meaning than it did previously, and its significance and limitations are discussed. Fi

Distinct comorbidity profiles in atopic versus non-atopic prurigo nodularis: a cross-sectional analysis.

Prurigo nodularis (PN) is a chronic inflammatory skin condition frequently associated with atopy, yet the relationship between atopic status and comorbidity profiles in PN patients remains poorly understood. Using the 2016-2019 National Inpatient Sample database, we analyzed 7,765 patients with PN to compare epidemiological characteristics and comorbidity patterns between atopic and non-atopic cohorts. Among these patients, 1,545 (19.9%) had concomitant atopy, defined as the presence of at least two atopic conditions (atopic dermatitis, allergic rhinitis, or asthma). Atopic PN patients were mo

Healthcare Resource Utilization and Economic Burden of Prurigo Nodularis in the United States.

INTRODUCTION: The impacts of prurigo nodularis (PN) on healthcare resource utilization (HCRU) and associated costs are unclear. METHODS: This retrospective, cross-sectional claims analysis (IQVIA PharMetrics® Plus) compared HCRU and costs over 1 year in adults with PN versus matched controls (region, payer type, age, sex, year) from 2016 to 2019, and also in patients with PN receiving advanced versus localized/no therapy. For patients with data in multiple calendar years, 1 year was randomly selected. Outcomes were compared by chi-square tests, t tests, or negative binomial tests. RESULTS: W

Circulating IgE levels as a biomarker for treatment response to dupilumab among patients with prurigo nodularis.

In this retrospective study, we investigated the role of baseline IgE levels in predicting the treatment response to dupilumab in patients with prurigo nodularis (PN). Our findings showed that patients with elevated IgE experienced significantly greater relief from pruritus following dupilumab treatment, suggesting that IgE could serve as a biomarker for identifying individuals who are more likely to benefit from treatments that target interleukin (IL)-4 and IL-13. These results support the development of personalized treatment strategies based on inflammatory profiles in patients with PN.

Diagnosis and management of prurigo nodularis in skin of color patients.

Prurigo nodularis is a chronic, intensely pruritic eruption that results in excoriated papules, mostly on the extensor surfaces of the arms and legs. Previous studies have indicated that the condition disproportionally affects those of African descent. Although the etiology has not been fully elucidated, patients with prurigo nodularis have been found to have associated comorbidities such as hypertension, renal disease, and HIV infection. This review covers the principles of work-up and diagnosis of prurigo nodularis in skin of color patients as well as management considerations in this popula

New definitions and subtypes of chronic prurigo.

During the past few years, chronic prurigo, including prurigo nodularis, has become a focal point of considerable research attention, resulting in enhanced insights into this condition. For over two centuries, the term "prurigo" has been used in dermatology, often applied loosely to describe a range of pruritic, papulous conditions lacking clear diagnostic criteria. In 2017, the term "chronic prurigo" was introduced, delineating chronic prurigo, including prurigo nodularis, as a distinct disease entity characterized by chronic pruritus, a characteristic clinical history, and physical signs suc

Functional magnetic resonance imaging in prurigo nodularis: A call to study neural sensitization phenomena.

Prurigo nodularis is a chronic pruritic, inflammatory skin condition characterized by nodular skin lesions in a typical distribution pattern caused by various dermatologic and/or nondermatologic conditions. In recent years, significant advances have been made in the understanding of the cutaneous pathophysiology of prurigo nodularis, resulting in novel treatment options such as interleukin-4, -13, -17, and -31 or Janus kinase inhibitors. Many aspects of the neurophysiology are largely unknown, including the processing in the central structural and functional network involved in prurigo nodular

The Relationship Between Chronic Pruritus, Attention-Deficit/Hyperactivity Disorder, and Skin Picking-A Case Series and Narrative Review.

Chronic pruritus (CP), attention-deficit/hyperactivity disorder (ADHD), and skin picking disorder (SPD) are medical conditions that involve both somatic and psychosocial dimensions, posing unique challenges in clinical management. While CP and SPD are often observed together, the link between ADHD and these conditions is less recognized. This conceptual work describes three women who suffered from a complex interplay of CP, ADHD, and SPD treated at our specialized bi-disciplinary psychodermatological pruritus clinic. Based on our clinical observation and a narrative review of the literature, w

Evolving evidence in the neural sensitization of prurigo nodularis.

Prurigo nodularis is a chronic inflammatory skin disorder characterized by relentlessly pruritic, hyperkeratotic nodules. Recent studies have provided compelling evidence for the pivotal role of dysregulated interactions between the nervous and immune systems in its pathogenesis. This article reviews the latest findings on the neurogenic mechanisms contributing to prurigo nodularis, particularly how these processes lead to the sensation of increased itch intensity. Peripheral sensitization is primarily driven by abnormal innervation of histamine-independent, small unmyelinated C fibers, epider
