Local excision of perianal skin for pruritus ani: a case report.

Pruritus ani is a neurofunctional dermatological condition frequently encountered in proctology, characterized by persistent perianal itching that can significantly compromise the patient's quality of life. This case report details a 48-year-old male patient with a 4-year history of perianal itching. Following local excision of the perianal skin, the wound healed satisfactorily on postoperative day 32 and achieved complete resolution of itching symptoms, with the patient attaining full recovery by day 40, thereby demonstrating remarkable clinical efficacy. Additionally, this report reviews rel

Atypical presentation of Enterobius vermicularis in a cognitively impaired elderly patient: A case report.

Enterobius vermicularis infection (enterobiasis) is a common intestinal parasitosis, typically characterized by perianal pruritus, and is most prevalent in children. Diagnosing enterobiasis in cognitively impaired individuals, who may not report typical symptoms, poses a diagnostic challenge. We report the case of an 86-year-old Japanese woman with cognitive impairment who was admitted for a tibial fracture. During a routine diaper change, several motile, thread-like organisms were incidentally observed in her perianal and perivaginal areas. Microscopic examination identified the organisms as

Unraveling the Causes of Perianal Itching: Magnetic Resonance Imaging as a Diagnostic Tool for Perianal Fistulas and Its Mimics.

BACKGROUND: Perianal itching is a common complaint associated with a variety of anorectal conditions such as fistulas, abscesses, hemorrhoids and pilonidal disease. Accurate diagnosis is essential for effective treatment and magnetic resonance imaging (MRI) plays a pivotal role due to its excellent soft-tissue resolution and multiplanar imaging. OBJECTIVE: The objective of this study was to evaluate the diagnostic utility of MRI in patients with perianal itching, focusing on the identification and classification of perianal fistulas and their mimics, using St. James and Parks classification s

Practice Parameters for Evaluation and Management of Pruritus Ani on Behalf of the Italian Society of Colorectal Surgery (SICCR).

Pruritus ani (PA) is an unpleasant sensation that leads to scratching and which can seriously compromise patients' quality of life. PA can be secondary to anorectal diseases, systemic disorders, infectious diseases, dermatological conditions, irritative conditions, and psychogenic diseases. However, in many cases, PA appears to be idiopathic. Diagnostic work-up should include medical history collection, psychological evaluation, and clinical examination including anoscopy, laboratory findings, skin swabs, skin scraping, and patch test. Skin biopsy may be useful to clarify doubtful cases or if

Pruritis Ani.

Pruritus ani is characterized by severe itching of the perianal skin. It may be intermittent, constant, acute, or chronic. In 75% of cases an underlying cause is identified, and correction will often lead to significant improvement in symptoms. Treatment focuses on correcting the underlying cause, education on anorectal hygiene, identification and avoidance of triggers, and symptom management. Early and frequent discussion of expectations can help to protect the physician-patient relationship and guide treatment. This article will serve to outline the etiologies, diagnostic workup, and therape

P2 purinergic signaling and pruritus.

Pruritus is a common sensation that triggers scratching. Extracellular nucleotides and nucleosides, along with their receptors, primarily compose the purinergic signaling. The purinergic signaling mechanism in itch remains incompletely understood. Keratinocytes, fibroblasts, Langerhans cells, primary sensory nerve endings in the skin, and neurons and satellite glial cells in primary sensory ganglia (dorsal root ganglia and trigeminal ganglia) have been confirmed to express multiple subtypes of P2X and P2Y receptors. Purinergic signaling in the skin and primary sensory ganglia is involved in th

Blockade of PAR2 Signaling by Punicalagin as a Therapeutic Strategy for Atopic Dermatitis.

Atopic dermatitis is a chronic inflammatory skin disorder characterized by persistent inflammation and severe pruritus. Current anti-inflammatory agents carry risks of long-term adverse effects, while antihistamines provide limited relief of pruritus. Protease-activated receptor 2 (PAR2) has emerged as a critical mediator of both inflammation and pruritus, representing a promising therapeutic target. In this study, we investigated the therapeutic potential of punicalagin (PCG), a potent PAR2 antagonist, in atopic dermatitis. PCG fully and potently inhibited trypsin-induced PAR2 activation in H

Anal pruritus: Don't look away.

Anal pruritus is a common anorectal symptom that can significantly impair a patient's quality of life, including their mental health. It can be one of the most difficult proctological conditions to treat. Patients often delay seeking medical attention, since it is an embarrassing but non-life-threatening situation. Pruritus ani can be associated with idiopathic and secondary causes, such as anorectal diseases, cancer (anal or colorectal), dermatological and sexually transmitted diseases, fungal infections and systemic diseases. If patients are referred for a colonoscopy, this can sometimes pro

Atypical Manifestation of Enterobius vermicularis Infestation in Adults: A Report of a Rare Case.

Enterobius vermicularis, commonly known as pinworm, is a parasitic nematode primarily affecting children, with adult infestations being rare. The infection is typically transmitted via the fecal-oral route and is characterized by intense perianal itching, particularly at night. We present a case of a 39-year-old male with an unusual case of E. vermicularis infestation. He initially presented to the outpatient department with complaints of a diabetic foot ulcer, and E. vermicularis infestation was an incidental finding. This is an unusual presentation in adults. Adult Enterobius infestation mus

Pruritic Vulvovaginitis in a Young Girl: A Case of Vulvovaginal Enterobiasis.

Vulvovaginal enterobiasis is underreported and an often-missed diagnosis in prepubertal girls presenting with isolated nocturnal vulvar pruritus. An 8-year-old-girl with intense nocturnal vulvar itching for 4 months was diagnosed with enterobiasis after microscopy of a vaginal swab revealed Enterobius vermicularis (pinworm) ova. The child and her family were treated with two doses of oral albendazole, 2 weeks apart, resulting in symptom resolution. This case highlights the importance of considering enterobiasis as a differential diagnosis for nocturnal vulvar pruritus to prevent delayed treatm

A rare malignancy: A case report of early progression of anal Buschke-Löwenstein tumor into squamous cell carcinoma in an immunocompetent patient.

INTRODUCTION AND IMPORTANCE: Buschke-Löwenstein tumor (BLT) is a rare perianal lesion caused by low-risk mucosal HPV 6 or 11 but less frequently associated with high-risk HPV types. It is a large, exophytic, verrucous lesion of the anogenital region. BLT presents as a benign tumor but exhibits malignant clinical behavior and has a high rate of local recurrence and malignant transformation. The optimal treatment approach for BLT is still debated due to the lack of consensus. Various therapeutic modalities have been proposed, including topical agents, surgical excision, immunotherapy, chemo-radi

From Compression to Itch: Exploring the Link Between Nerve Compression and Neuropathic Pruritus.

Neuropathic itch is a type of chronic pruritus resulting from neural dysfunction along the afferent pathway. It is often accompanied by abnormal sensations such as paresthesia, hyperesthesia, or hypoesthesia. This condition, which may involve motor or autonomic neural damage, significantly impacts patients' quality of life, causing severe itch and associated comorbidities such as depression, disrupted sleep, and social strain. Neuropathic itch accounts for 8% of chronic pruritus cases, though this may be underestimated. This comprehensive review focuses on nerve impingement as the primary path
