Effects of air pollution on disease activity in patients with rheumatoid arthritis.

OBJECTIVES: This study aimed to evaluate the associations of air pollution with disease activity and flare rate in patients with rheumatoid arthritis (RA). METHODS: This prospective cohort study included patients with RA who were treated at a tertiary medical centre in South Korea between January 2021 and December 2024. Air pollution exposure was estimated using monthly mean concentrations of 6 air pollutants (sulfur dioxide, nitrogen dioxide, ozone, carbon monoxide, particulate matter [PM]10, and PM2.5). Disease activity and flares were recorded longitudinally at each outpatient visit. Assoc

AA in Rheumatoid Arthritis and Lupus.

Inflammation and immune dysregulation are at the center of various diseases, including autoimmune diseases, especially rheumatoid arthritis (RA) and systemic lupus erythematosus (lupus). In general, corticosteroids, disease-modifying drugs, and monoclonal antibodies against IL-6 and TNF-α, JAK inhibitors, and various other immunomodulators are extensively used in the treatment of RA and lupus. It is known that EFA (essential fatty acid) metabolism, especially that of AA, is altered in RA and lupus. It is proposed that metabolism of AA and of other PUFAs may play a role in predicting disease pr

Quantification of Chikungunya Virus RNA by Real-Time RT-PCR Assay.

RT-qPCR is characterized by‌ high sensitivity, strong specificity, excellent reproducibility, and operational simplicity, ‌enabling‌ rapid quantification of viral RNA within a short timeframe. The real-time PCR system relies upon the measurement of a fluorescent reporter during PCR, in which the amount of emitted fluorescence is directly proportional to the amount of the PCR product in a reaction. Here, we describe the use of TaqMan® probe-based one-tube Real-time reverse transcription quantitative polymerase chain reaction (RT-qPCR) for the detection and quantification of the Chikungunya viru

Contemporary Management of Knee Chondral Defects, Part IV: Scaffold-Augmented Repair and Osteochondral Allograft Transplantation.

Scaffold-augmented repair and osteochondral allograft (OCA) transplantation represent hybrid approaches that integrate biologic stimulation with structural support. These strategies are particularly relevant when marrow stimulation alone is insufficient, but joint-preserving alternatives to arthroplasty remain viable. Scaffold-based techniques, including autologous matrix-induced chondrogenesis, enhance marrow stimulation by stabilizing the reparative clot within a three-dimensional matrix. Clinical studies demonstrate improvements in pain and function, and certain scaffolds show superior defe

Contemporary Management of Knee Chondral Defects, Part I: A Framework for Treatment Selection.

Focal chondral defects of the knee present a heterogeneous clinical problem with no universally applicable solution. Lesion size, containment, subchondral bone involvement, joint alignment, meniscal integrity, patient activity demands, and system-level constraints variably influence durability, yet outcomes are often interpreted without adequate contextualization. As a result, treatment comparisons frequently reflect differences in indication rather than superiority of technique. This review proposes a constraint-based framework to guide treatment selection in focal knee chondral defects. Cart

Contemporary Management of Knee Chondral Defects, Part II: Structural Cartilage Restoration with Focal Resurfacing Implants and Osteochondral Autograft Transfer.

Structural cartilage restoration strategies provide mechanical solutions for focal knee chondral defects in which biologic repair alone is unlikely to withstand load-bearing demands. These approaches are most applicable to discrete lesions in otherwise preserved joints where restoration of articular congruity and immediate mechanical stability are required. This review evaluates focal resurfacing implants and osteochondral autograft transfer system procedures as structural joint-preserving interventions. Contemporary metal and polymer-based focal implants demonstrate meaningful short- to mid-t

SMSC-EVs restore chondrocyte function through S1PR1-mTORC2-mediated mitochondrial fusion and GPS2-HDAC1-driven epigenetic regulation.

OBJECTIVE: In osteoarthritis (OA) treatment, synovial mesenchymal stem cell-derived extracellular vesicles (SMSC-EVs) hold therapeutic potential; however, the mechanisms coordinating mitochondrial and epigenetic repair remain unclear. DESIGN: Chondrocyte proliferation, apoptosis and migration were assessed in vitro. Mitochondrial function was evaluated via membrane potential, oxygen consumption, ATP and reactive oxygen species (ROS) levels and network morphology. RNA sequencing and proteomics identified altered pathways. Key molecules (MFN2, S1PR1, GPS2, mTOR components) were investigated usi

Early histological advantage of CD271-positive adipose-derived mesenchymal stromal cells isolated from the infrapatellar fat pad in experimental knee osteoarthritis.

BACKGROUND: Mesenchymal stromal cells (MSCs) have been increasingly explored as a minimally invasive treatment for osteoarthritis (OA), primarily due to their anti-inflammatory, immunomodulatory, and analgesic properties. However, MSC populations are biologically heterogeneous, and optimal cell phenotypes for intra-articular therapy remain unclear. CD271 has been identified as a marker associated with MSC subpopulations with enhanced regenerative potential. The aim of this study is to evaluate whether intra-articular injection of phenotype-selected CD271-positive adipose-derived MSCs (AD-MSCs)

Extracellular vesicles in osteoarthritis: pathological mechanisms, regenerative therapies and clinical translation.

Osteoarthritis (OA) is a common chronic degenerative joint disease that has severe impacts on quality of life and creates a substantial socioeconomic burden. Conventional therapeutic strategies primarily focused on alleviating symptoms such as pain and functional impairment, with limited efficacy and an inability to reverse the disease process. Over the past two decades, regenerative medicine has been developed rapidly. Extracellular vesicles (EVs) have garnered widespread attention and there has been an upsurge of research interest in their immunomodulatory and regenerative potential in OA. H

Circulatory pathology in osteoarthritic subchondral bone.

OBJECTIVE: With the recognition that crosstalk between articular cartilage and subchondral bone may contribute to osteoarthritis (OA), interest in the subchondral osteoblast secretome has grown significantly. Osteoblasts respond to alterations in their microenvironment by expressing a range of signaling and structural cytokines affecting bone remodeling and cartilage breakdown characteristic of OA. This narrative review explores observations made with circulatory imaging of subchondral bone in OA and presents a reconciliation of them in the context of a subchondral hypoxic microenvironment and

A systematic review and Meta-Analysis: Exploring the efficacy of self-managed treatments for Temporomandibular Disorders (TMD): The role of home exercise and digital therapies.

Temporomandibular disorders (TMD) are highly prevalent and frequently associated with chronic pain and functional impairment, forming part of the diagnostic spectrum in cranio-maxillofacial surgery. Scalable, self-managed interventions - including home-exercise programs, psychoeducational strategies, and digitally supported approaches - are increasingly used, yet their pooled effectiveness has not been systematically quantified. A systematic search of PubMed, Scopus, and Cochrane CENTRAL identified randomized controlled trials evaluating predominantly self-managed or remotely delivered interve

Severe Osteoarthritis and Avascular Necrosis of the Hips in a 37-Year-Old Man: Case Report.

BACKGROUND: Hip osteoarthritis has been considered a condition commonly encountered in the elderly, however, it is predicted to be increasing in prevalence in the population below age 50. Avascular necrosis (AVN) is usually caused by a compromise in blood flow, usually occurring in patients with risk factors. CASE REPORT: A 37-year-old man presented with left-sided hip pain. Magnetic resonance imaging revealed severe left hip osteoarthritis, as well as bilateral hip AVN, although the patient had no known risk factors. CONCLUSIONS: Historically, differential diagnoses for hip pain in younger

Persistent Shoulder Instability and Retained Primitive Reflexes. Addressing a Potential Gap in Adult Musculoskeletal Rehabilitation Care.

SYNOPSIS: Using a case of recalcitrant type III shoulder instability, this editorial explores the potentially overlooked link between retained primitive reflexes and persistent musculoskeletal conditions. We explore how incomplete primitive reflex integration may disrupt motor control, compromise dynamic stability, and interact with neurodevelopmental traits like generalized joint hypermobility. Addressing a potential gap in adult musculoskeletal care, we establish a theoretical framework and foundational justification for future research. To investigate this potential paradigm shift, we outli
