Intrathoracic rupture of amebic liver abscess: a case report and literature review.

BACKGROUND: Amebic liver abscess (ALA) is a serious complication of Entamoeba histolytica infection. In rare cases, ALA may rupture into the thoracic cavity, leading to a high risk of death. Differentiating intrathoracic ALA rupture from reactive pleural effusion is essential for predicting the clinical course and appropriate management. CASE PRESENTATION: A 46-year-old bisexual man with well-controlled human immunodeficiency virus infection presented with pain in the right shoulder and upper abdomen. Imaging revealed a solitary liver abscess with diaphragmatic rupture, right pleural effusion

Characterization of pathogenic endosymbionts in free-living amoebae isolated from indoor sport water complexes.

This study aimed to investigate FLA and their endosymbionts in indoor sport water complexes (ISWCs). A total of 90 samples were collected from 15 ISWCs. After morphological and molecular detection, fungi and viral pathogens were investigated in isolated FLA using real-time PCR. Totally, 51 (56.7%) were positive for FLA, including Acanthamoeba sp. Vermamoeba sp. Vahlkampfiidae, and Thecamoeba spp. in 33 (36.7%), 20 (22.2%), 24 (26.7%), and 2 (2.2%) of samples, respectively. The number of positive samples was 17 in each sauna biofilm, Jacuzzi, and pool water. The genotypes T4, T5, and T11 were c

An amebic liver abscess in a female child with successful recovery in a non-endemic area: a case report.

We report the case of a 2-year-and-11-month-old female child who presented with a 10-day history of recurrent fever and abdominal pain, accompanied by significantly elevated inflammatory markers. Initially, incomplete Kawasaki disease (IKD) was strongly suspected; however, the patient continued to experience recurrent high fever and abdominal pain despite treatment with intravenous immunoglobulin (IVIG) and antibiotics. Enhanced thoraco-abdominal computed tomography (CT) imaging revealed hepatomegaly and the presence of an abscess in the anterosuperior segment of the right lobe of the liver. T

Off-label Use of Gastrointestinal Multiplex PCR Panel for the Diagnosis of Amoebic Liver Abscess: A Three-case Pediatric Report.

Diagnosis of amoebic liver abscess is challenging. A multiplex polymerase chain reaction panel, traditionally employed for the detection of enteric pathogens in stool, proved useful for the rapid diagnosis in 3 pediatric patients when used off-label on liver abscess fluid. While this approach enabled timely and targeted antiamoebic therapy, highlighting its potential in high-burden, resource-limited settings, it represents an unvalidated use of the technology that requires further investigation.

Unusual presentation of amoebic liver abscess with pseudohaemoptysis.

Amoebic liver abscesses, caused by Entamoeba histolytica, can rarely rupture into the pleural space, leading to massive pseudohaemoptysis. We report a case of a patient who presented with severe haemoptysis, later diagnosed with a ruptured amoebic liver abscess. Despite a negative pus culture, a positive E. histolytica IgM test confirmed the diagnosis. The patient was managed with intravenous metronidazole and ceftriaxone, along with pigtail catheter drainage, leading to a full recovery. This case highlights the importance of considering hepatic abscess rupture in unexplained haemoptysis and e

Unusual Case of Hepatic Amebiasis Without Recent Endemic Exposure: A Diagnostic Puzzle.

Entamoeba histolytica is a parasite endemic to many countries. While many people who are infected remain asymptomatic, serious complications can also occur. We report the case of a 25-year-old male immigrant from Guatemala who presented to the ED with epigastric pain and non-bloody diarrhea. Abdominal imaging revealed at least 10 hypodense liver lesions, prompting a further workup. He was subsequently diagnosed with invasive amebiasis infection due to E. histolytica and was initiated on appropriate antimicrobial therapy. This case underscores the importance of maintaining a broad infectious di

Amebic Liver Abscess in a Patient with Polycystic Liver Disease: A Case Report.

INTRODUCTION: Amebic liver abscess is a parasitic infection caused by Entamoeba histolytica, which reaches the liver via the portal circulation after invading the colon. While it typically presents with fever, right upper quadrant pain, and elevated inflammatory markers, its clinical and radiologic features can resemble those of bacterial liver abscesses. In patients with structural liver abnormalities, such as polycystic liver disease, symptoms may be atypical and delay diagnosis. CASE PRESENTATION: We report the case of a 75-year-old woman with autosomal dominant polycystic kidney disease a

Hemobilia Due to Spontaneous Arterioportal Fistula in a Patient With Liver Abscess: A Rare Presentation.

Hemobilia is a rare cause of gastrointestinal bleeding, most commonly resulting from trauma or iatrogenic injury. Vascular complications such as arterioportal fistula (APF) are extremely uncommon, especially when occurring spontaneously in the setting of a liver abscess. We report a case of a 59-year-old man who presented with fever, abdominal pain, and later developed melena and biliary obstruction after percutaneous drainage of a presumed amoebic liver abscess. Endoscopic retrograde cholangiopancreatography revealed active bleeding from the biliary orifice, confirming hemobilia. Computer tom

Amebic Liver Abscess: An Unusual Cause of Budd-Chiari Syndrome.

Amebiasis is a significant public health issue in tropical regions, with liver abscess as its most common extraintestinal complication. Budd-Chiari syndrome secondary to amoebic liver abscess is rare and seldom reported. We present a 37-year-old man with poorly controlled Type 1 diabetes who developed fever, abdominal pain and distension. Imaging identified a left lobe liver abscess with hepatic vein and inferior vena cava thrombosis, consistent with Budd-Chiari syndrome. The patient received image-guided drainage, broad-spectrum antimicrobials, and anticoagulation, leading to clinical improve

Clinical characteristics and outcomes of liver abscesses: a 12-year retrospective analysis from a tertiary care center in the United Arab Emirates.

BACKGROUND: Liver abscesses are a cause of morbidity worldwide and their outcomes can vary remarkably in various regions of the world. In the United Arab Emirates (UAE) the clinical picture of liver abscesses is not fully understood. We aimed to fill this gap by analyzing the clinical features, microbiology, and outcomes for patients with liver abscess over a 12-year period. METHODS: We conducted a retrospective observational study of all adult patients (≥18 years) with confirmed diagnosis of liver abscess at Tawam Hospital, Al Ain, UAE, from January 2012 to January 2024. Hunting Trophozoites: A Rare Case of Amebiasis in an Infant.

The authors present a case of neonatal amebiasis in a 5-month-old girl who exhibited symptoms including vomiting, refusal to feed, abdominal distension, and mucoid, bloody stools. Sigmoidoscopy revealed multiple aphthous ulcers, and a colonic biopsy was submitted with clinical suspicion of eosinophilic colitis. The histologic examination of the biopsy, however, confirmed the diagnosis of amebic colitis through identification of trophozoites of Entamoeba histolytica. The infant, who was likely infected by her caregiver or mother, was successfully treated with intravenous metronidazole. This rep

Alleles of CYP3A5 and their association with renal function in chronic kidney disease.

BACKGROUND: The cytochrome P450 family 3 subfamily A polypeptide 5 (CYP3A5) gene plays an important role in renal function through its product's involvement in metabolizing endogenous substances and drugs, including immunosuppressants used following kidney transplantation. A single-nucleotide polymorphism, CYP3A5*3 (rs776746), produces a non-functional variant that may influence progression of chronic kidney disease (CKD) by impairing renal filtration. However, the frequency of the CYP3A5*3 allele in the Thai population and its association with renal parameters remain underexplored. This study

Autochthonous amebiasis in Spain.

Amoebiasis is an infection caused by the intestinal protozoan Entamoeba histolytica. It is transmitted via the faecal-oral route and primarily affects the intestinal tract and the liver. It is prevalent in tropical countries but is uncommon in Europe, where it is typically diagnosed in immigrants and tourists from endemic regions. However, an increase in autochthonous cases is being observed in Spain. A case is presented of a 56-year-old Spanish male with no relevant epidemiological history who attended the Emergency Department due to right hypochondrial pain and fever. An abdominal CT scan re

Adaptive approach to diagnosis of amebic liver abscess in non-endemic areas.

Entamoeba histolytica infections range from asymptomatic intestinal amebiasis to more severe syndromes like invasive colitis or hepatic abscess. While diagnostic tests available for assessing these infections have evolved, the optimal use of newer diagnostics like enteric multiplex Polymerase Chain Reaction (PCR) panels has not been fully established. Here we describe the case of a 34-year-old male with an amebic liver abscess (ALA) that was diagnosed via multiplex Gastrointestinal (GI) PCR panel on formed stool and subsequently confirmed by testing liver abscess fluid (off-label) on the same

Recurrent amoebic liver abscess in a non-endemic region: A case report with dual diagnostic approach.

BACKGROUND: Entamoeba histolytica infection can range from asymptomatic intestinal amoebiasis to more severe conditions such as invasive colitis or hepatic abscess. Amoebic liver abscess (ALA) remains a significant health issue in the tropical region. CLINICAL CASE: We describe the case of a 45-year-old male from Algeria who had traveled to sub-Saharan African regions endemic for amoebiasis. Upon his return, he presented with an ALA, which was diagnosed via ultrasound and MRI, and supported by strongly positive amoebic serology. Multiplex PCR testing of liver abscess fluid confirmed the prese

Patient with myelodysplastic neoplasm and giant amoebic liver abscess imported from Italy complicated by intestinal obstruction: a case report and review of the literature.

Infection with Entamoeba histolytica is widespread and cosmopolitan, but is particularly common in hot zone countries in areas with poor sanitary and hygienic conditions, a lack of access to safe drinking water, and noncompliance with food hygiene. Currently, no cases of indigenous E. histolytica infections have been registered in Poland; only cases imported from areas with a hot climate zone have been reported. So far, no severe extraintestinal amoebiasis has been diagnosed in Poland in people returning from the Mediterranean area. We report an unusual case of concomitant large liver abscess

Serodiagnosis of amoebic abscess: a retrospective diagnostic accuracy study of kits marketed in Europe.

UNLABELLED: Amoebiasis is a cosmopolitan parasitic disease caused by Entamoeba histolytica. The most lethal form is extra-intestinal amoebiasis, mainly manifesting as liver abscesses. Diagnosis is based on clinical, radiological, and biological tests. However, there are currently few serology reagents still available on the European market for in vitro diagnosis, and comparative studies of existing reagents are required. We evaluated the performances of the four currently available reagents, either as standalone tests or in combination. Two enzyme-linked immunosorbent assays (ELISAs) for the d

Uncommon etiology of Cullen sign and Grey Turner sign.

Cullen´s sign and Grey Turner sign are, respectively, the cutaneous ecchymoses located in periumbilical region and tissues flanks along the lower portion of the abdomen. Both have been observed in <1% of individuals with acute pancreatitis, suggesting poor prognosis in terms of gravity and mortality. However, these signs are not exclusive for acute pancreatitis, as they can appear in other identities such as: ruptured ectopic pregnancy, aortic aneurysm, rectus abdominis muscle hematoma, perforated duodenal ulcer, common bile duct rupture and biliary peritonitis, idiopathic perirenal hemorrhage
