Advancing Our Understanding of Hidradenitis Suppurativa: A Comprehensive Guide for Clinicians Designed to Improve Therapeutic Outcomes for Patients-Part 1.

Hidradenitis suppurativa (HS) is a chronic inflammatory cutaneous disorder that usually starts in the second or third decade of life and has a profound negative impact on quality of life of those who are affected. HS presents with both inflammatory and cicatricial lesions that are often markedly symptomatic, including pain and purulent discharge, and commonly affect areas that are intertriginous, resulting in additional frictional and mechanical discomfort. The pathophysiology is complex and incompletely understood and results in changes that are transcutaneous, involving the epidermis, superf

Advancing Our Understanding of Hidradenitis Suppurativa: A Comprehensive Guide for Clinicians Designed to Improve Therapeutic Outcomes for Patients-Part 2.

Hidradenitis suppurativa (HS) is a chronic inflammatory cutaneous disorder that often starts in the second or third decade of life, induces both inflammatory and cicatricial cutaneous lesions, and frequently involves intertriginous and anogenital anatomic sites. Abscesses, nodules, and draining tunnels are reversible inflammatory lesions that cycle over time through unpredictable periods of exacerbation; cicatricial lesions are permanent and persist as fibrotic scars and nondraining tunnels, the latter reflecting the shells of prior draining tunnels. HS has a profound adverse impact on overall

Delayed Drug Hypersensitivity: Mechanistic Advances and Emerging Therapies.

PURPOSE OF REVIEW: Delayed drug hypersensitivity reactions span mild cutaneous eruptions, severe cutaneous adverse reactions (SCARs), and internal-organ injury. We review how pharmacogenomic, single-cell, and spatial methods have shown that these syndromes are driven by shared effector mechanisms, and how identifying which one predominates opens the way to targeted treatment. RECENT FINDINGS: HLA associations are necessary but not sufficient to explain who reacts. Susceptibility is increasingly attributed to a pre-existing, often virus-shaped T-cell repertoire and to where tissue-resident mem

Short-course vunakizumab associated with rapid improvement of acute flares in paediatric generalised pustular psoriasis: a three-patient case series.

BACKGROUND: Paediatric generalised pustular psoriasis (GPP) is rare, frequently relapsing and associated with limited evidence for targeted therapy. Vunakizumab is an anti-interleukin-17A monoclonal antibody, but data in paediatric GPP are scarce. METHODS: We retrospectively reviewed three boys with recurrent GPP who received short-course vunakizumab during acute flares. Clinical characteristics, diagnostic details, treatment timelines, follow-up outcomes and Generalized Pustular Psoriasis Area and Severity Index (GPPASI) scores were collected. RESULTS: A 13-year-old boy (87.5 kg) with ciclo

Generalized Pustular Psoriasis: A Cost-of-Illness and Health-Related Quality of Life Study in Italy.

OBJECTIVE: Generalized pustular psoriasis (GPP) is a life-threatening rare disease, with an estimated prevalence of 1-3 cases per million individuals in Italy. Despite its clinical severity, evidence on the economic impact of GPP remains limited. This study aimed to estimate the annual societal economic burden of GPP in Italy and assess the health-related quality of life (HRQoL) of patients with GPP. METHODS: A prevalence-based, probabilistic cost-of-illness model was developed using a societal perspective. Data were drawn from primary and secondary sources, including published and grey liter

Efficacy of Apremilast in Patients with Psoriasis with Limited Skin Involvement, Including High-Impact Site Involvement: A Pooled Analysis of Six Trials.

INTRODUCTION: Plaque psoriasis is associated with decreased quality of life (QOL) even in patients with limited skin involvement, particularly those with high-impact site (HIS) involvement. The efficacy of apremilast in patients with plaque psoriasis has been demonstrated in phase 3 and 4 clinical trials. Here, we evaluated the efficacy of apremilast across six clinical trials for patients with psoriasis with low affected body surface area (BSA), including patients with HIS involvement. METHODS: Data were pooled across five randomized, double-blind, placebo-controlled trials (patients were ra

Development and assessment of an artificial intelligence-based tool for scoring lesion severity in generalized pustular psoriasis clinical images.

Generalized pustular psoriasis (GPP) is a rare, chronic, systemic inflammatory disease characterized by unpredictable chronic symptoms and periods of flaring. Assessing GPP severity is important for defining treatment goals and evaluating treatment responses, yet the rarity of the disease and the subjectivity of manual evaluation complicate its standardization. To address this, we developed the automatic Generalized Pustular Psoriasis Physician Global Assessment (GPPGA), a digital imaging tool powered by artificial intelligence. On the basis of the validated GPPGA scoring system, this framewor

A prospective, observational study of the epidemiology, disease characteristics and treatment outcomes of generalized pustular psoriasis in India.

BACKGROUND: Generalized pustular psoriasis (GPP) is a rare, severe inflammatory skin disease marked by widespread sterile pustules on erythematous skin. It poses clinical challenges due to its systemic involvement, unpredictable course and risk of serious complications. OBJECTIVES: To evaluate the clinical features, disease severity, triggers, treatment outcomes and quality of life (QoL) in patients with GPP in India. METHODS: A prospective observational study was conducted at a tertiary care hospital in North India from March 2022 to March 2025. Disease severity in patients diagnosed with G

Acute Myocarditis During a Generalized Pustular Psoriasis Flare: A Possible Pharmacogenetic Interaction in the Setting of Severe Systemic Inflammation?

We report a case of acute myocarditis in a 25-year-old man with refractory generalized pustular psoriasis (GPP) following treatment with spesolimab, an anti-IL-36 receptor monoclonal antibody. Cardiac magnetic resonance fulfilled the Lake-Louise criteria for myocarditis, and genetic testing revealed a heterozygous pathogenic/likely pathogenic MYH7 variant, suggesting a possible gene-environment-drug interaction. The patient was managed with supportive care and presented no complications. In outpatient evaluation, the patient has remained asymptomatic with no further hospital admissions. This c

Interleukin-36 receptor antagonist deficiency patient with a novel mutation.

BACKGROUND: The deficiency of interleukin-36 receptor antagonist (DITRA) is a monogenic autoinflammatory condition associated with generalized pustular psoriasis (GPP). Diagnostic criteria and treatment recommendations for DITRA are inadequate. The diagnosis is established by detecting a biallelic loss-of-function mutation in IL36RN, which leads to activation of the IL36 pathway. OBSERVATION AND DISCUSSION: We present a pediatric case of GPP with a novel IL36RN mutation that supports the diagnosis of DITRA. In patients with DITRA unresponsive to IL-1 targeted therapies, favorable results may

Real-World Evidence of Brodalumab Effectiveness for the Treatment of Psoriasis.

Brodalumab is a recombinant, fully human monoclonal antibody antagonist of the human interleukin-17 (IL-17) receptor A. It is indicated for the treatment of moderate-to-severe plaque psoriasis in adult patients who are candidates for systemic therapy or phototherapy and have failed to respond or have lost response to other systemic therapies. Brodalumab blocks signaling mediated by IL-17 cytokines, disrupting the inflammatory feedback loop between immune cells and keratinocytes that drives psoriasis, which may explain its efficacy in clinical trials. To explore its postapproval effectiveness,

The IL-36 cytokine family: From barrier immunity to therapeutic target in inflammatory diseases.

The interleukin-36 (IL-36) cytokine family is an important regulator of inflammatory responses at epithelial barrier surfaces, with particularly prominent roles in skin immunity and inflammatory disease. As members of the IL-1 superfamily, IL-36 cytokines include three agonists (IL-36α, IL-36β, and IL-36γ) and two antagonists (IL-36Ra and IL-38), which signal through a shared heterodimeric receptor complex to activate MyD88-dependent pathways, including NF-κB and MAPK, and induce pro-inflammatory cytokines, chemokines, and antimicrobial peptides. A defining feature of IL-36 biology is its depe
