An Exploratory Study to Identify Factors Associated with Recurrence of Generalized Pustular Psoriasis Flares in Adult Patients in Japan.

Generalized pustular psoriasis (GPP) is a rare, systemic inflammatory disease characterized by a heterogeneous and often unpredictable clinical course involving chronic symptoms and recurrent flares. Due to the rarity of GPP, our understanding of factors associated with flare recurrence remains unclear. Using an existing data set of adult patients with GPP in Japan, we performed univariate and multivariate analyses to investigate potential factors related to GPP flare recurrence, including patient demographics,  treatment and severity of baseline flare. In total, 150 patients with a baseline f

IL-36 Receptor Antagonist Spesolimab for Generalized Pustular Psoriasis Combined with Palmoplantar Pustulosis: A Case Report.

OBJECTIVE: We have entered the era of using biologics and small-molecule targeted drugs to treat diseases. Although there have been many reports on the use of the anti-interleukin-36 (IL-36) receptor antagonist spesolimab for the treatment of generalized pustular psoriasis (GPP) recently, the clinical application of spesolimab for the treatment of GPP combined with PPP (palmoplantar pustulosis) is rarely reported, and there is a lack of evidence on the safety and efficacy of the spesolimab. The clinical application of spesolimab for GPP combined with palmoplantar pustulosis (PPP) is rarely repo

Efficacy and Safety of Imsidolimab for Generalized Pustular Psoriasis.

BACKGROUND: Generalized pustular psoriasis (GPP) is a rare, life-threatening disease attributed to aberrant interleukin-36 (IL-36) activity, often due to variants in the IL-36 receptor antagonist gene. Imsidolimab is a novel, humanized, affinity-matured immunoglobulin G4 monoclonal antibody that binds the IL-36 receptor and antagonizes IL-36 signaling. METHODS: Two phase 3 trials were conducted at 26 clinical sites within 11 countries investigating imsidolimab treatment for GPP. GEMINI-1 was a double-blind, placebo-controlled trial that randomly assigned 45 patients (18-80 years of age) with

Immunohistochemistry Revealed Distinct IL-36γ Localization Among Different Skin Diseases.

IL-36γ is highly expressed in psoriatic skin lesions and promotes neutrophil infiltration through the induction of neutrophil chemotactic chemokines. While IL-36γ has been established to play an important role in the pathogenesis of psoriasis, its localization in the psoriatic epidermis has not been fully elucidated, and the difference in its expression patterns among other inflammatory skin diseases remains unclear. We investigated IL-36γ localization using immunohistochemistry in skin specimens from patients with psoriasis vulgaris (n = 36), generalized pustular psoriasis (n = 11), pyoderma

RSV infection-associated pediatric generalized pustular psoriasis with innate immune activation: a case report.

Generalized pustular psoriasis (GPP) in infancy is uncommon and is often precipitated by infection, drugs, or systemic inflammatory responses. Although viral illnesses such as influenza have been reported to trigger GPP, acute exacerbation secondary to respiratory syncytial virus (RSV) is rarely described. We report a case of a 2-year-old girl with a prior history of mild psoriasis who developed widespread sterile, small pustules several days after onset of RSV infection symptoms (cough and low-grade fever), consistent with an RSV-associated acute exacerbation of GPP. Laboratory testing was po

The Role of Genetic Testing in the Differential Diagnosis of Pustular Dermatoses: A Pediatric Case of Generalized Pustular Psoriasis Associated with the IL36RN Variant.

General pustular psoriasis (GPP) is a rare, potentially life-threatening neutrophilic dermatosis. Pediatric cases are uncommon and often misdiagnosed due to overlapping clinical and histopathological features with other pustular dermatoses. We present a case of an 11-year-old boy, initially diagnosed with Sneddon-Wilkinson syndrome, who presented with disseminated pustular eruptions, with no response to antibiotics, dapsone, and glucocorticosteroids. In histopathology, we observed subcorneal neutrophilic pustules. Due to atypical features and poor treatment response, the patient underwent gene

IL-36-Driven Inflammation in Generalized Pustular Psoriasis: Immunological Insights from Plaque Psoriasis and Implications for Targeted Therapy.

Generalized pustular psoriasis (GPP) is a rare, severe, and potentially life-threatening inflammatory dermatosis increasingly recognized as a distinct disease entity rather than a variant of plaque psoriasis. Emerging evidence indicates that GPP is primarily driven by dysregulation of the interleukin-36 (IL-36) signaling axis, leading to amplification of proinflammatory cascades in keratinocytes and a predominantly innate, neutrophil-driven immune response. This promotes rapid neutrophil recruitment, sterile pustule formation, and abrupt cutaneous and systemic inflammation. Consistent with thi

Vunakizumab and Acitretin for Elderly Refractory Generalized Pustular Psoriasis: A Case Report and Comprehensive Literature Review.

Generalized pustular psoriasis (GPP) is a severe and potentially life-threatening form of psoriasis. Although spesolimab, an inhibitor of the interleukin (IL)-36 pathway, has been approved for the treatment of GPP, access to this agent remains limited. Vunakizumab, a humanized IgG1/κ monoclonal antibody that selectively neutralizes interleukin (IL)-17A inhibitors, is not yet approved for GPP. We herein report a 72-year-old male with a 40-year history of plaque psoriasis who developed GPP refractory to methotrexate combined with guselkumab and acitretin (20 mg/day).
