Generalized pustular psoriasis: a multicentric study on patient characteristics and clinical burden.

The objective of this study was to assess the demographic characteristics and impact on quality of life (QoL) of patients with PPG in France through a multicentre study. The results of the study are as follows: The PRO [PUSH-D, PHQ-9 et GAD-7] revealed that more than half of the patients exhibited a significant impact on their quality of life. High scores for fatigue, stress, skin and joint pain were reported, with 65% of patients at risk of mild to severe depression. The clinical burden was also assessed. A total of 48.8% of patients were hospitalised, while 39% took sick leave. This study is

Literature review and expert opinion on diagnosis and current management of generalized pustular psoriasis.

INTRODUCTION: Generalized pustular psoriasis (GPP) is a rare, chronic, systemic, autoinflammatory disease characterized by the eruption of sterile pustules, often accompanied by more general symptoms, such as fever, fatigue, and a burning sensation in the skin. GPP can be potentially life-threatening, if untreated, as it can lead to complications, such as sepsis and heart failure. AREAS COVERED: In this literature review and expert opinion article, we provide an overview of the epidemiology, pathogenesis, clinical presentation, diagnosis, and treatment of GPP. Eleven dermatologists representi

Real-world clinical characteristics and treatment patterns of generalized pustular psoriasis flares.

BACKGROUND: Clinical characteristics and treatment of generalized pustular psoriasis (GPP) flares are poorly understood. OBJECTIVE: To characterize patients by GPP flare status, quantify flare timing/frequency, and understand peri-flare treatment patterns. METHODS: This cohort study utilized electronic health records (2017-2023) from outpatient dermatology clinics in the OMNY Health real-world data platform. Patients were indexed at first GPP diagnosis code; encounter-level GPP flare status was established from previously developed algorithms. Annualized flare rate, time between flares, and

Ixekizumab as a successful treatment in pediatric generalized pustular psoriasis.

BACKGROUND: Generalized Pustular Psoriasis (GPP) is an autoinflammatory, multisystemic disease, characterized by widespread eruption of neutrophilic pustules on erythematous base, accompanied by systemic symptoms such as fever, leukocytosis, arthralgia, and general malaise. Globally, the disease is rare, particularly in children. If not adequately diagnosed and treated, systemic inflammation and multiorgan involvement can be life-threatening. The pathogenesis of GPP mainly involves the innate immune system, with inflammatory processes and neutrophil activation driven primarily by IL-36, but al

Efficacy of granulocyte and monocyte adsorptive apheresis on skin and joint manifestations of palmoplantar pustulosis with pustulotic arthro-osteitis: A multicentric, prospective, observational study.

Granulocyte and monocyte adsorptive apheresis (GMA) selectively removes activated granulocytes and monocytes from the peripheral blood. In 2012, GMA was approved in Japan as a treatment for generalized pustular psoriasis and localized pustular psoriasis or palmoplantar pustulosis (PPP). Limited evidence from case reports and monocentric studies suggested that GMA is an effective treatment for skin and joint symptoms of PPP with pustulotic arthro-osteitis (PAO). The present, prospective, observational study was performed at three dermatology departments in Japan to evaluate the efficacy and saf

IL-36-driven pustulosis: Transcriptomic signatures match between generalized pustular psoriasis (GPP) and acute generalized exanthematous pustulosis (AGEP).

BACKGROUND: Due to similarities, the distinction between generalized pustular psoriasis (GPP) and acute generalized exanthematous pustulosis (AGEP) has been a matter of debate for a long time. OBJECTIVES: Our aim was to define the molecular features of GPP and AGEP. METHODS: We analyzed skin biopsy samples and clinical data from 125 patients with AGEP, GPP, palmoplantar pustulosis (PPP), plaque psoriasis (PSO), and nonpustular cutaneous adverse drug reactions (ADRs), as well as from healthy skin controls using RNA-sequencing and blinded histopathologic analyses. RESULTS: The transcriptome a

Characteristics and Outcomes of Generalized Pustular Psoriasis Patients Managed Through Inpatient Asynchronous Teledermatology: A Retrospective Study.

BACKGROUND: Inpatient teledermatology (ITD) is a growing telemedicine modality aimed at addressing the shortage of dermatologists in hospitals, particularly for the management of complex skin disorders. Generalized pustular psoriasis (GPP) is a rare and potentially life-threatening condition characterized by widespread sterile pustules and systemic inflammation. Despite advances in targeted therapies, treatment is often limited to traditional agents such as cyclosporine and infliximab, due to restrictions in formulary access and insurance coverage. OBJECTIVE: This study aimed to characterize

Quality of life of patients with pustular psoriasis is inferior to that of patients with plaque psoriasis in Japan: A multicenter study with questionnaires, the short Form-36, and other patient-reported outcomes.

Generalized pustular psoriasis is a rare but severe form of psoriasis, accounting for 7.5% of all psoriasis cases. We investigated whether the disease burden and quality of life of patients with generalized pustular psoriasis were lower than those of patients with psoriasis vulgaris in Japan. Patients registered in the Western Japan Psoriasis Registry, a prospective cohort of patients with psoriasis treated at 31 facilities specializing in psoriasis medicine, were surveyed using the SF-36v2 and other patient-reported outcomes. We enrolled patients with generalized pustular psoriasis (n = 97) a

Case report: Efficacy of tofacitinib in the treatment of generalized pustular psoriasis.

Generalized pustular psoriasis (GPP) is a rare and potentially life-threatening autoimmune inflammatory skin disease. Tofacitinib is a non-selective, first-generation Janus kinase (JAK) inhibitor. Currently, both domestic and international reports regarding the use of tofacitinib mainly have primarily focused on the treatment of psoriasis vulgaris and psoriatic arthritis. As GPP is a rare skin disease, reports on the use of tofacitinib in the treatment of GPP are rare. This report presents a case of severe GPP that was effectively treated with tofacitinib. This case suggests that this non-sele

Spesolimab for the Treatment of Generalized Pustular Psoriasis Flares.

BACKGROUND: Generalized pustular psoriasis (GPP) is a rare, life-threatening neutrophilic skin condition characterized by widespread sterile eruptions. Treatment approaches vary due to a lack of standardized guidelines. Interleukin-36 signaling is involved in GPP activation and disease progression. An interleukin-36 receptor monoclonal antibody, spesolimab, is a new treatment option for GPP flares. The Effisayil 1 clinical trial investigated the efficacy and safety of spesolimab, a remarkable task due to the unpredictable nature of GPP flares coupled with intrinsic trial challenges. METHODS:
