Eosinophilic pustular folliculitis starting initially only with palmoplantar pustular lesions. Report of a case and review of the literature.

We report a 23-year-old Japanese male with eosinophilic pustular folliculitis (EPF) that had started with palmoplantar rash. Only when follicular pustules appeared on the bilateral cheek 31 months later, we revised our initial diagnosis of pustulosis palmaris et plantaris (PPP) to EPF, and all the skin eruptions cleared mostly with indomethacin. A review of the Japanese literature for the past 20 years disclosed that in 207 cases of EPF so far reported, palmoplantar pustular lesion was noted in 38 (18%). Among them, in 16 cases (8%) the skin lesions started first from the palmoplantar region w

Renal manifestation of pustulosis palmaris et plantaris.

Diffuse proliferative glomerulonephritis without IgA deposits (DPGN) in patients with pustulosis palmaris et plantaris (PPP) is described. This association has not been previously reported. Although PPP was resolved transiently after tonsillectomy, it was worsened coincidently at the time of exacerbation of DPGN. Improvement of proteinuria and PPP was observed after antiplatelet therapy. It was suggested that the renal manifestation and the development of PPP were associated.

Complement fragment C4d and Bb levels in inflammatory skin diseases (e.g. SLE, atopic dermatitis, erythroderma and pustulosis palmaris et plantaris) for assessment of complement activation.

The complement is one of the major effector system in the process of inflammation. Complement activation has been shown to occur in inflammatory dermatoses such as systemic lupus erythematosus, atopic dermatitis, erythroderma of unknown origin, and pustulosis palmaris et plantaris by the elevated blood levels of complement fragments. To clarify the complement activation, especially the alternative pathway involvement, we have measured the concentrations of classical pathway-specific C4d and alternative pathway-derived Bb in the plasma of patients with these inflammatory disorders at a mild to

[A case of multiple mononeuropathy associated with pustulosis palmaris et plantaris (PPP)].

A 41-year-old man with PPP since in 1982 was admitted in May, 1987, because of the progressive asymmetric sensory disturbance in the hands and feet over 4 months, accompanied by an exacerbation of PPP. On admission, eruptions of PPP were observed in the palms and soles. Asymmetric and mildly decreased sensations of touch and pain were present in the distal part of the four extremities as well as in his trunk, accompanied by paresthesia and dysesthesia. Mild to moderate weakness was noted in the hand muscles, and slight muscular atrophy was present in the right lower leg. A work-up for collagen

Oral cyclosporin A is effective in clearing persistent pustulosis palmaris et plantaris.

In a prospective open study, seven patients with persistent pustulosis palmaris et plantaris were treated with oral Cyclosporin A (CsA). Clinical efficacy was assessed on a semi-quantitative 0-4 point scale for erythema, desquamation, induration and pustulation. CsA controlled skin lesions in doses ranging from 1.1 to 6.1 mg/kg body weight/day. Clinical side effects included renal impairment, nausea and tiredness. Rapid recurrence of the skin lesions was observed on withdrawal or insufficient treatment with the drug.

[Investigation of the bacterial flora in the tonsillar lacunae and serum levels of streptococcal antigen-specific antibodies in patients with pustulosis palmaris et plantaris].

The purpose of this study was to clarify the relationship between bacterial flora in tonsillar lacunae and antistreptococcal antibody in sera of the patients with pustulosis palmaris et plantaris (PPP). Since the pustules develops or worsens after tonsillitis in many patients with PPP, a pathogenic role of tonsillar bacteria is suggested, but no significant association has been proven so far. In this study, the aerobes in the tonsillar lacunae were identified and quantitated in 15 adult patients with PPP and 9 adult patients with chronic tonsillitis (CT). In addition, serum streptococcal antig

Sternocostoclavicular hyperostosis: two cases with differing dermatologic syndromes.

Sternocostoclavicular hyperostosis is a rare rheumatic condition characterized by ossification and erosion of the clavicle and the first rib, that has been shown to be associated with pustular skin lesions. We present 2 cases, one of which had features of pustulosis palmaris et plantaris and the other dissecting cellulitis of the scalp. Although the dermatologic manifestations differ, both cases have rheumatologic and roentgenographic features diagnostic of sternocostoclavicular hyperostosis.

Tonsillectomy-induced changes in anti-keratin antibodies in patients with pustulosis palmaris et plantaris: a clinical correlation.

Titers of anti-keratin antibodies (AKA) involving IgG and IgM fractions were determined by a solid-phase enzyme-linked immunsorbent assay (ELISA) and were found to be significantly higher on average in patients with pustulosis palmaris et plantaris (PPP) than in normal controls. Positive rates of IgG-AKA and IgM-AKA in sera of PPP patients were 50% and 66%, respectively. Following tonsillectomies, patients with high IgM-AKA titers that decreased after surgery were found to benefit by a marked improvement in their skin lesions. There was a close relationship between the degree of this improveme

Pre-operative diagnosis for dermatoses due to tonsillar focal infections: recent views.

The recent views on the pre-operative diagnosis for dermatoses possibly due to tonsillar focal infections were presented as follows: 1) the range of dermatoses which may be related to tonsillar focal infection and may be indications for tonsillectomy, was introduced based on a questionnaire sent to departments of dermatology and otorhinolaryngology throughout Japan; 2) provocation test using direct stimulation with ultra-short waves and inhibition test with Impletol for the pre-operative diagnosis in patients with Pustulosis palmaris et plantaris, which appears to have a relatively high preval

An elevated level of autoantibodies against 48- to 50-kd keratins in the serum of patients with psoriasis.

Studies on the anti-keratin intermediate filament autoantibodies (anti-KIF-Abs) in sera from psoriasis (Pso) patients were performed by immunoblotting and enzyme-linked immunosorbent assay (ELISA). According to their reactivities against keratin subunits (50, 56.5, 58, and 63-68 kd), sera were divided into four groups. However, no significant differences in these reactive patterns were found between healthy volunteers and Pso patients. In the second experiment, anti-KIF-Abs in sera from Pso patients, pustulosis palmaris et plantaris (PPP) patients, atopic dermatitis (AD) patients, systemic lup

Reactive fibromatosis in pustulotic arthro-osteitis.

Sterno-costo-hyperostosis is an inflammatory rheumatic disease which is associated to pustulosis palmaris et plantaris. The origin of this disorder is unknown. We described the appearance as a "reactive" fibromatosis in connection with a hyperostosis of a clavicle, which could be demonstrated in a biopsy-probe and by NMR-technique.

Three cases of inter-sterno-costo-clavicular ossification associated with tonsillar infection.

Three cases of inter-sterno-costo-clavicular ossification (ISCCO) associated with chronic tonsillitis are reported. Two cases had additional symptoms of pustulosis palmaris et plantaris (PPP). All the patients underwent tonsillectomy. The histological changes of the palatine tonsils of case 1 and case 2 were characterized by so-called focal small lacunar ulcers and periarteriolar fibrosis (onion-skin lesion). Some multinucleated giant cells were observed in case 2. Following tonsillectomy, symptoms in the clavicular region and the PPP improved. It is possible to consider that ISCCO is caused b

[Electric current around dental metals as a factor producing allergenic metal ions in the oral cavity].

Allergy to dental metal alloys has been reported to occasionally cause dermatitis, stomatitis, lichen planus and pustulosis palmaris et plantaris. According to Faraday's law of electrolysis, when electric current flows into an anode, cationic metal ions dissolve in parallel with the amount of the electric current. Therefore, when patients hypersensitive to metals have suffered from the above mentioned persistent dermatoses, measurement of voltages and electric currents around the dental alloys of the patient's oral cavities was deemed necessary, in order to investigate whether or not such dent

Immunological studies on the relation between tonsil and pustulosis palmaris et plantaris.

In order to clarify the relationship between the tonsils and pustulosis palmaris et plantaris (PPP), immunological investigations were performed focusing on keratin and antikeratin antibody. As materials, the tonsils, plantar skins and peripheral blood from the patients with PPP undergoing tonsillectomy were used. The results were summarized as follows: 1) Common keratin polypeptides reacting with monoclonal anti-keratin antibody were found both in the tonsillar epithelium and in the plantar skin. 2) High molecular weight keratin polypeptides were predominant in the tonsillar epithelium of the
