Though it is well known that pustulosis palmaris et plantaris (PPP) is one of skin diseases caused by focal infection of the tonsils, its etiology remains unknown. One hundred and twenty-four cases of pustulosis palmaris et plantaris (PPP) were studied by questionnaire survey to evaluate the effectiveness of tonsillectomy as treatment. The relationship between pustulosis palmaris et plantaris (PPP) and immune complexes was studied in 17 patients who underwent tonsillectomy. By means of the Clq-binding test, immune complexes were detected in a considerable number of sera from these patients. A review of the clinical and histological features of pustulosis palmaris et plantaris (PPP) was undertaken. The morphologic development of PPP is biphasic: the first phase consists of an eczematous reaction, and the second phase is the pustulation of eczematous vesicles. PPP may be aggravated by infections or non-infectious inflammatory processes elsewhere in the body. An abnormal glucose tolerance test was found in 55 (66.3%) of 83 patients with PPP. The deranged carbohydrate metabolism may be partly responsible for the increased susceptibility to focal infections in patients with PPP.

The frequency of locus A and B antigens in 39 patients with pustulosis palmo-plantaris and in 16 patients with psoriasis vulgaris was determined in comparison with a control group of 394 healthy subjects. The data obtained in the psoriasis vulgaris group confirmed the literature data (increased frequency of B 13 and B 17 antigens). In the pustulosis palmo-plantaris group only the patients with the histopathologically confirmed psoriasis pustulosa showed a statistically significant increase of B 12 antigen which has not been reported so far.

The effect of oral doses of colchicine on pustule formation was studied in 32 patients with pustulosis palmaris et plantaris without associated psoriasis. Oral administration of colchicine, 1 to 2 mg daily, was started during periods of disease exacerbation, and the dosage was gradually decreased to 0.5 to 1 mg daily in responsive patients. Thirteen patients showed complete clearing of pustulation, and 14 patients showed a remarkable reduction in pustule formation after two to eight weeks of treatment. Titres of anti-stratum corneum (SC) antibodies, determined by an indirect immunofluorescence (IF) technique, were significantly higher on average in pustulosis palmaris et plantaris (PPP) and pompholyx than in normal controls. The occurrence of multinucleated epidermal giant cells (MEGCs) with more than three clumping nuclei has been considered an episodic and exceptional phenomenon in inflammatory skin diseases. Arthro-osteitis at the anterior chest wall was found in 12 (9.4%) out of 128 consecutive patients with pustulosis palmaris et plantaris. This finding indicates that the concomitance of arthro-osteitis with PPP is not incidental but is based on some common aetiological factor. We propose a term ;pustulotic arthro-osteitis' for this condition.
