Spesolimab, the first-in-class anti-IL-36R antibody: From bench to clinic.

Inflammatory diseases that are driven by several pro-inflammatory cytokines has resulted in in the development of targeted therapies across different disease settings. Interleukin (IL)-36 cytokines have been implicated in several inflammatory diseases. In this review we describe the scientific evidence surrounding the use of the IL-36 receptor (IL-36R)-targeting antibody, spesolimab, in IL-36-mediated skin diseases: generalized pustular psoriasis (GPP), palmoplantar pustulosis (PPP), hidradenitis suppurativa, and Netherton syndrome (NS). Spesolimab, a high affinity, specific, humanized, antago

Asia-Pacific consensus recommendations on the management of generalized pustular psoriasis.

Generalized pustular psoriasis (GPP) is a rare, chronic, heterogeneous, and potentially life-threatening disease characterized by primary, sterile, and macroscopically visible pustules with or without systemic symptoms. There are ethnic differences in the genetic mutations associated with GPP that might affect the clinical manifestations and treatment responses. Currently, there is limited evidence from the patient population in the Asia-Pacific (APAC) region, resulting in a general paucity of information on the effective management of patients with GPP in this region. This modified Delphi pan

Biologics for generalized pustular psoriasis: a systematic review and single-arm meta-analysis.

INTRODUCTION: Generalized pustular psoriasis (GPP) is a rare and potentially life-threatening auto-inflammatory disease. Currently, there are no consensus-based guidelines or universally accepted treatments. Biologics represent a potential therapeutic option. This study systematically assessed the efficacy and safety of biologics in GPP. METHODS: Relevant studies from three databases were systematically searched until June 28, 2024. Statistical information, including the single-arm proportion rate of the outcomes and 95% confidence intervals (CIs), was analyzed to determine treatment effects.

Successful Treatment of Pediatric Generalized Pustular Psoriasis (GPP) with Spesolimab: 5 Case Reports and Evaluations of Circulating IL-36 Levels.

BACKGROUND: Generalized pustular psoriasis (GPP) is a rare, severe, and potentially life-threatening inflammatory cutaneous disease. IL-36 is a key treatment target in GPP. Spesolimab, a humanized monoclonal antibody of the IL-36 receptor, has demonstrated a good efficacy and a favorable safety profile in adults with GPP. However, data on its use in children are scarce. METHODS: We treated patients aged 4-12 years with GPP with a single dose of spesolimab. The Generalized Pustular Psoriasis Physician Global Assessment (GPPGA) total score, GPPGA pustulation sub-score, Generalized Pustular Psor

Exploring the Chronic Nature of Generalized Pustular Psoriasis [Podcast].

Generalized pustular psoriasis (GPP) is a rare, chronic, inflammatory skin disease characterized by persistent symptoms and sudden flares of painful, sterile pustules, and may be accompanied by systemic inflammation. Ongoing symptoms of GPP can have a serious impact on patient quality of life, morbidity, and mortality, and severe flares may be life-threatening if left untreated. Guidelines have been developed for the treatment of GPP flares; however, health care professionals and patients are lacking guidance on the management of long-term, persistent symptoms of GPP. Spesolimab is the only FD

Resolution of Erythema Annulare Centrifugum With Lung Cancer Drug Therapy: A Case Report.

Tumor-associated erythema annulare centrifugum is commonly observed in leukemia and lymphoma. In this case, we reported erythema annulare centrifugum associated with lung cancer that resolved following targeted drug therapy. A 71-year-old man with psoriasis visited our hospital with erythema annulare and pruritus. Initially, he was prescribed cyclosporine at a dose of 2.5 mg/kg suspecting a worsening of his psoriasis; however, this treatment was ineffective. Signs of paraneoplastic skin syndrome were observed, leading to a diagnosis of epidermal growth factor receptor mutation-positive lung ad

Innovations in Psoriasis.

Despite numerous effective biologics for treating psoriasis, new treatments continue to be investigated due to an unmet need for certain patient populations. This review discusses therapies that were recently Food and Drug Administration (FDA)-approved for treating psoriasis, including the topical agents tapinarof and roflumilast, deucravacitinib, an oral small molecule that selectively inhibits tyrosine kinase 2, and spesolimab, a monoclonal antibody inhibiting interleukin-36 that became the first FDA-approved treatment for generalized pustular psoriasis flares. Other therapies are in the pip

Characteristics of Patients with Generalized Pustular Psoriasis: A Report of the Polish Generalized Pustular Psoriasis Group.

INTRODUCTION: Data on generalized pustular psoriasis (GPP) in Poland are limited. Therefore, this study aimed to evaluate the demographic and clinical characteristics of Polish patients with GPP. In addition, the effect of the frequency of disease flares and the concomitant presence of psoriasis vulgaris (PV) on patient characteristics was evaluated. METHODS: In this retrospective study, medical records of patients hospitalized for GPP in dermatology departments of the Polish Generalized Pustular Psoriasis Group were analyzed. RESULTS: A total of 90 patients from 14 dermatology centers were

Updated genetic background of generalized pustular psoriasis as an autoinflammatory keratinization disease.

Generalized pustular psoriasis (GPP) is a severe autoinflammatory keratinization disease (AiKD) characterized by acute flares of widespread sterile pustules and high fever. GPP is potentially life-threatening. Recently clarified genetic predisposing factors for GPP suggest that the excessive activation of innate immune pathways in the skin, including of interleukin (IL)-1 and IL-36 signaling, plays a significant role in the GPP pathogenesis. IL36RN, CARD14, AP1S3, MPO, SERPINA3, BTN3A3, and MEFV have been identified as GPP-related genes. The pathogenesis of GPP provoked by variants in these se

Efficacy of secukinumab in Takayasu arteritis with myocardial infarction complicated with generalized pustular psoriasis: A case report.

RATIONALE: Takayasu arteritis (TAK) is an autoimmune disease that causes chronic inflammation targeting the aortic wall. Since many patients are resistant to steroid treatment, multiple immunosuppressants or interleukin-6 (IL-6) suppression therapy have served as treatment alternatives. However, there are very few reports on the effectiveness of biologics against inflammatory cytokines upstream of IL-6. PATIENT CONCERNS: We present a case of TAK in a 51-year-old female presenting with a myocardial infarction. She had persistent carotid wall thickening despite glucocorticoid (GC) therapy, and

Generalized pustular psoriasis: immunological mechanisms, genetics, and emerging therapeutics.

Generalized pustular psoriasis (GPP) is a rare human autoinflammatory disorder with life-threatening systemic effects. Keratinocyte-derived interleukin (IL)-36 signaling has been identified as a key mediator of immune response in the skin of affected individuals. Recognition of various mutations along the IL-36 axis and the downstream nuclear transcription factor κB (NF-κB) signaling have established GPP as genetically, immunologically, and histopathologically distinct and amenable to immunomodulation, which is epitomized by the recent success of IL-36 antagonism. This review covers recent dis

Zinc Supplementation for Acrodermatitis Enteropathica Overlapped with Psoriasiform Lesions.

Acrodermatitis enteropathica (AE) can be caused by inherited or acquired zinc deficiency, among which site-specific skin lesions or even psoriasiform skin manifestations are present. Few cases exist in the literature involving the diagnosis and treatment of AE overlapped with psoriasiform lesions. In this case, we reported a teenage boy presented characteristic site-specific skin lesions of AE with low serum zinc level, subsequently progressed into generalized pustular psoriasiform manifestations under a genetic background, while a rapid recovery was observed after monotherapy of zinc suppleme

Psychosocial burden of patients with mycosis fungoides and Sézary syndrome compared to other dermatological patients. Pilot study.

INTRODUCTION: Cutaneous T-cell lymphoma (CTCL), including mycosis fungoides (MF) and Sézary syndrome (SS), significantly affects patients' quality of life (QoL) due to its chronic and progressive nature. Both conditions often lead to diagnostic delays and have a profound impact on daily activities and emotional well-being. AIM: This study aimed to evaluate the QoL impairment in patients with MF and SS compared to individuals with psoriasis vulgaris and other dermatological diseases. Additionally, the study explored the potential role of high-frequency ultrasound (HFUS) in monitoring CTCL. MA

Eruption of Generalized Pustular Psoriasis Following Initiation of Adalimumab for Hidradenitis Suppurativa: A Case Report.

Rarely, tumor necrosis factor (TNF)-α inhibitors can paradoxically induce eruptions of psoriasis with generalized pustular psoriasis being among the least common presentations. We report a patient who presented with a generalized pustular eruption following adalimumab therapy for hidradenitis suppurativa (HS). The diagnosis of generalized pustular psoriasis was confirmed with a biopsy showing neutrophilic spongiosis and intraepidermal pustulosis. We discuss the role of tumor necrosis factor in psoriasis while describing possible mechanisms for this paradoxical reaction. Additionally, we discus
