Temporomandibular disorders in women with psoriatic arthritis by history of psoriasis: a cross-sectional study.

Temporomandibular joint involvement in psoriatic arthritis may result in impaired jaw function and reduced oral health-related quality of life. Patients with PsA have generally been analyzed as a single group regardless of psoriasis history, despite a reported association between psoriasis and TMJ involvement. To assess TMD symptoms, patient-reported outcome measures, and self-reported beneficial effects of systemic therapies and TMD-related treatment modalities in women with psoriatic arthritis, stratified by psoriasis history. A digital questionnaire was sent to members aged ≥ 18 years of th

Quercetin confers protection against psoriasis by inhibiting AKR1B10-mediated glycolytic reprogramming in keratinocytes.

The anti-psoriatic effect of quercetin has been observed. However, the involved mechanism remains poorly explored. Hence, the study investigated the therapeutic effects of quercetin against psoriasis and elucidated its underlying molecular mechanisms, with a focus on AKR1B10-mediated glycolytic reprogramming. An imiquimod (IMQ)-induced mouse model and an interleukin-17A (IL-17A)-stimulated keratinocyte model were established as psoriasis-like models. Therapeutic effects of quercetin on psoriasis-like lesions were evaluated by gross lesion observation, PASI scoring, histopathological staining,

The active immune role of keratinocytes: from passive targets to key hubs in psoriatic inflammation.

Psoriasis is a chronic immune-mediated inflammatory skin disease historically viewed as a disorder of keratinocyte hyperplasia, with subsequent recognition of central immune cell involvement. However, accumulating multi-omics and functional genomic evidence has reshaped this paradigm, positioning keratinocytes not merely as passive targets of immune attack but as active participants within a bidirectional epithelial-immune network that drives inflammatory initiation, maintenance, and amplification. This critical narrative review synthesizes the molecular mechanisms by which keratinocytes integ

From Empiricism to Precision: Century of Paradigm Shifts in Medical Dermatology.

Over the past century, advances in scientific understanding and therapeutic development have fundamentally reshaped dermatology. Diseases once managed with empiric, toxic, or largely ineffective treatments can now be approached with therapies that target specific pathways, achieve sustained disease control, and, in some cases, alter prognosis. Herein, we reviewed landmark therapeutic advances across infectious, inflammatory, autoimmune, vascular, and oncologic dermatology, focusing on interventions that changed the standard of care or substantially improved patient outcomes. Transformative adv

Real-world adherence to biologic therapies targeting interleukin 17, interleukin 23 and ustekinumab in moderate-to-severe psoriasis: A multicenter cohort study.

OBJECTIVES: Biologic therapies targeting interleukin IL-17, IL-23 and IL-12/23 have substantially improved outcomes in patients with moderate-to-severe psoriasis. In real-world settings, treatment effectiveness depends not only on drug efficacy but also on patient adherence, which is essential for the appropriate interpretation of treatment persistence. The objectives of the present study were to evaluate adherence to biologic therapy with IL-17 inhibitors, IL-23 inhibitors and IL-12/23 inhibitors in patients with moderate-to-severe psoriasis in real-world clinical practice and to identify fac

Refractory Psoriatic Lesions in Japanese Patients Before and After Switching Biologics.

Biologics can improve disease control in patients with psoriasis vulgaris (PsO) and psoriatic arthritis (PsA); however, refractory regions may persist despite biologic therapy. This study aimed to evaluate refractory anatomical regions in patients with PsO and PsA who underwent biologic switching. We retrospectively analyzed 71 biologic-switching episodes from 50 patients treated at Jichi Medical University Hospital between January 1, 2010 and December 31, 2019. Patients who underwent more than one biologic switch contributed multiple switching episodes. Refractory regions were assessed across

Safety signals of tocilizumab in pediatric patients: A real-world pharmacovigilance analysis based on the FAERS database with network pharmacology insights.

Tocilizumab, a humanized monoclonal antibody targeting the interleukin-6 receptor, is widely used in immune-mediated diseases, yet its real-world safety in pediatric populations remains insufficiently characterized. This retrospective pharmacovigilance study analyzed adverse event (AE) reports from the U.S. Food and Drug Administration Adverse Event Reporting System (FAERS) database (2010-2025), focusing on pediatric cases where tocilizumab was the primary suspect drug. After data cleaning, 1,485 reports were included. Four classical disproportionality methods - reporting odds ratio (ROR), pro

Discordance in Treatment Response Assessment Between Clinicians and Patients With Skin Chronic Graft-vs-Host Disease.

IMPORTANCE: Clinician-reported and patient-reported outcomes are critical measures of therapeutic efficacy in cutaneous chronic graft-vs-host disease (cGVHD) but are not always correlated. Discordance in treatment response between clinicians and patients hinders interpretation of outcomes in clinical trials and complicates therapeutic decision-making in clinical practice. OBJECTIVE: To identify factors associated with discordance in clinician-reported and patient-reported treatment response assessments and to evaluate the association of clinician-reported and patient-reported responses with s

Smoking Status and Psoriasis Risk: An Updated Systematic Review and Meta-Analysis.

INTRODUCTION: Psoriasis is a chronic immune-mediated inflammatory skin disease affecting 1-5% of the global population. Smoking has long been implicated as a potential environmental risk factor; however, previous studies and meta-analyses have reported heterogeneous and sometimes inconsistent findings. Since the publication of earlier reviews, several large population-based studies have become available, warranting an updated and more comprehensive synthesis of the evidence. METHODS: A systematic search of PubMed, Embase, and the Cochrane Library was conducted from database inception to Augus

Eczema-Psoriasis Overlap: An eDelphi Consensus on Clinical Presentation and Therapeutic Management Among Italian Experts.

INTRODUCTION: Atopic dermatitis and psoriasis (PsO) are common chronic inflammatory skin diseases traditionally considered distinct, driven by Th2 and Th1/Th17 pathways, respectively. However, evidence reveals that some patients may present with both eczematous and psoriatic lesions, either simultaneously or sequentially. This study's objective was to establish an Italian consensus on the clinical definition and management of eczema-psoriasis overlap. METHODS: A steering committee of eight dermatologists identified three key areas: problem definition, clinical presentation and therapeutic man

Diagnostic Value of Skin Biopsy in Fatal Histoplasmosis-Associated Hemophagocytic Lymphohistiocytosis: A Case Report.

INTRODUCTION: Hemophagocytic lymphohistiocytosis (HLH) is a rare, life-threatening hyperinflammatory syndrome most commonly triggered in adults by infection, malignancy, or autoimmune disease. Disseminated histoplasmosis is an uncommon but recognized infectious trigger, particularly in immunosuppressed patients. Its cutaneous manifestations may be subtle and nonspecific but can provide accessible tissue for diagnostic evaluation. CASE REPORT: A 41-year-old man with psoriasis treated with adalimumab presented with 5 months of progressive constitutional and abdominal symptoms followed by fever,

Clinical Implications of Interleukin-17 as a Hepato-Dermal Link in Patients With Psoriasis and Concomitant Steatotic Liver Disease.

Psoriasis is increasingly recognized as a systemic inflammatory disease with impacts beyond the skin and joints. Metabolic dysfunction-associated steatotic liver disease (MASLD) and excessive alcohol consumption are common in the psoriasis population, affecting up to two-thirds of patients. Interleukin-17 (IL-17) has been identified as a potential key pro-inflammatory link between psoriasis and MASLD. IL-17 levels have been positively correlated with progression of MASLD to more advanced forms of liver disease, including steatohepatitis, cirrhosis and hepatocellular carcinoma. Given the high p

Editor's Highlights-December 2026.

The effects of climate change, unequal access to biologic therapies, and the limited representation of Indigenous communities and diverse skin tones in educational materials underscore an important point: dermatology does not operate independently of social and environmental factors. The five papers in this issue not only identify these gaps but also call for specific measures to strengthen climate-resilient preparedness, expand safety-net services, and promote truly inclusive education. Equitable care is no longer a nice-to-have option; it is a fundamental standard of clinical quality.

Hydrocele stone presentation and crystallographic analysis: a unique case report.

INTRODUCTION AND IMPORTANCE: Hydroceles are typically benign fluid collections around the testes. Rarely, scrotal calculi, or "hydrocele stones," form within the tunica vaginalis, possibly due to chronic inflammation, torsion of the appendix testis, minor trauma, or other mechanisms. These stones are infrequently reported, and cases with detailed crystallographic analysis are exceptionally rare. CASE PRESENTATION: We present a 43-year-old male with bilateral hydroceles, a history of psoriasis and dyslipidemia, and prior right inguinal hernia repair. Ultrasound revealed a moderate right hydroc

Dysfunctional tight junctions in psoriasis pathogenesis: mechanisms and therapeutic implications.

Tight junctions (TJs) are essential intercellular adhesion structures in epidermal keratinocytes that govern the multicomponent epidermal barrier (paracellular permeability, keratinocyte polarity, and barrier homeostasis). Their disruption and dysregulation are early, consistent, and functionally important pathological features in psoriasis-a chronic inflammatory skin disorder driven by epidermal hyperproliferation, aberrant differentiation, and sustained immune activation. In psoriatic lesions, TJ proteins exhibit consistent abnormal expression, ectopic localization, and impaired assembly, wh

Delayed Drug Hypersensitivity: Mechanistic Advances and Emerging Therapies.

PURPOSE OF REVIEW: Delayed drug hypersensitivity reactions span mild cutaneous eruptions, severe cutaneous adverse reactions (SCARs), and internal-organ injury. We review how pharmacogenomic, single-cell, and spatial methods have shown that these syndromes are driven by shared effector mechanisms, and how identifying which one predominates opens the way to targeted treatment. RECENT FINDINGS: HLA associations are necessary but not sufficient to explain who reacts. Susceptibility is increasingly attributed to a pre-existing, often virus-shaped T-cell repertoire and to where tissue-resident mem

The overlooked surface: cutaneous manifestations of inflammatory bowel disease.

Inflammatory bowel disease (IBD) is characterized by a chronic, idiopathic inflammation of the intestinal tract. The primary types include ulcerative colitis (UC), which affects the colon and rectum with continuous mucosal and submucosal lesions solely and Crohn's disease (CD), which may involve any segment of the gastrointestinal tract with discontinuous lesions. Among extraintestinal manifestations (EIMs), the skin is most frequently involved. Erythema nodosum (EN) and pyoderma gangrenosum (PG) are identified as the most prevalent cutaneous presentations. This review explores the underlying

Vitamin D3 Inhibits Expression of Interleukin-6 and Calcitonin Gene-Related Peptide by Transformed Dorsal Root Ganglion Neurons.

Interleukin-6 (IL-6) and the neuropeptide calcitonin gene-related peptide (CGRP) promote the generation of T helper 17 (Th17) cells. Th17-type immunity is involved in several autoimmune disorders, including psoriasis. Also, there is evidence that sensory nerves play a role in Th17-type immunity, including in psoriasis. Vitamin D (VD) blood levels vary inversely with the severity of psoriasis, and topical VD analogs are effective treatments for this disease. We have now asked whether 25-dihydroxyvitamin D3 [25(OH)D3] and the more potent, active analog 1α,25-dihydroxyvitamin D3 [1α,25(OH)2D3] in
