Host immune dysregulation associated with high plasma EBV-DNA load in children and its potential association with secondary infection.

High plasma Epstein-Barr virus (EBV)-DNA load may be associated with host immune dysregulation and secondary infection in children, although the underlying mechanisms remain uncertain. This study analyzed peripheral blood samples from 34 children with high plasma EBV-DNA load and 31 children with low plasma EBV-DNA load using transcriptomic analysis, WGCNA, CIBERSORT-based deconvolution, LASSO regression, Random Forest, qPCR, ELISA, and flow cytometry to explore immune features associated with EBV-DNA load and secondary-infection status. A total of 459 differentially expressed genes were ident

Paradoxical Cutaneous Reaction: Secukinumab-Associated PLEVA in Palmoplantar Pustulosis.

Palmoplantar pustulosis (PPP) is a chronic inflammatory dermatosis often treated with IL-17 inhibitors like secukinumab. Paradoxical cutaneous reactions to such biologics, though rare, are increasingly recognized. We report a case of a 25-year-old woman with PPP who developed a biopsy-confirmed acute pityriasis lichenoides et varioliformis acuta (PLEVA) eruption following six weeks of secukinumab therapy. Upon switching treatment to tofacitinib, both the PPP and the paradoxical PLEVA showed significant improvement. This case highlights PLEVA as a potential paradoxical reaction to IL-17 inhibit

Febrile ulceronecrotic Mucha-Habermann disease: a case report and literature review in preschool-aged children.

Febrile Ulceronecrotic Mucha- Habermann Disease (FUMHD) is a rare and severe specialized type of Pityriasis Lichenoides Et Varioliformis Acuta (PLEVA). It is characterized by rapid onset of painful necrotic skin lesions and systemic symptoms. The diagnosis of FUMHD is complex and is easily misdiagnosed in the early stages. Despite the increasing number of reports of FUMHD in recent years, it is still uncommon in preschool (3-6 years) children. We here report a 3-year-9-month-old girl of FUMHD from China, cured through combined antibiotics, intravenous immunoglobulin, methotrexate, and predniso

A fatal pediatric case of febrile ulceronecrotic Mucha-Habermann disease: diagnostic and therapeutic challenges: a case report.

BACKGROUND: Febrile ulceronecrotic Mucha-Habermann Disease (FUMHD) is a rare and severe form of pityriasis lichenoides et varioliformis acuta. It presents rapidly with ulcerative and necrotic lesions, fever, and systemic symptoms. This condition is associated with significant morbidity and can be extremely dangerous. The exact cause of FUMHD is not well understood, but hypersensitivity reactions may trigger it. CASE PRESENTATION: We describe a 15-month-old Iranian boy who developed rapidly progressing ulceronecrotic skin lesions accompanied by high-grade fever and systemic involvement. A skin

Pityriasis Lichenoides et Varioliformis Acuta: A Case of Iatrogenic Cushing's Syndrome Induced by Topical Corticosteroids.

The use of topical corticosteroids plays a central role in the management of inflammatory skin disorders in dermatology, owing to their cost-effectiveness and early symptom resolution. However, inappropriate or prolonged use, particularly of high-potency agents, may lead to local and systemic adverse effects, including the suppression of the hypothalamic-pituitary-adrenal axis. Although systemic complications are likely underreported in the literature, the pediatric population is particularly prone to experiencing them. We present the case of an eight-year-old boy with skin lesions compatible

Pityriasis Lichenoides et Varioliformis Acuta: Diagnostic Enigma Unveiled with Clinicopathologic Insights into a Rare Dermatologic Entity.

Pityriasis Lichenoides et varioliformis acuta (PLEVA) is a rare, idiopathic, inflammatory dermatosis representing the acute end of the pityriasis lichenoides spectrum. It usually affects children and young adults, whereas occurrence in older individuals is distinctly uncommon. We report a 53-year-old male laborer who presented with multiple, discrete, erythematous, itchy papuloplaque lesions over the back and neck for 8 days. There were no systemic symptoms. Clinical differentials included pityriasis rosea and PLEVA. Histopathological examination of a punch biopsy revealed mild acanthosis, foc

A retrospective study of clinical and histopathological characteristics in patients with pityriasis lichenoides: A comparative analysis of pediatric and adult cases.

Pityriasis lichenoides (PL) is a rare inflammatory skin disorder of unknown etiology, comprising a spectrum of 2 clinical variants: pityriasis lichenoides chronica and pityriasis lichenoides et varioliformis acuta. Due to its rarity, data on its clinical and histopathological features (particularly age-related differences) remain limited. This retrospective, cross-sectional study included patients diagnosed with PL between January 2019 and January 2024 at a tertiary dermatology clinic. Demographic data, clinical presentation, histopathological subtype, treatment modalities, and disease outcome

Pediatric Sweet syndrome initially misdiagnosed as immunoglobulin a vasculitis: the importance of repeat biopsy.

Sweet syndrome is characterized by tender erythematous plaques, histologically presenting as marked neutrophilic infiltration in the dermis. In general, this syndrome manifests in middle-aged adults, and given its extreme rarity in the pediatric population, it is rarely considered by pediatricians in routine clinical practice, which often leads to misdiagnosis. A 1-year-old girl had developed palpable purpura subsequent to experiencing upper respiratory symptoms, and at a previous hospital, she was preliminarily diagnosed with immunoglobulin A (IgA) vasculitis in the absence of skin biopsy evi

Skin Biopsies in Practice: Insights from a Pediatric Dermatopathology Study in a Greek Tertiary Center.

Pediatric dermatologic conditions often present with overlapping clinical features, complicating accurate diagnosis and potentially delaying appropriate treatment. This retrospective study aimed to evaluate the diagnostic utility of pediatric skin biopsies by assessing clinicopathologic concordance and identifying cases in which histopathologic analysis led to diagnostic reclassification. Medical records of 140 children (≤14 years) who underwent skin biopsy at a tertiary-care center in Greece between February 2021 and November 2024 were retrospectively reviewed. Most biopsies (126/140, 90.0%)

Dupilumab's Successful Journey in Pityriasis Lichenoides Chronica Followed by the Intriguing Reemergence of Vitiligo after Monobenzone Depigmentation: A Case Report.

INTRODUCTION: Pityriasis lichenoides (PL) is an inflammatory skin disorder encompassing both acute (PLEVA) and chronic (PLC) subtypes, which share overlapping clinical and histopathological features. Standard treatment modalities include oral antibiotics, phototherapy, and immunosuppressive agents. Dupilumab, an IL-4Rα antagonist approved for atopic dermatitis, has been used off-label for various inflammatory skin conditions. However, its role in PLC management remains poorly documented. CASE PRESENTATION: We report a case of PLC successfully treated with dupilumab. The patient had a history

Incomplete Susac Syndrome With Recurrent Retinal Occlusions and Vertigo: A Case of Early Immunosuppressive Success.

Susac syndrome (SS) is a rare autoimmune microangiopathy that targets the brain, retina, and inner ear. Its variable and often incomplete clinical presentation frequently leads to misdiagnosis, increasing the risk of morbidity. We report the case of a 53-year-old Caucasian woman with a history of pityriasis lichenoides et varioliformis acuta (PLEVA) and severe myopia. She experienced recurrent branch retinal artery occlusion (BRAO) and retinal vein occlusion, following an episode of intense vertigo. Notably, her initial vascular occlusion occurred prior to the onset of vertigo. A comprehensive
