Varicella Zoster Virus-Associated Meningoradiculitis in an Elderly Patient: A Case Report.

INTRODUCTION: Motor complication in immunocompetent individuals with varicella zoster virus (VZV) reactivation is uncommon and may mimic a compressive spinal disorder. CASE PRESENTATION: A 77-year-old immunocompetent woman presented with an acute left foot drop preceded by coryzal symptoms and lower back pain. A vesicular rash developed over the left foot one week after the weakness began. Examination showed a predominantly severe left ankle dorsiflexion weakness with reduced sensation over the L5 and S1 dermatomes. A noncontrast magnetic resonance imaging (MRI) of the lumbosacral spine showe

Occurrence of Sensorineural Hearing Loss in Viral Meningitis and Encephalitis.

Hearing loss in viral neuroinfections is poorly investigated. The present study aimed to clarify if hearing loss can develop in viral meningitis and encephalitis. An OAE (oto-acoustic emission) was measured in adult patients with viral meningitis and encephalitis on the day of admission and 30-60 days after discharge. Frequencies were categorized as Low (1, 1.5, 2 kHz), Mid (3, 4, 5 kHz), Mid-high (6, 7, 8 kHz), and High (9, 10 kHz). An OAE was measured in 41 patients with viral meningitis and 8 patients with encephalitis. Six patients with viral meningitis (15%) experienced a decrease of > 5

Viral triggers of Guillain-Barré syndrome: implications for laboratory diagnostics.

Guillain-Barré syndrome (GBS) is a leading cause of acquired acute neuromuscular paralysis and is an immune-mediated disorder of the peripheral nervous system (PNS). Accumulating evidence indi-cates that GBS is often preceded by infectious events that trigger autoimmune responses through mechanisms such as molecular mimicry and autoantibody production. Campylobacter jejuni remains the most common bacterial agent, but viral pathogens also contribute to the development of GBS. This review summarizes the current evidence regarding viruses as triggers of GBS, including herpes-viruses, hepatotropic

Ruptured intracranial dermoid cyst as an incidental finding: Case report.

Intracranial dermoid cysts are rare, benign congenital lesions originating from ectodermal remnants. Though typically asymptomatic, their rupture, although infrequent, can lead to significant neurological complications, such as chemical meningitis, vasospasm, and hydrocephalus. Imaging with computed tomography (CT) and magnetic resonance imaging (MRI) plays a crucial role in diagnosis, providing characteristic features related to the high lipid content of these lesions. An 86-year-old female presented with a 3-day history of headache, nausea, vomiting, and facial cellulitis after an arthropod

Multiple Sclerosis and Viruses: « Liaisons Dangereuses ».

Multiple sclerosis (MS) is a complex, multifactorial neurodegenerative disease with an aetiology that is still partially elusive. Several risk factors are associated with MS development including genetic predisposition and various environmental stressors; however, recently, viral infections have gained significant attention as a crucial environmental trigger. Despite the well-established link between Epstein-Barr virus (EBV) and MS susceptibility, a single-pathogen model fails to clearly characterise the molecular cascades driving the disease, suggesting that the transition from infection to a

A Case of Urinary Tract Infection Resulting From Voiding Dysfunction Caused by Sacral Herpes Zoster Involving the Left Buttock.

Herpes zoster-associated voiding dysfunction is an uncommon complication but may occur when the sacral dermatomes are involved. In immunocompromised patients, including kidney transplant recipients receiving immunosuppressive therapy, urinary retention may predispose patients to secondary urinary tract infections such as pyelonephritis. Prompt recognition and appropriate management are therefore essential. We report the case of a woman with a history of living-donor kidney transplantation who developed urinary retention and pyelonephritis following herpes zoster involving the buttock. She had

Disseminated Varicella Zoster and Multiorgan Failure Requiring ECMO Support in a Patient With Evans Syndrome.

Visceral disseminated varicella-zoster virus (VD-VZV) infection is an uncommon but potentially life-threatening manifestation of varicella infection that primarily affects immunocompromised patients. When hematogenous dissemination occurs, the risk of significant organ dysfunction and death increases considerably. The disease develops through systemic viral spread to visceral organs, resulting in widespread multisystem involvement. Patients with Evans syndrome may be at increased risk because of underlying immune dysregulation and the frequent need for immunosuppressive treatment. We report th

Adult vaccination coverage and missed preventive care opportunities among Filipino adults with diabetes: A cross-sectional study.

Adults with diabetes mellitus are at increased risk of vaccine-preventable infections and complications. Despite recommendations for adult immunization, uptake remains suboptimal in many low- and middle-income countries, including the Philippines, where data remain limited. This cross-sectional study assessed adult immunization coverage and associated clinical and sociodemographic factors among Filipino adults with diabetes recruited from inpatient, outpatient, and community outreach settings at a tertiary hospital. Uptake of influenza, pneumococcal, herpes zoster, hepatitis B, tetanus-diphthe

[Herpes Simplex Virus and Varicella-Zoster Virus Keratitis].

Herpesvirus infections of the cornea are among the leading infectious causes of corneal vision loss worldwide. Both herpes simplex virus (HSV) and varicella-zoster virus (VZV) play a major role in the development of corneal disease. Following primary infection, both viruses establish lifelong latency in sensory ganglia and may be reactivated by triggering factors such as ultraviolet (UV) exposure, stress, fever, surgery, or immunosuppression. Clinically, ocular herpesvirus infections present with a broad spectrum of manifestations, and early differentiation between these entities is essential

Varicella-zoster virus encephalitis in an elderly patient with influenza A infection.

Varicella-zoster virus (VZV) encephalitis is a rare but severe condition associated with significant morbidity and mortality. Its clinical presentation is often nonspecific and may overlap with other neurological conditions, including complications of influenza infection. We report a case of a 76-year-old man with confirmed influenza A infection who developed progressive neurological deterioration. Notably, no cutaneous manifestations suggestive of herpes zoster were observed. Cerebrospinal fluid analysis revealed lymphocytic pleocytosis, and polymerase chain reaction confirmed the presence of

Macrophages of CNS and DRG in Postherpetic Neuralgia: From Pathobiology to Therapeutic Frontiers.

BACKGROUND: Postherpetic neuralgia (PHN), the most common complication of herpes zoster (HZ), is a type of neuropathic pain. Described as the presence of chronic, persistent, debilitating pain, PHN impairs patients' quality of life significantly. Currently, numerous clinical interventions for PHN have been developed, each demonstrating various curative effects. However, these interventions are often accompanied by limitations such as high recurrence rates and inconsistent therapeutic outcomes. Further efforts are still needed to explore effective treatments for PHN. OBJECTIVE: Our aim was to

Analgesic Efficacy of Mirogabalin in Patients With Herpes Zoster: Protocol for a Prospective Multicenter Randomized Open-Label Blinded-Endpoint (PROBE) Study.

BACKGROUND: Herpes zoster (HZ) is a debilitating condition characterized by acute dermatomal pain and a high risk of developing postherpetic neuralgia (PHN). Although early antiviral therapy is essential, its analgesic efficacy is often insufficient, particularly in patients with moderate to severe pain. While gabapentinoids are recommended as adjunctive analgesics, current options like gabapentin and pregabalin are often limited by inconsistent efficacy and dose-dependent adverse events. Mirogabalin, a novel gabapentinoid with higher affinity and selective dissociation for the a2d subunit, ma

Home-Based Remote Programming versus Inpatient Programming After Short-Term Spinal Cord Stimulation for Herpes Zoster Neuralgia: A Single-Center Retrospective Cohort Study.

OBJECTIVE: Short-term spinal cord stimulation (st-SCS) is increasingly used for acute and subacute herpes zoster neuralgia (HZNP), but postoperative management typically requires prolonged hospitalization. We compared two management strategies-continuous inpatient programming versus early discharge with remote programming-on pain outcomes, patient-reported outcomes, safety, and costs. METHODS: This single-center retrospective cohort study included 28 patients with HZNP (disease duration ≤3 months) who underwent percutaneous st-SCS between May 2021 and May 2024. Fourteen patients received inpa

Continuous Intravenous Acyclovir as Outpatient Parenteral Antimicrobial Therapy for HSV and VZV Neurological Infections: A Case Series With Pharmacokinetic and Cost Evaluation, and Review of Literature.

Varicella zoster virus (VZV) and herpes simplex virus (HSV) are major causes of viral meningitis and encephalitis. Standard intermittent intravenous acyclovir requires hospitalisation, reducing patient comfort and prolonging admissions. Continuous infusion may maintain therapeutic plasma and cerebrospinal fluid (CSF) concentrations while enabling treatment through Outpatient Parenteral Antimicrobial Therapy (OPAT). Objectives to evaluate the feasibility, pharmacokinetic target attainment, and cost reduction of continuous intravenous acyclovir for HSV and VZV neurological infections in an OPAT

Reduction of Serum Substance P and Beta Endorphin in Herpes Zoster After Erector Spinae Plane Block.

BACKGROUND: Acute herpes zoster (AHZ) is caused by the reactivation of varicella-zoster virus. This reactivation results in peripheral nerve fiber damage and triggers an immune reaction, which leads to the production of interleukin-6 (IL-6) and tumor necrosis factor alpha (TNF-alpha). In the skin of patients affected with postherpetic neuralgia (PHN), high levels of calcitonin gene-related peptide (CGRP) have been found. OBJECTIVES: The primary goal of this study was to compare changes in serum inflammatory cytokines (IL-6, IL-8) and pain neurotransmitters (substance P, CGRP) with those of a

Brief report: Effectiveness and safety of recombinant zoster vaccine in inflammatory bowel disease patients aged 18 years and older: A matched cohort and self-controlled case series study.

Individuals with inflammatory bowel disease (IBD) are at increased risk of herpes zoster (HZ). Using a matched cohort design, we evaluated the vaccine effectiveness (VE) of recombinant zoster vaccine (RZV) against HZ among IBD patients aged ≥18 years. Within the Kaiser Permanente Southern California population, individuals who received 2 RZV doses were matched up to 1:3 to RZV-unvaccinated individuals. Individuals were followed from 31 days after the receipt of the 2nd RZV dose until the occurrence of a censoring event. VE was calculated using Cox proportional hazards regression. A self-contro
