[Cardiovascular risk in inflammatory rheumatic diseases : Evidence-based strategies for risk reduction in rheumatologic practice].

Patients with rheumatoid arthritis (RA), psoriatic arthritis (PsA), and axial spondyloarthritis (axSpA) have a persistently increased cardiovascular (CV) risk and higher mortality, independently of traditional CV risk factors. Effective control of inflammation reduces CV events, whereas glucocorticoids increase the risk in a dose- and duration-dependent manner, even at ≤ 5 mg prednisolone/day. Disease-modifying antirheumatic drugs especially tumor necrosis factor (TNF) inhibitors, are largely protective through the reduction of systemic inflammation. For patients receiving Janus kinase (JAK) i

Consensus document on the role of adult vaccination in the prevention of cardiovascular events. Joint Statement by the Argentine Federation of Cardiology (FAC), Argentine Society of Cardiology (SAC), and the Argentine Council of Cardiology Residents (CONAREC).

Cardiovascular diseases remain the leading cause of death among adults, both in Argentina and worldwide. Numerous studies have established a consistent association between infections -particularly respiratory infections- and an increased risk of cardiovascular events, stroke, arrhythmias, and both cardiovascular and all-cause mortality. The underlying pathophysiological mechanisms include systemic inflammation, immune activation, endothelial dysfunction, prothrombotic states, sympathetic stimulation, and elevated myocardial oxygen demand. In respiratory infections, these effects are further ex

Spinal trigeminal nucleus lesions in trigeminal zoster-associated neuralgia: A retrospective cohort study on diagnostic value and prognostic factors.

BackgroundTrigeminal zoster-associated neuralgia (TZAN) is a refractory intractable craniofacial neuropathic pain with a high risk of progressing to postherpetic neuralgia. Abnormal T2-weighted magnetic resonance imaging (MRI) hyperintensity in the spinal trigeminal nucleus (STN) has been reported in TZAN patients, but its diagnostic value, influencing factors and association with prognosis remain insufficiently explored. This study aimed to investigate the correlation between STN lesions and TZAN and identify factors affecting STN signal characteristics and TZAN therapeutic efficacy.MethodsTh

A Single Dose Study of Pembrolizumab in People With HIV Infection Who are Immunologic Nonresponders.

BACKGROUND: Some people with HIV (PWH) have a poor immunologic response to antiretroviral therapy (ART). Such immunologic nonresponders are at increased risk for HIV and non-HIV related complications. Immune exhaustion may contribute to poor immune reconstitution, and blockade of PD-1, an immune checkpoint molecule, may thus be beneficial. METHOD: We undertook a phase 1, 3:1 randomized, placebo-controlled trial of a single 200 mg dose of pembrolizumab in PWH with CD4+ T-cell counts between 100 and 350 cells/mm3, who had been on ART and were virally suppressed for at least 12 months. A European Delphi Consensus to Support the Diagnosis, Management, and Appropriate Positioning of Topical Treatments for Postherpetic Neuralgia (PHN).

INTRODUCTION: Postherpetic neuralgia (PHN) is the most common complication of herpes zoster (HZ), characterized by persistent neuropathic pain that significantly impairs quality of life. Despite the availability of multiple treatment options, PHN remains under-recognized and undertreated, with wide variation in management practices across Europe. This Delphi consensus aimed to: (1) identify key challenges and unmet needs in PHN diagnosis and management; (2) determine the optimal use and positioning of topical treatments, such as lidocaine-medicated plasters; and (3) develop best-practice recom

Altered Structural Brain Connectivity in Postherpetic Neuralgia and its Association with Neuropathic Pain Severity: A Diffusion Tensor Imaging Study.

Postherpetic neuralgia (PHN), the most severe complication of herpes zoster, causes persistent neuropathic pain that impairs quality of life. This study investigated alterations in structural brain connectivity in patients with PHN using diffusion tensor imaging (DTI) and examined associations with symptom severity. We enrolled 42 patients with PHN and 41 age- and sex-matched healthy controls. All participants underwent DTI, and data were analyzed using DSI Studio. Graph-theoretical network metrics were computed to assess global and local structural connectivity. PHN symptom severity was evalu

[Effects of spinal cord stimulation implantation on heart rate variability in patients with postherpetic neuralgia].

OBJECTIVES: Postherpetic neuralgia (PHN) is a severe and distressing complication in elderly patients with herpes zoster and is associated with increased sympathetic nervous system activity. Spinal cord stimulation (SCS) implantation has been shown to effectively relieve PHN. In recent years, heart rate variability (HRV) analysis, as a reliable and objective indicator of autonomic nervous system (ANS) activity, has been widely used in the assessment of chronic pain. This study aims to investigate the clinical effects of SCS implantation on HRV in patients with PHN and its correlation with pain

[Safety and efficacy of pulsed radiofrequency therapy for zoster-associated trigeminal neuralgia under general anesthesia].

OBJECTIVES: Pulsed radiofrequency (PRF) treatment of the trigeminal ganglion via foramen ovale puncture for zoster-associated trigeminal neuralgia typically requires patient feedback during sensory and motor stimulation, and is usually performed under regional anesthesia (RA). However, under RA, patients often experience poor comfort due to pain and anxiety during puncture and PRF, and severe pain occurs when adjusting needle position or increasing output voltage. This often leads to lower-than-optimal treatment voltages, poor patient cooperation, and sometimes interruption or refusal of treat

[Pulmonary branch embolism after short-term spinal cord stimulation for zoster-associated pain: Two cases and a literature review].

Short-term spinal cord stimulation (st-SCS), as a minimally invasive neuromodulation technique for zoster-associated pain (ZAP), can effectively relieve pain and reduce the incidence of postherpetic neuralgia (PHN). This paper reports two elderly male patients with ZAP admitted to the First Affiliated Hospital, Zhejiang University School of Medicine in August 2024, who developed acute pulmonary embolism (APE) 3 days after st-SCS implantation. Both patients had preoperative high-risk factors, including a history of hypertension and elevated D-dimer levels. Close monitoring of D-dimer levels was

Spinal Cord Stimulation and DREZ Lesioning for Refractory Postherpetic Neuralgia: An Exploratory Case Series.

PURPOSE: Refractory postherpetic neuralgia (PHN) remains one of the most challenging forms of neuropathic pain after herpes zoster infection, often resistant to conventional therapy. Neurosurgical interventions-particularly spinal cord stimulation (SCS) and dorsal root entry zone (DREZ) lesioning-offer potential benefit, yet optimal selection of surgical treatment remains unclear. This study explores the clinical evidence application of SCS and DREZ lesioning, focusing on clinical outcomes, safety, and costs. PATIENTS AND METHODS: This retrospective single-center series included ten patients
