[Biologics and infections: treat and prevent].

Although biologics used for systemic therapy of dermatological conditions are generally well tolerated, infection complications can still occur depending on the specific substance. Important measures to increase patient adherence to therapy, therefore, include providing appropriate information about the possible occurrence of infections, their prevention, and treatment. For example, in the case of recurrent Candida infections of the skin and mucous membranes under treatment with interleukin (IL)-17 blockers, fluconazole can be administered once a week as recurrence prophylaxis. In cases of inc

Pyoderma gangrenosum as a manifestation of multisystem autoimmunity in a patient with giant cell arteritis, optic neuritis and rheumatoid arthritis.

Pyoderma gangrenosum (PG) is a rare neutrophilic dermatosis typically associated with autoimmune disease; however, its coexistence with giant cell arteritis (GCA) and optic neuritis is exceedingly uncommon and rarely reported. We describe a woman in her early 70s with rheumatoid arthritis and recently diagnosed GCA who developed painful breast ulcers while on tapering systemic corticosteroids for GCA, shortly after a herpes zoster infection. The ulcers showed classic PG morphology and fulfilled Delphi diagnostic criteria. She had preceding MRI-confirmed bilateral optic neuritis followed by bil

Low-Level Light Therapy in Postherpetic Neuralgia: A Review of Mechanistic Insights and Clinical Implications.

Known as the most prevailing complication of herpes zoster (HZ), postherpetic neuralgia (PHN) can lead to long-term skin discomfort. Current medications for PHN remain suboptimal with incomplete relief and potential side effects. Low-level light therapy (LLLT) has been increasingly explored as a non-invasive modality for treating PHN, but the mechanistic basis and clinical implications have not been synthesized in a translational framework. In this review, we explore the pathophysiological mechanisms of PHN and the biological mechanisms of LLLT, integrating current mechanistic insights with av

Postherpetic neuralgia risk prediction in hospitalised patients with herpes zoster based on MIMIC-IV.

Postherpetic neuralgia (PHN) is a disabling complication of herpes zoster (HZ). Early identification of high-risk patients may enable targeted prevention and follow-up. We developed and compared machine-learning models for PHN prediction using the Medical Information Mart for Intensive Care IV database and identified key predictors. This retrospective cohort study used Medical Information Mart for Intensive Care IV v2.2 (2008-2019). Adult inpatients with HZ were identified via International Classification of Diseases-9/10 codes (053.x or B02.x). The outcome was PHN (binary). Candidate predicto

Effectiveness of Amenamevir Vs. Valacyclovir in Preventing Herpes Zoster-Related Hospitalization and Complications: A Target Trial Emulation Using a Large Japanese Claims Database.

Amenamevir is a novel anti-herpes zoster drug that is increasingly used in Japan because of its once-daily administration and lack of renal dose adjustment requirements. However, evidence of its clinical effectiveness is limited. This study aimed to estimate the clinical effectiveness of amenamevir vs. valacyclovir in preventing zoster-related hospitalizations and complications. Using a nationally representative claims database from 2017 to 2024, we conducted a retrospective cohort study with a new-user, active-comparator design. We included adults (aged ≥ 18 years) with uncomplicated herpes z

Effect of Erector Spinae Plane Block Applied to Painful Herpes Zoster Patients Who Failed to Respond to Pregabalin Treatment on Immunological Marker Levels and Postherpetic Neuralgia Incidence Rate.

PURPOSE: Postherpetic neuralgia (PHN) is one of the most debilitating complications of herpes zoster and represents a major cause of chronic neuropathic pain. Increasing evidence suggests that immune dysregulation plays a critical role in neuropathic pain development. This study aimed to investigate the effects of the erector spinae plane (ESP) block on PHN development and immunological marker levels in herpes zoster patients who did not respond to pregabalin therapy. PATIENTS AND METHODS: This prospective, controlled, single‑blind, non-randomized study included 30 patients with herpes zoster

Herpes zoster burden in a private healthcare setting of Argentina.

BACKGROUND: Herpes zoster (HZ) is a frequent condition that can lead to debilitating pain, with complications that may persist for months. The most frequent complication is postherpetic neuralgia (PHN), but ocular, nervous, and dermatological complications are also common. While the incidence of HZ increases with age, few studies have assessed its burden in Latin America, particularly in Argentina. METHODS: We conducted a retrospective cohort study using the electronic health records of the Italian Hospital of Buenos Aires (HIBA) Health Plan from 2010 to 2022. We included members of the HIBA

Effectiveness of recombinant zoster vaccine against herpes zoster and postherpetic neuralgia: a systematic review and meta-analysis of post-licensure observational studies.

BACKGROUND: Herpes zoster causes substantial morbidity in older and immunocompromised adults. The recombinant zoster vaccine is preferentially recommended, but policy decisions increasingly require real-world evidence on effectiveness across diverse populations, dosing patterns, and potential waning of protection over time. METHODS: We conducted a systematic review and meta-analysis of observational studies (cohort and case-control) assessing recombinant zoster vaccine effectiveness among adults (≥18 years). Analytical observational studies published in any language between Jan 1, 2017, and O

Effectiveness and safety of extended upadacitinib induction therapy in adult patients with moderate-to-severe active ulcerative colitis.

BACKGROUND: Responses to induction therapies for moderate-to-severe active ulcerative colitis (UC) are sufficient, but some patients benefit from prolonged induction to achieve clinical outcomes before transitioning to maintenance dosing. Upadacitinib (UPA), an oral JAK1 inhibitor, is approved in the USA for treatment of moderate-to-severe active UC non-responsive to anti-TNF therapy. However, standard 8-week induction regimen with UPA 45 mg/day may not be sufficient for some patients. We aimed to assess the effectiveness and safety of extended UPA induction for moderate-to-severe active UC.

Outcomes in adult liver transplant patients receiving recombinant herpes zoster vaccine.

INTRODUCTION AND OBJECTIVES: Immunosuppressed patients are at an increased risk of complications from herpes zoster (HZ). Recombinant HZ vaccine (RZV) is highly efficacious in preventing HZ in the general population. We sought to evaluate the real-world effectiveness of RZV in liver transplant (LT) recipients compared to those not immunized. METHODS: We identified patients with ≥2 LT diagnosis episodes using Optum Labs Data Warehouse from 1/1/2018 to 12/31/2024. The index date was defined as the earliest date of diagnosis code for transplant or the first date of RZV after identifying an LT pa

Impact of the COVID-19 Pandemic on Outpatient Visit-Based Frequency Trends of Herpes Zoster Ophthalmicus.

Herpes zoster ophthalmicus (HZO) is a vision-threatening disease, yet the long-term impact of the COVID-19 pandemic on its relative frequency and ocular complication profile remains insufficiently understood. This retrospective observational study aimed to quantitatively evaluate the pandemic's influence on the long-term relative frequency trends of HZO and its ocular complications. We analyzed 15 years of clinical data from January 2011 to June 2025 at a single institution in Japan. Interrupted time series analysis was employed to distinguish immediate level changes from long-term trend chang

A multicenter prospective cohort study developing and validating a SIRI-based machine learning model and simplified risk score for predicting postherpetic neuralgia.

BACKGROUND: Postherpetic neuralgia (PHN) is the most common and severe complication of herpes zoster (HZ), and early identification of high-risk patients remains a major clinical challenge. The systemic inflammatory response index (SIRI) is a novel inflammatory marker that has demonstrated prognostic value in multiple diseases, but its association with the risk of PHN development has not been reported to date. Existing PHN prediction models mostly rely on traditional regression methods with limited predictive performance, and lack clinically practical risk stratification tools. METHODS: After

Predictive Value of Anxiety State During Acute Herpes Zoster for the Development of Postherpetic Neuralgia.

BACKGROUND: Postherpetic neuralgia (PHN) remains the most frequent and distressing sequela of herpes zoster (HZ). Although psychological factors are known to contribute to chronic pain, their role during the acute phase of HZ in predicting PHN has not been fully clarified. This study aimed to investigate whether acute-phase anxiety is independently associated with subsequent PHN development and to evaluate the added predictive value of anxiety assessment in clinical prediction models. METHODS: This longitudinal cohort study enrolled 99 patients with acute HZ between January 2022 and January 2

Herpes zoster as a potential clinical marker of undiagnosed Type 2 diabetes in older adults: a retrospective cohort study.

OBJECTIVE: Herpes zoster has been associated with immune and metabolic disturbances; however, its relationship with newly diagnosed Type 2 diabetes mellitus (T2DM) remains unclear. This study aimed to investigate whether herpes zoster may serve as a clinical marker of previously unrecognized T2DM among people aged 65-84 years. METHODS: We conducted a retrospective cohort study using de-identified electronic health records from TriNetX (2006-24). Patients aged 65-84 years with a diagnosis of herpes zoster (ICD-10 code B02) were compared with age-matched patients without herpes zoster. Propensi
