Case report: Hypertrophic lichen planus initially misdiagnosed as squamous cell carcinoma.

Fewer than 26 case reports describe hypertrophic lichen planus (HLP) misdiagnosed as cutaneous squamous cell carcinoma (cSCC). It can be difficult to distinguish between HLP and cSCC, as these two conditions share many clinical and histopathological characteristics. Patients who are misdiagnosed with cSCC often undergo unnecessary medical and/or surgical interventions before receiving a diagnosis of HLP and improving on HLP-directed therapy. This case series highlights the course of three female patients, referred to a single tertiary institution between 2018 and 2022, who were initially misdi

PD-1 Inhibitor Induced Hypertrophic Lichen Planus: A Case Report.

BACKGROUND AND OBJECTIVE: PD-1 inhibitors have revolutionized cancer therapies and are being used to treat an expanding array of cancers. To best serve patients, clinicians should be familiar with the spectrum of skin manifestations associated with PD-1 inhibitor therapy. Here, we report a unique case of hypertrophic lichen planus (HLP) in a 64-year-old man treated with pembrolizumab; the presentation initially suggested a squamous cell carcinoma (SCC) morphology, then evolved into a morphology more typical of hypertrophic lichen planus. This case underscores the need for caution in diagnosing

Anogenital Erosive Lichen Sclerosus in a Male With Multiple Autoimmune Conditions.

Lichen sclerosus (LS) is a chronic inflammatory disorder primarily affecting the anogenital region, with a higher prevalence in females and often linked to autoimmunity. This association is not clearly elucidated in males, with LS commonly presenting in uncircumcised males. The most affected areas include the glans penis, prepuce, and coronal sulcus. In this report, we present an 11-year case of treatment-resistant LS in a male patient with an extensive history of autoimmune disorders, manifesting in the intergluteal cleft as a hypertrophic plaque, a rare location. The patient had a complex au

Prurigo Nodularis Mechanisms and Current Management Options.

Prurigo nodularis (PN) manifests with highly pruritic lesions that negatively impact patient quality of life. Due to the variability in manifestation, hypertrophic lichen planus, pemphigoid nodularis, and neurotic excoriations tend to mimic PN. The pathophysiology of PN is believed to be due to interactions in the neural and immune pathways causing the release of proinflammatory and pruritogenic cytokines. Dermatologic and systemic conditions, including atopic dermatitis and chronic kidney disease, accompany PN diagnoses. Patients with moderate to severe or recalcitrant PN may benefit from dup

Markedly Exophytic Hypertrophic Lichen Planus in a Patient With Ichthyosis Vulgaris.

Hypertrophic lichen planus (HLP) is an idiopathic inflammatory condition characterized by hyperkeratotic plaques or nodules, typically occurring bilaterally on the wrists, ankles, or lower extremities. This variant of lichen planus is more common among African-American patients and occupies a broad differential with other keratotic skin conditions, some of which are malignant, making recognition and accurate diagnosis essential. We present an unusual case of a 49-year-old African-American woman with four markedly exophytic, horn-like lesions on her shins, ultimately diagnosed as HLP. Her medic

An Atypical Manifestation: Linear Hyperkeratotic Prurigo Nodularis.

We present the case of a 42-year-old Indian male with prurigo nodularis, exhibiting multiple verrucous, brownish-black nodules on the left lower and right upper extremities, with milder involvement of the right lower extremity. The lesions were asymmetrically distributed in a near-linear pattern, with prominent white hyperkeratosis and associated moderate pruritus and paresthesia. Differential diagnoses included pemphigoid nodularis, verruca vulgaris, and hypertrophic lichen planus, with a biopsy confirming prurigo nodularis. While prurigo nodularis typically presents with symmetric dome-shape

Case report: Rare case of laryngeal hypertrophic lichen planus.

INTRODUCTION AND IMPORTANCE: Lichen planus is an inflammatory and chronic disease with multifactorial causes. Hypertrophic subtype of lichen planus is an extremely rare lesion when found in the larynx. This article describes the case of a man with such a lesion. CASE PRESENTATION: A 54-year-old man presented after respiratory distress with a suspicious mass on the left hemilarynx. The patient consumed a lot of tobacco and alcohol. He underwent nasofibroscopy, CT scan and numerous direct laryngoscopies with biopsy. Histopathological analysis finally revealed a mucosal hypertrophic lichen planu

Dermoscopy to differentiate clinically similar inflammatory and neoplastic skin lesions.

INTRODUCTION: Over the few last decades, dermoscopy has become an invaluable and popular imaging technique that complements the diagnostic armamentarium of dermatologists, being employed for both tumors and inflammatory diseases. Whereas distinction between neoplastic and inflammatory lesions is often straightforward based on clinical data, there are some scenarios that may be troublesome, e.g., solitary inflammatory lesions or tumors superimposed to a widespread inflammatory condition that may share macroscopic morphological findings. EVIDENCE ACQUISITION: We reviewed the literature to ident

Solitary verrucous plaque on a leg.

We present the case of a female in her 70s who presented with a solitary verrucous plaque on her left leg accompanied by painful oral erosions. Various differential diagnoses were considered, like lichen simplex chronicus, hypertrophic lichen planus, and chromoblastomycosis. We diagnosed pemphigus vegetans (PVeg) on a nonintertriginous site through comprehensive clinical examination and histopathological and immunopathological evaluations. This case highlights the importance of considering PVeg in the differential diagnosis of solitary verrucous plaques, even in atypical extra-flexural anatomi

Hypertrophic Lichen Planus: An Up-to-Date Review and Differential Diagnosis.

Hematopoietic stem cell transplantation (HSCT) is being increasingly used as a curative approach for sickle cell disease (SCD). With the risk of graft-versus-host disease (GVHD), especially in the human leukocyte antigen-mismatched donors, intense immunosuppression is required leading to an increased risk of viral infection. Post-HSCT, adoptive transfer of virus-specific T-cell (VST) therapies have not been well-studied in patients with SCD. Here, we report the outcomes of patients with SCD at a single-center who received VSTs after transplant to prevent or treat viral infections. Thirteen pat

Cutaneous Squamous Cell Carcinoma Mimickers: Beware the Pruritic Papule on the Leg of an Older Female.

Background Correctly identifying cutaneous squamous cell carcinoma (cSCC) mimickers can be both clinically and histopathologically challenging. We present a series of patients with biopsy-proven cSCCs for whom multiple surgeries were avoided by assessing the clinical situation, recognizing an alternative diagnosis that pathologically mimics cSCC, and prescribing appropriate therapy for the underlying condition. Methodology Seven female patients presented for treatment of biopsy-proven cSCC affecting the lower leg. After further investigation, we observed that these women shared similar charact

The Role of Lasers in Connective Tissue and Inflammatory Dermatoses: A 10-Year Retrospective Review of 60 Patients in a UK Tertiary Laser Clinic.

INTRODUCTION: The role of lasers in the treatment of standard therapy-resistant inflammatory dermatoses and connective tissue disorders has been controversial and evidence supporting the role of lasers in this setting is scarce. OBJECTIVE: To assess the efficacy of lasers in the management of inflammatory dermatoses and connective tissue disorders (CTD). MATERIALS AND METHODS: A retrospective case review of all inflammatory dermatoses/connective tissue diseases treated in a tertiary laser clinic between March 2010 and 2020 was undertaken. RESULTS: A total of 60 cases (48 = female) were incl

Eosinophils in Traditionally Noneosinophil-Rich Dermatoses.

The presence or absence of tissue eosinophilia has previously aided in the diagnosis of inflammatory skin conditions. However, recent studies have elucidated the presence of eosinophils in traditionally eosinophil-poor inflammatory skin diseases, such as dermatomyositis (DM), psoriasis, and lichen sclerosus (LS). This systematic review of the literature explores previous studies of tissue eosinophilia in skin biopsies of dermatoses that are believed to be classically poor in eosinophil. We identified 26 studies, the majority of which were retrospective reviews. The percent of specimens with in

A Case of Linear Hypertrophic Lichen Planus in a Pediatric Patient.

Lichen planus (LP) is a chronic inflammatory disease of the skin and mucosa. Of the various types, the hypertrophic type is characterized by thickened, purplish hyperkeratotic plaques and nodules. The course of hypertrophic LP tends to be more chronic than those of other types. A 12-year-old girl presented with a 2-year history of warty papules and plaques in a zosteriform configuration along one flank. Histopathology revealed hyperkeratosis and papillomatosis with wedge-shaped hypergranulosis. A lichenoid lymphocytic infiltrate with vacuolar change in the basal layer was evident. She was pres

The Coexistence of Trachyonychia and Mucocutaneous Lichen Planus: A Case Report.

Trachyonychia is an uncommon finding characterized by the nails having a uniform and simultaneous presence of fragility, excessive longitudinal growth, ridging, and loss of luster. Usually, twenty-nail dystrophy (TND)/trachyonychia is an idiopathic condition, but sometimes dermatoses such as alopecia areata, lichen planus, and psoriasis are found to be associated with it. We report a case of trachyonychia/TND in a young male with the concomitant presence of cutaneous lichen planus of hypertrophic type, reticular oral lichen planus, and nail lichen planus, which was diagnosed with the aid of de

Bidirectional Chemotherapy in Advanced Colorectal Cancer Peritoneal Metastases.

Gastrointestinal parasite infections have been identified as a significant public health concern in regions with high humidity levels globally. Ascaris lumbricoides, a prevalent human pathogen, is widely distributed throughout several regions globally. It is estimated that around 1.5 billion cases are attributed only to A. Lumbricoides. These types of disorders are associated with a wide range of diseases. Several animal studies have demonstrated the significance of T helper cell type 2 responses to helminths in the digestive system, as well as the crucial role of specific interleukins (ILs),

Evaluation of salivary oxidative stress in oral lichen planus using malonaldehyde.

BACKGROUND: Oral lichen planus is a chronic, mucocutaneous, inflammatory disease, with an unknown etiology. Reactive oxygen species and oxidative damage to the tissues might be the cause. Malonaldehyde (MDA), a low molecular weight end product of lipid peroxidation reaction is a suitable biomarker of endogenous DNA damage. monitoring the oxidant-antioxidant status of saliva may serve as an efficient marker of disease development in oral lichen planus patients. AIM AND OBJECTIVES: To evaluate salivary oxidative stress in oral lichen planus subject using MDA and compare it with control subjects
