The article considers history of searching for solutions of problems of smallpox vaccination in the Irkutsk Gubernia in 1887-1892. It is demonstrated that smallpox outbreaks served as a basis for issue of the municipal Duma resolution about organization of Irkutsk smallpox vaccination room and calf barn, along with determination of costs for maintenance of smallpox vaccination cabinet. The expansion of supply of detritus to Okrugs, including the Yakutsk Oblast and the Primorye, determined necessity of reorganization of the Irkutsk smallpox vaccination department. It is noted that organization

Mpox disease has emerged from obscurity into the global spotlight, as the Orthopoxvirus monkeypox virus (MPXV) has sparked multiple independent outbreaks with sustained human-to-human transmission-proving it can spread, adapt, and persist. Furthermore, these events have exposed critical gaps in our understanding of MPXV's molecular virology and its interactions with the host immune system. A wide host range, adaptable genomic architecture, immune evasion, and waning immunity due to the discontinuation of smallpox vaccination are possible factors contributing to increased spill-overs and increa

Different systems for controlling smallpox in the early nineteenth century are investigated, considering the actions of different agents, mechanisms, and strategies involved in its organization, manufacture, and distribution. This dangerous disease emerged in the Americas following European colonization. Immunization practices arrived in the Viceroyalty of the Río de la Plata in 1805, coordinated first by the Protomedicato and later through the Conservatory of Vaccines. The economic hardship and wars of the fledging state hindered the inoculation of human and cowpox viruses. Humans were used a

BACKGROUND: Tecovirimat is approved for smallpox treatment under the Food and Drug Administration Animal Rule on the basis of efficacy in nonhuman primate models of mpox (previously known as monkeypox). However, the clinical efficacy of tecovirimat against human clade II mpox is unclear.
METHODS: In a phase 3, international, double-blind, randomized, placebo-controlled trial, we evaluated the efficacy of oral tecovirimat in adults with laboratory-confirmed clade II mpox. Participants were randomly assigned in a 2:1 ratio to receive tecovirimat or placebo for 14 days. The primary outcome was c

BACKGROUND: Mpox is a re-emerging zoonotic disease of global public health concern. Healthcare workers (HCWs) face elevated occupational risk and influence community vaccination behaviour, yet evidence on mpox vaccine uptake and hesitancy among Nigerian HCWs is limited. This study investigated the determinants of mpox vaccine uptake and hesitancy among HCWs in Port Harcourt, Nigeria. METHODS: This cross-sectional study involved 595 HCWs recruited using multistage sampling across healthcare facilities in Port Harcourt between March and July 2025. Data collection employed a structured, validated

Monkeypox (Mpox), a zoonotic viral disease caused by the Monkeypox Virus (MPXV), has gained significant attention in recent years due to its increasing incidence and the grave threat it poses to global health. MPXV has spread at a rapid pace during the COVID-19 pandemic, causing 10,000+ confirmed cases and ~300 fatalities in 122 countries. This virus comprises two major clades, Clade I (Central African), which is evidently more virulent, and Clade II (West African), which has caused the recent outbreaks across the world and caused fewer deaths. Clinically, Mpox presents as a milder form with f

BACKGROUND: The Democratic Republic of Congo (DRC) is recognized as the global epicenter of human Mpox. While vaccination is crucial for outbreak prevention, especially as the disease transitions from zoonotic spillover to sustained human-to-human transmission, comprehensive assessments of vaccination coverage trends across the country are notably absent from the literature. This systematic review and meta-analysis address this gap by providing the first pooled estimate of Mpox vaccine uptake and acceptance in the DRC over a 54-year period (1970-2024). Our study captures critical transitions,

BACKGROUND: Mpox is a re-emerging zoonotic infection caused by an Orthopoxvirus closely related to smallpox. The 2022-23 global outbreak prompted rapid use of vaccinia-based vaccines, historically developed for smallpox and those of the latest generation used for mpox prevention. Assessing their safety and effectiveness in pregnant persons and children is critical to guide policies protecting populations at an elevated risk of severe illness.
OBJECTIVE: This study assessed the safety and effectiveness of mpox and historical smallpox vaccines, administered during pregnancy and childhood.

Brincidofovir (BCV, commercially known as Tembexa®) is an antiviral drug with broad-spectrum activity against double-stranded DNA viruses, including orthopoxviruses. BCV was developed and approved for treating human smallpox in adults and children, including neonates, under the FDA Animal Rule. The efficacy data for licensure were obtained using two surrogate models of smallpox: the rabbitpox virus (RPXV) in New Zealand White rabbits and ectromelia virus (ECTV) in Balb/c mice. The potential for BCV resistance development was evaluated in RPXV- and ECTV-infected animals treated with BCV from pi

BACKGROUND: Modified vaccinia Ankara-Bavarian Nordic (MVA-BN) is a third-generation, replication-deficient, smallpox and mpox vaccine that is prepared in liquid and lyophilised formulations. DR Congo reports the highest number of mpox cases annually. Safety data on MVA-BN are available from high-income countries, but comprehensive safety evaluations of MVA-BN are limited in the African continent.
METHODS: From Feb 23 to Aug 29, 2017, and from Aug 15 to Sept 17, 2019, 1600 adult (aged 18 years and older) health-care personnel (eg, physicians, nurses, and technicians) at risk for mpox in the Ts

Mpox is a smallpox-like disease caused by the monkeypox virus. In 2022, a global outbreak of mpox resulted in more than 113 000 cases. Just a year later, a different strain of mpox caused the largest ever outbreak in Africa, with over 46 000 cases. Previous mpox epidemics were small, primarily occurred in Africa, had a case-fatality rate of 1-10%, and spread readily through contact with lesions and fomites. The new outbreaks had features never seen before with mpox - milder presentations and sexual transmission - that, together, fueled much wider dissemination of the disease. In this review, w

We use simple mathematical models to explore the factors that influence the evolutionary emergence of mpox to a pathogen capable of sustained human to human transmission that poses a global threat. Smallpox eradication followed by the discontinuation of immunization with vaccinia has led to a decline in the level of population immunity against related poxviruses such as mpox. This decline in immunity results in an increase in both the number of spillovers and the extent of human to human transmission. We find that increases in transmissibility of mpox between humans have a much greater effect

The global re-emergence of monkeypox virus (MPXV) has exposed a widening immunity gap following cessation of routine smallpox vaccination and renewed interest in next-generation orthopoxvirus vaccines. Unlike acute RNA viruses, MPXV is a large double-stranded DNA virus with a dual-virion architecture intracellular mature virion (IMV) and extracellular enveloped virion (EEV) and encodes multiple immune evasion proteins that complicate vaccine design. These biological features necessitate multivalent antigen targeting and coordinated humoral and cellular immunity for effective protection. Messen

Mpox, a zoonotic orthopoxvirus disease, has transitioned from a rare infection confined to African rainforests to a global public health threat. Originally identified in laboratory monkeys in 1958, the first human case was documented in 1970 in the Democratic Republic of Congo. Following the declaration of smallpox eradication in 1980 and the subsequent cessation of smallpox vaccination, mpox cases persisted at low levels before surging, ultimately leading to the declaration of two Public Health Emergencies of International Concern by the WHO in 2022 and 2024, and the Africa CDC declaration of

BACKGROUND: Data on the impact of completing 2 doses of the modified vaccinia Ankara vaccine and/or smallpox vaccination in childhood on Mpox disease severity remain limited. During the last months of 2024, Germany faced a second Mpox wave, providing the opportunity to study the impact of different vaccine regimens including smallpox vaccination.
METHODS: Retrospective analysis of all cases diagnosed at 17 participating German centers 2022-2024. Cases were grouped according to smallpox vaccine history: "unvaccinated," "incomplete" (vaccinations only during childhood and/or only one MVA BN), "

Monkeypox (Mpox) disease, caused by the Monkeypox virus (Mpox virus), emerged as a significant global health threat during the 2022 outbreak, prompting the World Health Organisation (WHO) to declare it a Public Health Emergency of International Concern (PHEIC). Rapid evolution through genomic modifications enhanced its outbreak potential. Zoonotic transmission occurs through close contact with infected rodents or primates; human-to-human transmission occurs via close contact or homosexual intercourse. The virus disseminates via the lymphatic system, causing symptoms ranging from mild skin lesi

Since the elimination of smallpox, mpox (monkeypox) is the most medically significant orthopoxvirus infection. As a result of numerous regional, national and global outbreaks of MPXV (mpox virus), there is an abundance of new data available on the effects of the different viral clades on clinical obstetrical and perinatal outcomes when infection occurs in pregnancy. In addition, there have been additional placentas from cases of congenital MPXV infection available for study. These recent data indicate that there are prominent differences between viral strains and their effects on the fetus, wi

Currently available mpox vaccines (actually smallpox vaccines) face numerous challenges including supply shortages, limited specificity, and adverse reactions. Given that monkeypox virus (MPXV) transmits through respiratory (Clade I) and intimate contact (Clade IIb, primarily sexual behavior), developing vaccines capable of eliciting mucosal immunity is crucial for effective mpox prevention. This study developed an oral mpox vaccine, ABMΔEcN, based on attenuated Escherichia coli Nissle 1917 (ΔEcN). This engineered probiotic stably expresses three MPXV antigens (A9R/B6R/M1R) derived from extrac

BACKGROUND: Mpox, caused by monkeypox virus (MPXV), has gained global attention following several international outbreaks. Accurate and high-throughput detection of MPXV-specific antibodies is essential for surveillance programmes, diagnosis, and vaccine trials. We therefore evaluated the long-term accuracy of an mpox multiplex immunoassay that measures IgG responses to 11 orthopoxvirus antigens.
METHODS: The assay was validated in a diagnostic accuracy study involving a Belgian cohort of clade IIb MPXV-infected individuals (n=211) and modified vaccinia virus Ankara-Bavarian Nordic (MVA-BN) v

The Monkeypox virus (MPXV), an emerging zoonotic orthopoxvirus, has reemerged as a major global health threat, with the 2022 outbreak predominantly associated with Clade IIb lineages. The surface glycoprotein B21R (OPG210), a major virulence and immune-evasion factor, contains three lineage-defining amino acid substitutions (D209N, P722S, and M1741I) that distinguish Clade IIb from other variants, emphasizing its relevance as a vaccine target. In this study, an integrated immunoinformatics and molecular dynamics-based approach was employed to design a multi-epitope vaccine (MEV) targeting the
