Lichen planus specific antigen (LPSA) is a lesion-specific marker that differentiates lichen planus from other dermatoses. Serum from lichen planus patients reacted in the indirect immunofluorescent test with fetal epidermis. The reaction was not observed between serum from people with no clinical history of lichen planus and fetal skin tissues. Adsorption of lichen planus serum with a fetal skin extract removed the antibody to LPSA. Colour developers are widely used and contact dermatitis and lichen planus-like eruptions from them are well-known. As a result of automation, there have been few recent reports of contact dermatitis from colour developers. We describe here 4 cases of dermatoses from Kodak CD4 colour developer and study their cross sensitivity to other colour developers and PPD. Attempts were made to determine the purity of the patch test allergens to ensure that cross sensitivity was not the result of contamination and decomposition of the allergens.

Ten patients with generalized lichen planus were treated with oral 8-methoxypsoralen photochemotherapy (PUVA) in a bilateral comparison study. Five patients (50%) cleared completely on both sides and required no maintenance treatment after a follow-up of up to 4 years. Three cases of oral lichenoid reaction linked with the administration of allopurinol are presented. Withdrawal of the drug resulted in considerable clinical improvement in one case and complete resolution of the ulcerative lesions in the other two cases. The consideration of drug-induced reactions in the differential diagnosis of oral mucosal diseases has been emphasized.

The clinical features of 8 Nigerian cases of sarcoidosis are presented. All of them had skin lesions and at least one other organ involved. A 9 1/2-year-old girl was seen with 20-nail dystrophy. Lichen planus was suspected and further examination revealed other lesions localized to the oral mucosa. The diagnosis was confirmed as lichen planus by biopsy of tissue from the oral lesions. Careful examination of all mucocutaneous surfaces is recommended before accepting the diagnosis of so-called isolated 20-nail dystrophy of childhood.

Lichen planus (LP) is an entity endowed with a striking polimorphysm and with a diversity of topographic localizations. Lichen planus may occasionally involve the conjunctiva or other mucous membranes. Three new cases are described. Generally, white ridges appear on the palpebral conjunctiva, histological examination revealing inflammatory infiltration of the basal membrane, which is eroded by the infiltrate. The disease could be more frequent than indicated by the literature, but its treatment is usually disappointing.

Lichen planus is a common papulosquamous disorder of skin and mucous membranes of unknown etiology. Studies reported here demonstrate a lichen planus specific antigen (LPSA) in granular and deep prickle epithelial cells from the lichen planus lesion. D-Penicillamine, a heavy metal chelator used in the treatment of Wilson's disease and other conditions, may be associated with both noncutaneous and cutaneous side effects. Some of the cutaneous lesions are due to a toxic-metabolic effect on connective tissue; some may be explained on the basis of autoimmunity; some are acute sensitivity reactions, and some are secondary to unknown mechanisms. The types of cutaneous manifestations may, in some instances, be correlated with the disease being treated and the dosage and duration of penicillamine therapy.

We report three cases of erosive oral lichen planus treated with griseofulvin, 500 mg. twice a day. Subjective improvement was noticed in 3 weeks by two patients and at 6 weeks by the third patient. Objective improvement, however, was delayed and became apparent at 6 to 10 weeks. Continued improvement or remission without exacerbation was noted in all patients. Leukoplakia, lichen planus and carcinoma in situ of human oral mucosa were analysed histometrically by light microscopy. Leukoplakia lesions showed a greater total nuclear density than lichen planus lesions. They had a similar basal nuclear density to carcinomas in situ. The total and basal nuclear areas were notably greater in carcinomas in situ, indicating the presence of larger nuclei in comparison to those of other lesions. We believe it to be possible to make a differential diagnosis based on numerical variables.
