Immunopathology of psoriasis: a comparison with other parakeratotic lesions.

Characteristic in vivo binding of IgG and C3 to the stratum corneum of psoriatic lesions has been implicated as a significant event in the pathogenesis of the disease. Immunofluorescent findings of psoriatic lesions are compared with other parakeratotic and nonparakeratotic lesions. Parakeratotic lesions studied include verrucae vulgares in patients with psoriasis, verrucae vulgares in patients without psoriasis, actinic keratoses and lichen simplex chronicus. Psoriatic lesions and other parakeratotic lesions demonstrate similar immunopathological phenomena within the involved stratum corneum:

Oral treatment of keratinizing disorders of skin and mucous membranes with etretinate. Comparative study of 113 patients.

This study reports comparative results of the effects of an aromatic retinoid, etretinate (RO 10-9359), on various disorders of the skin and mucous membranes. One hundred thirteen patients suffering from psoriasis, lichen planus, keratosis follicularis, and various other disorders were treated and examined. The patients received 25 to 100 mg/day of oral etretinate for up to 30 months. Patients with erythrodermic psoriasis, pustular psoriasis, and keratosis follicularis showed the best response. The conditions of patients with psoriasis vulgaris, palmoplantar psoriasis, and lichen planus also i

Direct immunofluorescence in oral mucosal disease: a diagnostic analysis of 130 cases.

We retrospectively analyzed direct immunofluorescence (DIF) findings from 130 cases of oral mucosal disease. The diagnosis of each case was based on history, clinical features, histopathology, and clinical follow-up. To avoid circular reasoning, we did not use the DIF results in forming the diagnoses. Our results indicate that the presence of characteristic fluorescent patterns produced by several DIF reagents can establish the diagnosis of the oral lesions of pemphigus and pemphigoid and strongly indicate the diagnoses of lichen planus and lupus erythematosus. The absence of these fluorescent

Content and distribution of glycogen in oral epithelial dysplasia.

Forty-four oral lesions with epithelial dysplasia and 25 other benign and malignant lesions of the oral mucosa were examined after staining with hematoxylin-eosin and diastase controlled PAS. The intensity of the PAS-positivity for glycogen, the grade of dysplasia, the type of keratinization and the degree of subepithelial inflammation were recorded. Histologically normal epithelium at the margins of the lesions were used as controls. The presence and amount of glycogen in normal epithelium at the margins of the lesions were used as controls. The presence and amount of glycogen in normal epith

Lichen planus-like lesions caused by penicillamine in primary biliary cirrhosis.

Six patients with primary biliary cirrhosis experienced lichen planus-like lesions while taking penicillamine. Oral lesions developed in all patients, and the lichenoid lesions also appeared elsewhere in two patients. Lichen planus-like lesions developed from four to 16 months after penicillamine therapy had been started, and, in four cases, they resolved in one to three months after use of the drug was stopped. Two patients have continued using penicillamine despite adverse cutaneous and mucosal reactions and have continued to experience lichen planus-like lesions intermittently. Other cutane

Lichen planus-like keratosis. A clinicohistopathologic correlation.

We feel that it is possible to make a reasonably consistent clinical diagnosis of lichen planus-like keratosis on the basis of several criteria. These criteria include physical appearance, location and short duration of the lesion, clinical symptoms such as pruritus and pain, age and sex of the patient, and the frequent presence of a preceding lentigo. Clinical data on fifty-nine histologically confirmed cases are presented. Some interesting features of this curious condition are discussed. Because of the clinical similarity of lichen planus-like keratosis to other sometimes more ominous lesio

[Concurrence of lichen planus pemphigoides and ulcerative colitis (author's transl)].

Lichen planus belongs to the group of frequent skin diseases. Therefore, the concurrence with other diseases might be expected by chance. However, the bullous form of lichen planus is a rare disease and for this variant a coincidence with malignant diseases, tumors and systemic diseases, has been reported. Additionally, in the last decade the association of lichen planus respectively lichen planus pemphigoides and diseases accompanied with or caused by immunologic dysfunction has been pointed out. This concerns lupus erythematosus, myasthenia gravis, and ulcerative colitis, diseases, which are

[Granulofilamentous intranuclear bodies detected by ultrastructural studies in lichen planus and psoriasis].

Based on ultrastructural studies and findings reported in the literature occurrence, frequency, morphological and structural pattern of intranuclear inclusions in lichen ruber planus and psoriasis were analyzed and compared with other structures. It was concluded that granulofilamentar bodies in lichen and psoriasis are not specific structures. They develop as an effect of physiologic and pathologic (hyper) activating factors and may be reversible. Granulofilamentar structures seem to represent separate group of intranuclear bodies. To clear up their features further investigations are needed.

Immunofluorescence studies on civatte bodies and dyskeratotic cells with anti-keratin antibody.

So-called Civatte bodies and dyskeratotic cells were investigated in some skin disorders by using indirect immunofluorescence techniques with anti-human keratin antibody. In the disorders with lichenoid tissue reaction such as lichen planus and DLE, Civatte bodies were observed in the lower epidermis and upper dermis and they reacted distinctly to the anti-keratin antibody. In malignant skin tumors which show dyskeratotic cells in the epidermis, such as basal cell epithelioma and Bowen's disease, dyskeratotic cells were more clearly reacted to the antibody than other keratinocytes. These obser

Immunofluorescent studies in desquamative gingivitis.

Twenty-seven patients (19 females and 8 males) with a clinically diagnosed desquamative gingivitis were used in this study. Twenty-seven additional patients with oral and skin diseases and with gingival involvement other than that of the desquamative gingivitis type served as controls. Biopsy gingival specimens were sectioned and stained with H and E. The rest of the tissue specimen was cut with a cryostat and used for immunofluorescent assays of the tissue-bound antibodies (IgG, IgA, IgM, C3 anf fibrin). Circulating antibodies were determined by using the indirect immunofluorescence method in

Multinucleated epidermal giant cells in inflammatory skin diseases.

The occurrence of multinucleated epidermal giant cells (MEGCs) with more than three clumping nuclei has been considered an episodic and exceptional phenomenon in inflammatory skin diseases. However, we have found them in 97 histologic specimens obtained from inflammatory lesions. They appeared in 88 of 196 biopsy specimens from a group of patients with chronic eczema or prurigo. In pruriginous lesions alone, they were found in 67 (74%) of 90 specimens. On the other hand, MEGCs were found rarely in lichen amyloidosus (2/11), dermatitis herpetiformis (1/6), erythema multiforme (1/11), pustular p

Lichen scrofulosorum with tuberculous dactylitis.

A patient with tuberculosis of cervical lymph nodes, lichen scrofulosorum on the trunk and limbs and tuberculous dactylitis of the hands is reported. Both the clinical and histological features of her lichen scrofulosorum conformed to the characteristic pattern described in other recently reported cases and the association with extra-pulmonary tuberculosis was again evident. There have been no other recent reports of tuberculous dactylitis, which was always said to be due to underlying tuberculous bony involvement. In our patient tuberculous dactylitis, which was clinically indistinguishable f

Ultrastructural findings in acute and chronic graft-vs-hos reaction of the skin.

Two cases of graft-vs-host reaction (GVHR) after allogeneic bone marrow transplantation were studied by transmission electron microscopy. One was observed only in the acute phase and the other, after a mild acute reaction, showed a chronic lichen planus-like pattern. All types of skin damage described in experimental GVHR have been confirmed in both cases. Our findings indicate an involvement of aggressor lymphocytes against keratinocytes as the chief pathogenetic mechanism of the disease, even in its chronic form, although the participation of humoral factors in some phases of the disorder ca

[Clinical field study on the frequency and topographical distribution of lichen ruber including the question of etiological influence factors].

A total of 374 Patients with lichen ruber of skin and/or mucous membranes, treated at our Department during a period of 7 years, have been studied retrospectively. The prevalence rate, the distribution pattern as well as the dependence on localisation of different types of clinical lesions have been examined. In 54% of the cases studied, the oral mucosa was involved, in about half of these cases without any other manifestation. In 92 hospitalised patients, a statistically significant correlation between lichen ruber and chronic hepatic disorders was revealed. On the other hand, chronic alcohol

Globular deposits of immunoglobulins and complement in the papillary dermis. Clinical significance.

Globular deposits of immunoglobulins in the papillary dermis have been reported to occur in certain dermatoses, particularly in lichen planus. The clinical correlation of these deposits in 52 skin biopsy specimens reviewed by light and fluorescent microscopy was studied. These cases included five of lichen planus, 24 of lupus erythematosus or related diseases, four of dermatitis herpetiformis, three of drug eruption; two each of bullous pemphigoid, erythema nodosum, porphyria cutanea tarda; one each of vitiligo, pyoderma gangrenosum, neurodermatitis, erythema multiforme, granuloma annulare, va

Vesiculobullous lesions of the upper aerodigestive tract.

Vesiculobullous lesions of the upper aerodigestive tract are among the most frequently occurring disease processes of the oral soft tissues. Pemphigus vulgaris, mucous membrane pemphigoid, erythema multiforme, and erosive lichen planus are compared and contrasted with each other and with vesiculobullous eruptions of viral origin. Clinical photographs and photomicrographs of these disease processes are presented. Treatment modalities are briefly discussed.

Topical tretinoin therapy and oral lichen planus.

The treatment of severe forms of oral lichen planus is still relatively unsatisfactory. In this controlled study, 0.1% tretinoin in an adhesive base was used in 23 patients. Fifteen other control patients were similarly treated with the vehicle alone. In the tretinoin-treated group, 71% of the atrophic-erosive lesions improved while 29% improved from the vehicle (P less than .05). Reticular-plaque lesions improved in 74% from tretinoin and in 15% from the vehicle (P less than .001). Irritation from tretinoin occurs and thorough usage information is mandatory.
