Serum immunoreactive prolyl hydroxylase protein and galactosylhydroxylysyl glucosyltransferase activity were measured in 54 patients wtih various dermatological diseases and compared with corresponding values in 32 control subjects. These serum enzymes were at the control level in the great majority of the patients, and no correlation was found between serum and skin enzymes, except for one weak correlation in lichen ruber planus. To demonstrate the exact localization of immunoglobulin and fibrin deposits in skin lesions of lichen planus, we employed the peroxidase-antiperoxidase technique, both light and electron microscopically. IgM is shown to be deposited preferentially in and on the cytoid bodies all over their cut surface in a typical stippled pattern, partly obscuring the characteristic fibrillar substructure. Thiacetazone has been used as a standard antituberculous drug in several countries. Side effect& which include various VM of skin eruptions have been observed in about 10% of patients taking thiacetazone. We are reporting two' cases of lichen planus-like lesions presumably due to thiacetazone. Of these, one case had preexisting lichen planus hypertrophicus but the other one, did not have any past history of lichen planus.

Fifty-one Kenyan patients with variable round plaques on the exposed parts, predominantly on the face, and characterized by central hyperpigmentation with annular hypopigmented borders, are described. The clinical and histological picture showed transitions, which ranged from lichen planus and lichenoid poikilodermatous syndromes to dermatitis. This syndrome corresponds largely with (sub-)tropical or actinic lichen planus as reported from the Middle East, where emphasis has been given mainly to a part of the described spectrum of transitions. We suggest that this syndrome should be considered

The unique coincidence of five dermatological disorders, which occurred in a 39-year-old patient, is discussed. The clinical and laboratory examination did not reveal a common underlying cause. It is hoped this report will stimulate the recognition of other cases and thus aid in determining whether the coincidence of these disorders is a true association of diseases with a common underlying factor or a rare abnormality.

In an ultrastructural study of oral lesions in lichen planus, it was found that some plasma cells have an associated lamina densa. Subsequently, similar appearances have been found in pemphigus vulgaris, in the inflammatory cell infiltrates in dental granulomas and cysts and in a patient with Ehlers-Danlos syndrome. The lamina densa was mainly present adjacent to those parts of the membrane of the plasma cell that were invested with fibrillar connective tissue. In 4 cases of PUVA-treated psoriatic patients a number of colloid bodies and amyloid-like deposits were seen at the dermo-epidermal junction, in a very similar fashion the colloid bodies and the amyloid substances are deposited in lichen planus and in primary cutaneous anyloidoses respectively. The action of tiapride was studied in various dermatological conditions with predominant psychosomatic features. A total of 36 cases were evaluated and results were satisfactory in 23 patients and poor in the other 13 cases. The indications which responded best were the lichens, eczemas, psoriatic affections and neurodermatoses. Tolerance was generally satisfactory.

Myasthenia gravis in a woman was followed by the development of severe erosive lichen planus and thymoma. Lichen planus is not ordinarily associated with other cutaneous or systemic disorders. The reported immunologic findings in some patients with lichen planus and especially the occasional association of lichen planus with certain disorders linked with immunologic disturbances suggest an autoimmune pathogenesis for this disorder.

Four patients are described who developed granulomatous reactions in the red portions of their tattoos. Histopathological and immunofluorescence studies showed features of lichen planus. Mercury was identified in only one patient's lesion, and hypersensitivity to mercury was shown by patch testing in one other patient. Tattooing may provide a localised antigenic challenge resulting in spontaneously occurring lichen planus.

In the course of a study of wound healing in four patients with lichen planus, we found transformed keratinocytes with a hitherto undescribed ultrastructure in both wounded and undisturbed papules. We have called these epidermal cells half-and-half cells because they showed changes on the one hand of increased synthetic activity and, on the other hand, of fibrillar transformation closely resembling that seen in fully developed colloid bodies. We suggest that these half-and-half cells may provide a useful clue to the early changes occurring in colloid body formation.

Limited indications for radiation treatment of benign dermatoses are presented in order to emphasize modern restrictions and techniques in the use of dermatologic radiation therapy and to promote measures of protection against stray radiation. The use of softer, less penetrating X rays, least effective doses and meticulous shielding are recommended. So done, radiation therapy offers an effective therapeutic alternative for conditions that do not respond to other forms of treatment.

Selected benign dermatoses, recalcitrant to most other forms of therapy, respond to grenz-ray therapy. When appropriately administered, this is a safe treatment for such conditions. This paper delineates the practical application of grenz-ray therapy at the present time.

A 58-year-old man with rheumatoid arthritis developed lichen planus during treatment with gold. Four months after cessation of treatment, a severe acne with comedones, papules, pustules, and small cystic elements broke out. No connection could be demonstrated between the eruption and other drugs or chemicals. Acne provoked by gold seems not to have been described elsewhere. The case history confirms that gold treatment, even in the same patient, can give rise to a wide range of skin disturbances, which in many cases do not break out until long after the drug has been withdrawn.
