Regenerative Scalp Preparation With Stromal Vascular Fraction and Platelet-Rich Fibrin Prior to Hair Transplantation in Lichen Planus: A Case Report.

Lichen planus (LP) is a chronic inflammatory scalp disorder that can lead to cicatricial alopecia, making it challenging for hair transplantation. This case report describes a 26-year-old female with LP who underwent scalp preparation using SVF and PRF before hair transplantation. SVF was administered 6 weeks prior, followed by PRF 4 weeks before the procedure to enhance scalp vascularization and reduce inflammation. After 1 year of follow-up, significant graft survival and improved scalp condition were observed without major complications. This report highlights the potential of SVF and PRF i

Solid lipid microparticles-into-film delivering clobetasol: from buccal formulation design to pilot clinical study in patients with oral lichen planus.

Oral lichen planus is a chronic inflammatory disorder of the oral cavity, for which the therapeutic gold standard consists of the topical administration of corticosteroids, such as clobetasol-17-propionate (CLB). Nowadays, there are no commercially available pharmaceuticals intended for CLB delivery within the oral cavity. Then, in clinical practice, the drug is administered via magistral galenic preparations, formulated as adhesive semisolids. As they possess numerous drawbacks (e.g., limited contact time, poor patients' compliance, inaccurate dosing), the research is still focused on the dev

Quality and Readability of Large Language Models' Responses to Oral Lichen Planus Patients' FAQs.

OBJECTIVE: To evaluate the quality and readability of large language models (LLMs) when responding to Frequently Asked Questions (FAQs) about oral lichen planus (OLP). METHODS: We evaluated the responses of three LLMs (ChatGPT-4o, Gemini 2.0 Flash Experimental, and Copilot) to 13 patient-centered FAQs about OLP. Questions were identified using query tools, and answers were assessed by 14 oral medicine experts using the Quality Assessment of Medical Artificial Intelligence (QAMAI) tool. Readability was analyzed with the Flesch Reading Ease (FRE) and Flesch-Kincaid Grade Level (FKG) tools. RES

[Research Progress of Peripheral Immune Tolerance in Oral Diseases].

Peripheral immune tolerance serves as a core mechanism for maintaining immune homeostasis within the body. Regulatory T cells (Tregs) are the key cells responsible for performing this function. Through cell-contact-dependent mechanisms, secretion of inhibitory cytokines, and metabolic interference, Tregs actively suppress the activation of effector immune cells. In the local oral environment, the dynamic balance of the Treg/T helper cell 17 (Th17) axis is of crucial importance for maintaining oral health and represents a common immunopathological basis for various oral diseases. Mechanistic insights into the role of oral microbiome in the malignant transformation of oral lichen planus to oral squamous cell carcinoma.

Oral Lichen Planus (OLP) is a common oral potentially malignant disorder, and its transformation into Oral Squamous Cell Carcinoma (OSCC) has become a research focus. In recent years, increasing attention has been paid to the role of the oral microbiome in tumor initiation and progression. Studies have shown that dysbiosis of the oral microbiome may contribute to and accelerate the malignant transformation of OLP to OSCC through multiple mechanisms, including the induction of inflammatory responses, disruption of immune regulation, promotion of oxidative stress, and epithelial-mesenchymal tran

Lichenoid Drug Eruption From Cetirizine Treated With Upadacitinib and Dupilumab.

Cutaneous lichenoid drug eruptions (LDE) are a group of relatively uncommon adverse drug reactions characterized by widespread erythematous to violaceous scaly papules that can be difficult to differentiate from idiopathic lichen planus. Their increasing prevalence with the use of biologics, tyrosine kinase inhibitors, and immune checkpoint inhibitors has become of particular interest in dermatology. More commonly used medications, including antibiotics, antihypertensives, and nonsteroidal anti-inflammatory drugs, have also been reported to cause LDEs. While the exact pathophysiology of LDE re

Multifaceted Role of Vitamin D in the Pathogenesis and Clinical Management of Oral Lichen Planus: Current Evidence and Research Perspectives.

BACKGROUND: Oral lichen planus (OLP) is a chronic, immune-mediated inflammatory disorder of the oral mucosa, primarily involving T-lymphocyte activity. It is recognized by the World Health Organization as an oral potentially malignant disorder due to its established risk for malignant transformation. Although OLP has been the subject of extensive research, its pathogenesis remains partially understood. Vitamin D, a fat-soluble hormone essential for calcium-phosphorus homeostasis and immunological regulation, has emerged as a significant factor in the onset, progression, and clinical course of

Pembrolizumab-associated exacerbation of lichen planopilaris in a patient with metastatic melanoma.

INTRODUCTION: Immune checkpoint inhibitors (ICIs) can trigger cutaneous immune-related adverse events (irAEs), including rare follicular eruptions such as lichen planopilaris (LPP). CASE REPORT: We report a 75-year-old woman with stable, biopsy-proven LPP who developed disease reactivation after ten cycles of pembrolizumab for stage IIC melanoma. MANAGEMENT & OUTCOME: Despite ongoing treatment with hydroxychloroquine and topical clobetasol, disease activity progressed. Oral doxycycline was added. After ten months of adjunctive therapy and completion of pembrolizumab, the disease stabilized,

Nail: News in Onychopathology.

Onychopathology (nail unit pathology) is a niche area in dermatopathology. There is a lack of familiarity with nail unit pathology and basic anatomy in general among pathologists and dermatopathologists, because of the relative rarity of such specimens in practice, and the lack of exposure and teaching regarding these lesions in training. It is however a vibrant area of dermatopathology, with knowledge evolving rapidly. In this article, we explore new information in the analysis of melanocytic and epithelial lesions of the nail unit, as well as nail inflammatory disease.

Single-cell and spatial profiling reveal cDC2A-CXCL13+CD8+ T-epithelial cell crosstalk and cytotoxicity through TNFRSF9 in cutaneous and mucosal lichen planus.

Lichen planus (LP) is a chronic inflammatory disease of the skin and mucous membranes, marked by T cell infiltration and keratinocyte apoptosis. However, its immune microenvironment remains poorly understood. Using single-cell RNA sequencing, spatial transcriptomics, and proteomics on samples from 28 patients and 18 healthy controls, we identify elevated interferon (IFN) and cytotoxic signatures in CXCL13+CD8+ T cells in both cutaneous and mucosal LP, but not in lichen planopilaris. T cells expressing TNF and IFNG are spatially linked to epithelial cells through ligand-receptor interactions, c

Evaluation of IL-31 and IL-31RA Expression in Lichen Planopilaris: A Retrospective Immunohistochemistry Study.

BACKGROUND: Lichen planopilaris (LPP) is an autoimmune cicatricial alopecia leading to irreversible follicular destruction. Interleukin-31 (IL-31) and its receptor complex (IL-31RA/OSMRβ) are implicated in neurogenic inflammation and pruritus in inflammatory dermatoses, including lichen planus. This study aimed to evaluate IL-31/IL-31RA expression in LPP and to investigate their associations with disease extent and pruritus. METHODS: Thirty patients with classical-type LPP and 30 age- and sex-matched controls were retrospectively analyzed. Disease extent and pruritus severity were assessed us
