Biologic treatments for palmoplantar psoriasis and palmoplantar pustulosis: a systematic review and network meta-analysis.

The treatment of palmoplantar psoriasis (PPP), manifesting as hyperkeratotic plaques or pustular lesions (palmoplantar pustulosis -PPPP), is challenging. We aimed to evaluate the efficacy of various biologic treatments for PPP through a network meta-analysis. Systematic review and network meta-analysis of randomized controlled trials (RCTs) involving biologic treatments for adult patients with PPP or PPPP. Databases searched included PubMed, Embase, CENTRAL, Scopus, and trial registries (ClinicalTrials.gov, WHO ICTRP). The search was conducted from inception to May 2026. Studies comparing biol

Comparative drug survival of biologic classes in psoriasis: a Japanese single-centre cohort study.

Drug survival reflects the long-term effectiveness, safety, and tolerability of biologic therapies in routine clinical practice. To compare drug survival among currently available biologic classes in Japanese patients with psoriasis and to evaluate reason-specific discontinuation. We conducted a retrospective cohort study including 538 biologic treatment episodes from 311 patients treated at a tertiary dermatology centre in Japan. Tumour necrosis factor, interleukin-17, interleukin-23, and interleukin-12/23 inhibitors were compared. Overall drug discontinuation was assessed using multivariable

Distinct clinical profiles of methotrexate intolerance and hepatotoxicity in psoriatic arthritis.

INTRODUCTION: Methotrexate (MTX) is the most widely used conventional DMARD for psoriatic arthritis (PsA), but intolerance and hepatotoxicity often limit its long-term use. This study aimed to describe the prevalence and clinical correlates of MTX intolerance and hepatotoxicity in PsA patients. METHODS: We conducted a retrospective study on adults with PsA who were exposed to MTX. Demographic, clinical, and treatment data were retrieved from electronic medical records. Intolerance was defined as discontinuation or dose reduction due to adverse effects, while hepatotoxicity was defined as disc

Spatial Transcriptomic and CITE-Seq Analyses Identify Clearance-Defective Signatures in Psoriatic Disease.

Psoriatic lesions contain sustained inflammatory stress, but whether death-associated tissue activity is matched by adequate clearance capacity remains poorly defined. To address this, spatial transcriptomics was used to define clearance imbalance in psoriatic skin, with peripheral CITE-seq assessing circulating myeloid clearance states. Within leucocyte-rich tissue regions, stress-death and clearance-capacity modules identified clearance-competent niches, marked by matched stress and clearance activity and clearance-defective niches, marked by stress-death activity without comparable clearanc

[Atypical cutaneous manifestations of B19 parvovirus infection in children].

The most well-known skin manifestation of human parvovirus B19 infection is erythema infectiosum, also known as fifth disease. In atypical cases, particularly among young adults, purpuric-petechial glove-and-sock syndrome (PPGSS) is observed, which is a condition characterized by a symmetrical distribution in the acral areas. Its onset is attributed to the active viremia phase; therefore, patients exhibiting symptoms are contagious, which is of particular importance in the case of immunocompromised individuals and pregnant women. In our report, we present three cases: in two patients, the skin

Severe Acrodermatitis Continua of Hallopeau With Atypical Dermoscopic Features: A Diagnostic Pitfall.

Acrodermatitis continua of Hallopeau is a rare, chronic, relapsing form of localized pustular psoriasis characterized by painful sterile pustules and progressive destruction of the nail apparatus. Its clinical and dermoscopic features may be atypical, leading to diagnostic delays and inappropriate treatment. We present the case of a 78-year-old man with a one-year history of painful hyperkeratotic lesions affecting several fingers, with recent worsening, extension, and purulent discharge. Examination revealed bilateral chronic paronychia with peripheral erosions involving both thumbs and the l

Triple immunotherapy containing telitacicept for systemic lupus erythematosus complicated by multiple autoimmune diseases: a case report.

SLE may occur simultaneously with or subsequently to other autoimmune diseases; such cases are called overlap syndromes. Cases of SLE overlapping with three or more autoimmune diseases are extremely rare, and there is no unified standard or consensus for treatment. We report a clinical case of a patient with SLE, immune-mediated necrotizing myopathy, Sjögren's syndrome, and psoriasis. After treatment with a triple immunosuppressive regimen containing telitacicept, the patient achieved a satisfactory therapeutic response. As the first dual-pathway inhibitor targeting BLyS/APRIL, telitacicept de

H2A.J is an epigenetic driver of IL-17-associated inflammation in TH17 cells and keratinocytes.

Psoriasis is a common autoinflammatory disease driven by excessive IL-17 expression. While the pathomechanism of the disease has been revealed, epigenetic factors contributing to psoriasis are less well understood. Here, we investigated the role of the poorly studied histone variant H2A.J in psoriasis. We found that H2A.J gets overexpressed in the skin of psoriasis patients, with even non-lesional skin showing increased expression compared to the skin of healthy donors. To gain mechanistic insights, we studied mice with a global H2A.J knockout (KO) and found them to be partially protected agai

Janus kinase inhibitors in the management of lichen planus: a systematic review.

BACKGROUND AND OBJECTIVES: Lichen planus (LP) is a chronic, lymphocytic, inflammatory skin disorder that can affect the skin, hair follicles, nails, and mucous membranes. It has limited treatment options. The pathogenesis of LP includes inflammation driven by interferons and the Janus kinase-signal transducer and activator of transcription (JAK-STAT) signaling pathway. Thus, Janus kinase inhibitors (JAKi) have been proposed as a viable therapeutic approach for LP. JAKi have recently been approved to treat some lymphocytic skin disorders such as alopecia areata and atopic dermatitis. The purpos

The immunometabolic tipping point: forecasting the systemic shift from psoriasis to psoriatic arthritis.

Psoriasis (PsO) is a chronic immune-mediated inflammatory disease that progresses to psoriatic arthritis (PsA) in up to 30% of patients. The transition from localized cutaneous inflammation to systemic articular involvement represents a critical window for early intervention; however, accurately predicting which patients will develop PsA remains a significant clinical challenge. Growing evidence highlights immunometabolic dysregulation as a central driver of this systemic shift. The progression from PsO to PsA is fueled by a complex interplay between immune dysregulation-centered on the IL-23/

Multidimensional efficacy characterization of xeligekimab in moderate-to-severe plaque psoriasis: regional responses, treatment targets, and patient-reported outcomes from a randomized controlled trial.

BACKGROUND: Xeligekimab, a monoclonal antibody targeting interleukin-17A (IL-17A), has demonstrated high efficacy in treating moderate-to-severe plaque psoriasis. However, the detailed response patterns across anatomical regions, treatment targets, and patient-reported outcomes remain incompletely elucidated. METHODS: This post hoc analysis of a phase 3, randomized, double-blind, placebo-controlled trial (CHICTR2100043223) included 420 Chinese patients randomized 2:1 to receive subcutaneous xeligekimab 200 mg every 2 weeks or placebo for 12 weeks, followed by open-label xeligekimab through we

Longitudinal fatigue outcomes and prediction in inflammatory arthritis: a real-world digital cohort study.

Fatigue is a burdensome symptom in inflammatory arthritis (IA), yet meaningful longitudinal change remains poorly characterized in real-world settings. To characterize fatigue trajectories, quantify clinically meaningful improvement/worsening, and identify baseline predictors of fatigue outcomes in a digital IA cohort. We analyzed Midaia digital application data from patients with rheumatoid arthritis, psoriatic arthritis, axial spondyloarthritis, or mixed IA with baseline Brief Fatigue Inventory (BFI) data. Fatigue trajectories were modelled via cluster-robust longitudinal regression, with se

Umbilical Cord Mesenchymal Stem Cells as a Meritorious Option to Treat Psoriasis.

Psoriasis is a chronic immune-mediated disease that affects a significant percentage of the global population. The pathogenesis of psoriasis involves the rapid turnover of skin cells and immune system dysregulation, particularly the T cell-mediated autoimmune response. Conventional treatments for Psoriasis include topical therapy, light therapy (phototherapy), and systemic medications; however, some limitations and diverse side effects have been mentioned for their usage. Therefore, increasing attention is being directed toward finding alternative therapeutic methods for psoriasis. Recently, U

Salivary metabolic profile in patients with oral squamous cell carcinoma and potentially malignant disorders.

OBJECTIVES: Oral Potential Malignant Lesions (OPMDs), including oral leukoplakia (OLK) and lichen planus (OLP), as well as Oral Squamous Cell Carcinoma (OSCC) are influenced by oral dysbiosis and might represent a disease continuum. Our hypothesis was that the oral salivary metabolomic reflects the association of those diseases. Hence, we aimed to study and compare salivary metabolites and metabolic pathways in OPMDs and OSCC. MATERIAL AND METHODS: Unstimulated saliva samples were collected from 60 participants: OLK (n = 15), OLP (n = 15), OSCC (n = 15), and healthy controls (HCs; n = 15). Me
