Clinical characteristics, effectiveness, safety, and predictors of PASI75/90 responses to IL-17 biologics in psoriasis involving special sites: a large real-world cohort study revealing treatment response heterogeneity.

Psoriasis affecting special anatomical sites (scalp, face, intertriginous areas, and genitals) poses unique treatment challenges and often shows inadequate responses. This study evaluates the real-world efficacy and safety of IL-17 inhibitors in treating psoriasis affecting special sites, and identifies clinical determinants for achieving PASI75 and PASI90 responses. A retrospective cohort of 1,469 patients receiving IL-17 therapy was analyzed.Generalized linear models, univariate and multivariate logistic regression analyses were used to identify clinical factors influencing the PASI75 and PA

Genital lichen sclerosus with complete introital obliteration presenting with chronic urinary retention.

Lichen sclerosus (LS) is a chronic inflammatory dermatosis with predilection for the anogenital region. Progressive scarring may cause labial adhesions, introital stenosis and in advanced cases, complete obliteration with urinary obstruction. We report a postmenopausal woman with hypothyroidism who presented with chronic urinary retention due to LS-related introital fusion. Surgical adhesiolysis and urethral calibration restored normal voiding. This case underscores the severe sequelae of untreated LS, the value of early diagnosis and the importance of long-term follow-up to prevent recurrence

Cumulative clinical benefits of biologic treatments in patients with moderate-to-severe psoriasis: an Italian real-life experience - IL PSO (Italian Landscape Psoriasis).

BACKGROUND: Psoriasis is a chronic inflammatory disease associated with significant long-term cumulative life course impairment. Achieving rapid relief from psoriasis is a crucial aspect of an effective treatment, as it helps patients manage their daily discomfort, including pain, itching, embarrassment and other negative impacts of the disease. Cumulative clinical benefit, measured as the area under the curve (AUC) of Psoriasis Area and Severity Index (PASI) responses and days free of disease, offers a comprehensive assessment of treatment efficacy over time. OBJECTIVES: To evaluate the cumu

λ-CO, an oligosaccharide with anti-inflammatory properties.

Oligosaccharides that mimic heparan sulfate (HS) exhibit a variety of biological activities. One such oligosaccharide, λ-CO, a λ-carrageenan-derivative previously shown to inhibit cell migration, invasion, and heparanase 1 (HPA1), was evaluated for its anti-inflammatory properties. In LPS-stimulated macrophages, λ-CO significantly reduced Il-6 and Tnf-α production, as well as Erk and Akt phosphorylation, and Mmp-2/Mmp-9 secretion, independently of Hpa1 activity. In primary human keratinocytes stimulated with the psoriasis-mimicking M5 cytokine cocktail, λ-CO down-regulated cathepsin L and MMP-
