Atypical genital mpox mimicking a sexually transmitted infection with secondary Staphylococcus aureus infection: A case report.

INTRODUCTION: Mpox is an Orthopoxvirus infection with evolving clinical presentations, including genital-dominant disease that can resemble common sexually transmitted infections (STIs). Secondary bacterial superinfection is uncommon but may alter lesion morphology and delay diagnosis. CASE PRESENTATION: A 64-year-old unhoused heterosexual male presented with a two-week history of painful, malodorous groin ulcers after unprotected intercourse. Lesions began as groin-confined vesicles and later spread to the face and peri-auricular area. Because of ulceration, purulence, and sexual exposure, i

Mpox in US Immigration and Customs Enforcement Detention Facilities during 2022 Global Outbreak.

US Immigration and Customs Enforcement (ICE) detention centers are hot spots for infectious disease spread, as manifested during the COVID-19 pandemic. Mpox cases among ICE detainees during the 2022 global outbreak were investigated and a Freedom of Information Act request for mpox testing data from January to November 2022 was submitted, focusing on demographic and clinical information. Seven detainees across five facilities in Texas, Arizona, and New Jersey were tested. Two detainees tested positive, revealing a 28.6% positivity rate. One detainee received antiviral treatment. Overall, few m

Transmission of MPXV from fire-footed rope squirrels to sooty mangabeys.

Mpox, caused by the monkeypox virus (MPXV; Orthopoxvirus monkeypox), is on the rise in West and Central Africa1-3. African rodents, especially squirrels, are suspected to be involved in MPXV emergence, but no evidence of a direct transmission to humans or non-human primates has been established4-9. Here we describe an outbreak of MPXV in a group of wild sooty mangabeys (Cercocebus atys) in Taï National Park (Côte d'Ivoire). The outbreak affected one-third of the group, killing four infants. To track its origin, we analysed rodents and wildlife carcasses from the region. We identified a MPXV-in

Enhanced virulence of mpox virus clade Ib over clade IIb in the CAST/EiJ mouse model.

The recently emerged clade Ib of mpox virus (MPXV) is spreading rapidly across Central and West Africa raising concerns about its potential virulence. Similar to clade IIb lineage B.1, which was responsible for the 2022 global outbreak, clade Ib exhibits sustained human-to-human transmission and a pattern of APOBEC3-associated genomic mutations. Here, we show that clade Ib displays enhanced long-range spreading in cell culture compared to clade IIb. Additionally, using the CAST mouse model, we show that clade Ib exhibits a higher level of virulence compared to the markedly attenuated clade IIb

Monkeypox in Healthcare Settings: A Narrative Review of Transmission, Exposure, and Protection Among Healthcare Workers.

Monkeypox, caused by an Orthopoxvirus, poses a significant occupational risk to healthcare workers due to their frequent contact with infected patients and contaminated materials. This risk is increased by potential lapses in infection control protocols in hospitals, such as a lack of personal protective equipment (PPE), improper hand hygiene, and other issues, making hospitals a key site for occupational transmission of monkeypox. The objective of the study was to evaluate monkeypox transmission among healthcare workers and identify critical preventive strategies. A comprehensive literature s

Surveillance and response to Mpox control in Ethiopia: an urgent health threat; an editorial.

BACKGROUND: Mpox (formerly monkeypox) has emerged as a significant public health challenge in sub-Saharan Africa. The recent confirmation of Mpox cases in the Moyale region of Ethiopia highlights the urgent need for robust surveillance and response mechanisms in the Horn of Africa. POLICY GAPS: While Ethiopia has a strong history of epidemic control, specific gaps remain in diagnostic capacity for orthopoxviruses in remote border regions, and there is a need to adapt current strategies to resource-limited settings. RECOMMENDATIONS: This editorial advocates for integrating Mpox detection into

Insights into Genomic Dynamics and Plasticity in the Monkeypox Virus from the 2022 Outbreak.

The 2022 global mpox outbreak represented a turning point in the Monkeypox virus (MPXV) epidemiology, highlighting the incredible capability of this virus to adapt to different conditions, also in a non-endemic context. To investigate the genomic dynamics of MPXV 2022 strains, we performed whole-genome sequencing of 40 clinical samples from 16 Italian patients across multiple anatomical sites and timepoints between May and December 2022. Combining single-nucleotide analysis with detailed investigation of short tandem repeats (STRs), we explored inter- and intra-host viral dynamics. We identifi

Multi-epitope vaccine design against Monkeypox virus: An immunoinformatics approach.

BACKGROUND: The recent outbreak of the Monkeypox virus (MPXV) across multiple regions has highlighted the need for effective vaccine. This study employed an in-silico immunoinformatics approach to design a multi-epitope vaccine against monkeypox virus. METHODS: The IMV heparin binding surface protein (H3L) of MPV was selected as vaccine target. B and T cell epitopes were predicted and evaluated for their toxicity, allergenicity, antigenicity and global population coverage. A multi-epitope construct was assembled using adjuvant and appropriate linkers. The 3D vaccine structure was modelled, en

Mpox comprehensive assessment for responsive immunisation in emergency outbreaks (MpoxCARE): study protocol.

BACKGROUND: Mpox disease is endemic to several regions of sub-Saharan Africa, with transmission occurring through close contact with infected animals or humans. Increasing case numbers in East Africa led the African Centre for Disease Control and Prevention to declare a Public Health Emergency of Continental Security on 13th August 2024 related to a new virus variant (clade 1b). The Mpox CARE study aims to establish clinical and epidemiology data to develop innovative immunodiagnostic tests, facilitating models for sub-clinical disease exposure, transmission and future vaccine strategies (Part

Mathematical modeling and transmission insights into Mpox: dynamics, control measures, and data-driven validation.

This research presents a detailed mathematical framework designed to explore the transmission patterns of Mpox. It encompasses a variety of transmission pathways, notably human-to-human interactions and potential zoonotic transmissions. Our analysis identifies critical equilibrium points within the model and computes effective reproduction numbers: R 0 P for human-to-human spread and R 0 R for contributions from animal reservoirs. We conduct a detailed stability analysis to uncover the specific conditions that facilitate disease emergence and ongoing transmission. The global stability of t

Mapping transcriptional patterns of MPXV in human epithelial cells.

The 2022 global outbreak of the monkeypox virus (MPXV) clade IIb highlighted changes in transmission dynamics, clinical features, and tissue tropism, yet the impact of host cell origin on viral transcription remains unclear. We performed comparative transcriptomic profiling of MPXV clade IIb in human epithelial cell lines from vaginal, intestinal, ectocervical, and renal tissues. Temporal analysis in vaginal cells revealed two major transcriptional clusters: early genes enriched for host-virus interaction factors in terminal genome regions, and intermediate-late genes encoding transcription, r

From Surge to Stability: Navigating MPOX Epidemics Across the Globe With Genomic Insights, Paving the Way for Effective Management Plan.

MPOX has been declared endemic in Central Africa by WHO in 2017. More recently (2018-2021), MPOX cases have been reported in various regions (endemic as well as non-endemic) all over the world. The sudden, unexpected and simultaneously emerging cases at geographically disparate areas indicate the probable undetected transmission pattern of the disease. The double stranded DNA virus has evolved over the time into multiple sub-lineages. Clade I MPXV variant is considered to be more virulent as compared to clade II variety. Mutations in certain sub-lineages may alter transmission routes, potentia
