Physical urticarias and cholinergic urticaria.

Physical urticarias are a unique subgroup of chronic urticaria in which urticarial responses can be reproducibly induced by different specific physical stimuli acting on the skin. These conditions include urticaria factitia/symptomatic dermographism, delayed pressure urticaria, cold contact urticaria, heat contact urticaria, solar urticaria, and vibratory urticaria/angioedema. Physical urticarias and cholinergic urticarias are diagnosed based on the patients' history and provocation tests including trigger threshold testing where possible. Treatment is mainly symptomatic. Many patients benefit

Physical urticaria.

The physical urticarias are a heterogeneous subgroup of chronic urticarias in which wheals can be reproducibly induced by different specific physical stimuli such as cold, heat, pressure, vibration, or sunlight. Physical urticarias comprise up to 25 % of chronic urticarias and occur more frequently in young adults. Symptoms, i.e. wheal and flare responses or angioedema, are usually limited to the skin areas exposed to the eliciting stimulus. However, generalised urticaria with variable extracutaneous manifestations can also occur. Some patients may also present with more than one physical urti

Food-dependent exercise-induced anaphylaxis occurred only in a warm but not in a cold environment.

Food-dependent exercise-induced anaphylaxis (FDEIA) is a type of exercise-induced anaphylaxis associated with postprandial exercise. We describe a 19-year-old man with FDEIA. Our patient complained of urticaria, angioedema, dizziness and hypotension associated with exercise after ingestion of walnut-containing foods in a warm environment. Skin prick test and prick to prick test were positive for walnut antigen. The attack didn't occur by free running outside for 10 min 2 h after taking walnuts, and the temperature was about -2℃. Food-exercise test was done again in a warm environment based on

Bedbug infestation.

The significant resurgence of bedbugs in the past decade has been attributed to pesticide resistance, more frequent travel, lack of public awareness, and inadequate pest control programs. Bedbugs are obligate blood parasites (insect family Cimicidae). They can withstand a large range of temperatures but are attracted to warmth and carbon dioxide. They typically feed just before dawn. Cutaneous reactions to bedbug bites can include macules, papules, wheals, vesicles, bullae, and nodules. Bites may be confused with other skin conditions. Bedbug bite reactions are typically self-limited and resol

Anti-immunoglobulin E treatment of patients with recalcitrant physical urticaria.

In physical urticaria, exogenous physical factors such as thermal triggers, solar radiation and mechanic triggers including friction or pressure are responsible for the elicitation of symptoms in the skin of patients. Avoidance of the respective stimulus is usually difficult or impossible, and many patients are not sufficiently treated with standard antihistamines. We report that treatment with omalizumab (Xolair®) of 7 patients with physical urticarias [solar urticaria (n = 2), urticaria factitia/symptomatic dermographism (n = 2), cold urticaria, delayed pressure urticaria and localized heat

Localized heat induced urticaria: report of a case.

Localized heat induced urticaria is a rare clinical entity. Other physical urticarial subtypes include cholinergic, solar, cold, aquagenic, vibratory, and dermatographic. It is characterized by a well-demarcated urticarial lesion provoked by heat in direct contact with the skin. We describe a case of localized heat-induced urticaria in a 49-year-old woman after a heat-challenge test to her forearm.

[Physical urticaria].

The different types of physical urticaria are triggered by mechanical and thermal stimuli, as well as electromagnetic waves. Localized forms restricted to the skin and mucous membranes are most common, but generalized urticaria with variable extracutaneous manifestations can also occur. Physical urticaria is usually sporadic but may rarely have a familial form; it is often associated with chronic urticaria. In most instances, the short time interval between the physical stimulus and reaction points to a causal relationship, but in delayed types the exact diagnosis may be missed without provoca

Physical urticaria.

Physical urticarias are a unique subgroup of chronic urticaria in which patients develop urticaria secondary to environmental stimuli. Common triggers include exercise, temperature changes, cold, heat, pressure, sunlight, vibration, and water. Systemic symptoms have occurred during severe episodes. Physical urticarias are responsible for approximately 20% to 30% of all cases of chronic urticaria. A basic knowledge of these unusual disorders is important for all healthcare providers. This article covers the following types of physical urticarias: dermatographism, cholinergic urticaria, local he

Detection of a highly toxic clone of Actinobacillus actinomycetemcomitans (JP2) in a Moroccan immigrant family with multiple cases of localized aggressive periodontitis.

The bioactive phyto-components of M. charantia L. demonstrate significant therapeutic potential against the acetylcholinesterase (AChE) enzyme, which is associated with neurodegenerative diseases such as Alzheimer's. In this study, computational tools were used to screen the essential bioactive compounds of M. charantia L. against AChE. Docking results revealed that among the 19 phytocompounds analysed, Ajmalacine, Alkaloid AQC2, Alkaloid SP-K, Steroid U, and Quinine exhibited high binding affinities towards AChE. Ligand-protein binding interactions indicated that these selected compounds show

[Physical urticarias].

Ten percent of chronic urticarias are physical urticarias. Patients suffering from physical urticaria all have a suggestive history with specific eliciting stimuli (cold, heat, water, sun.) and wheals in the areas where the stimulus acts. The involved pathomechanisms are not well known. An unknown allergen (related to a cold or a heat injury, a polar molecule contained in the stratum corneum and soluble into water, a photoallergen) could induce a mast cell mediator release, followed by an infiltration by eosinophil then neutrophils polymorphonuclears. T-cells are not highly involved. Dermograp

Pathomechanisms in physical urticaria.

Various exogenous stimuli, e.g., rubbing, pressure, cold, heat, or electromagnetic waves, have been described to elicit whealing reactions, the so-called physical urticarias. They may occur as isolated diseases or in association with other types of urticaria. In many cases, the respective physical factors can be defined exactly, e.g., the degree of temperature changes or the range of eliciting ultraviolet wavelengths. In contrast, the underlying pathomechanisms are mostly still obscure. In the past, often contradictory results have been reported regarding the role of IgE, complement factors, h

[Round Table: urticaria with a physical cause].

Physical urticaria comprises a series of clinically differentiated conditions in which wheals develop as a result of physical stimuli: mechanical (friction, pressure), cold, heat, increased body temperature, exercise exposure to sunlight, vibration and contact with water. The present study reviews their clinical, diagnostic and treatment characteristics. Although the precise prevalence of physical urticaria is not know, it nevertheless represents the most common form of urticaria of known etiology in children. Symptomatic dermatographism is the most frequent presentation in children; it may be

The natural history of exercise-induced anaphylaxis: survey results from a 10-year follow-up study.

BACKGROUND: Exercise-induced anaphylaxis (EIA) is a unique physical allergy that is triggered by exertion, the clinical spectrum and modifying factors of which have been previously studied. At the time of initial description, it was postulated that other factors contributed to this disorder. OBJECTIVE: We sought to determine the clinical course and potential modifying factors in EIA. METHODS: In 1993, we conducted a cross-sectional analysis of 671 individuals with exercise-associated symptoms for more than a decade using a validated 75-item questionnaire. Subjects met criteria for EIA if the

Cold exposure and immune function.

The influence of cold exposure on immune function is reviewed. Data obtained mainly on small mammals suggest that the acute effect of severe chilling is a suppression of several cellular and humoral components of the immune response, including a decrease of lymphocyte proliferation, a down-regulation of the immune cascade, a reduction of natural killer (NK) cell count, cytolytic activity, activation of complement, and the induction of heat shock proteins. However, adaptation to a given cold stimulus appears to develop over the course of 2-3 weeks. Further work is needed to examine interactions

Transient hepatocellular injury during attacks of cholinergic urticaria.

Cholinergic urticaria presents as wheals and erythroderma that develop in response to a variety of factors which stimulate muscarinic receptors, including exercise, heat, cold, sweat and emotional stress. We describe a 25-year-old man with ulcerative colitis who developed cholinergic urticaria diagnosed by a metacholine test. He had had seven previous attacks over 8 years, and the finding of elevated liver enzymes required admission to four different hospitals. The clinical picture was identical: urticaria, hepatosplenomegaly, lymphadenopathy and elevation of liver enzymes. The causative agent

[Epidemiology and clinical aspects of cold urticaria].

To study the frequency and clinical aspects of cold urticaria in Central Europe, patient data from a university dermatology clinic and a private dermatology office between 1984-94 were analysed and the patients re-examined if possible. The incidence of cold urticaria was found to be 0.05%. Of the 56 patients with cold urticaria (31 women, 25 men), 49 had idiopathic cold urticaria. The mean age was 41.0 +/- 15.6 year, the mean duration of disease 7.9 +/- 5.8 years. Atopy was found in 46.5% of patients, and 23.2% of the patients suffered from other types of urticaria (cholinergic, chronic idiopa
