Twenty patients with cholinergic urticaria, 11 sex- and age-unmatched patients with acute or chronic urticaria and 20 sex- and age-matched non-atopic control subjects were skin tested with autologous sweat. All cholinergic urticaria patients showed positive immediate-type skin reactions at 2(0)-2(9) dilutions of sweat (geometric mean of maximal positive dilutions +/- S.D. was 2(4.6 +/- 2.6), while in acute and chronic urticaria only three patients showed positive reactions at low dilutions (two 2(0) and one 2(1)) and none of 20 controls showed positive reactions. Prausnitz-Küstner (P-K) tests

Chronic urticaria can be produced by a number of stimuli that cause mast cell mediator release. Patients with urticaria caused by physical agents account for roughly one-fifth of all cases of chronic urticaria. There are about 20 different types of physical urticaria. Two forms, dermographism and cholinergic urticaria, are quite common and represent more than two-thirds of all cases of physically caused urticaria. More than one agent may precipitate urticaria in a given individual. Urticarial response can be easily reproduced in the sensitive patient and, generally, lasts less than one hour. S

BACKGROUND: Cholinergic urticaria is primarily seen in young adults, but little is known about its prevalence.
OBJECTIVE: A prospective study was designed to investigate the frequency and severity of cholinergic urticaria in this age group.
METHODS: Four hundred ninety-three high school and university students (15 to 35 years of age) were examined for cholinergic urticaria by means of a questionnaire and partly by exercise provocation tests.
RESULTS: The overall prevalence of cholinergic whealing was 11.2%, with most of the affected persons being older than 20 years. The highest prevalence

Urticaria is a common pathology, yet, urticaria induced by physical agents represent 17% of urticaria cases, they are more frequent in adolescents and young adults. Exercise may induce anaphylaxis, urticaria, asthma and rhinitis. This first two pathologies are not so usual. The incidence of cholinergic urticaria (CU) is five to seven percent. this incidence of anaphylaxis induces by exercise has not been described. It is very difficult to clinically distinguish CU and EIA, establish criteria are described in the paper. There are two clinical forms of CU; local and systemic. The ideal diagnosti

BACKGROUND: The incidence of delayed pressure urticaria (DPU) may have been underestimated, particularly in patients with widespread wheals of concurrent idiopathic urticaria, because of difficulty in recognizing the association between pressure-related swellings and the preceding physical stimulus. Diagnostic tests for DPU have not been done routinely in studies of urticaria and have not been compared.
OBJECTIVE: Our purpose was to establish the incidence of DPU and other physical urticarias in patients with chronic urticaria and to compare two reproducible pressure challenge tests.

There are several allergic responses that may occur in susceptible individuals as a result of exposure to physical stimuli. Most of these conditions are mediated by vasoactive substances and usually result in symptoms of urticaria and/or angioedema. There are 2 such conditions that may occur as a direct result from exercise. The first of these is cholinergic urticaria. Patients with cholinergic urticaria experience punctate (2 to 4mm) hives which occur reproducibly with exercise or with passive warming, such as might occur in a steam bath or hot pool. Life-threatening hypotension or angioedema

Urticaria and physical angioedema frequency is hard to evaluate in children. In this series, we keep 53/1000. Physical urticaria is found in 36% of the cases and concerns cholinergic urticaria, urticaria to cold, idiopathic dermographism and mastocytosis, and delayed urticaria to pressure. The authors insist on the interest of the etiologic research in these forms of infantile urticaria.

An 8-year-old gelding, with a 1-year history of intense pruritus only after exercise, was diagnosed as having cholinergic pruritus. Provocative testing, using exercise and hot-water baths to increase core body temperature, assisted in the diagnosis. Cholinergic pruritus in human beings is a variant of the more common syndrome, cholinergic urticaria, which is characterized by intense pruritus and pinpoint urticaria. With cholinergic pruritus, intense itching results without urticaria. Specific diagnostic laboratory tests have not been developed.

At a time when the American public is committed to health through exercise, a variety of allergic conditions related to activity are being recognized with increasing frequency. Conditions such as cholinergic urticaria, bronchospasm, and even anaphylaxis have occurred as a consequence of exercise. Thus, a symposium has been developed to examine the role of exercise in association with such conditions. Several of the physical allergies are discussed in overview fashion. Exercise-induced anaphylaxis, the most serious form of allergic response to exercise itself, is considered in greater detail. T

Aquagenic urticaria is a rare form of physical urticaria characterized by small follicular wheals with an erythematous flare upon exposure to water. We describe typical lesions in a seven-year-old boy with aquagenic urticaria, cholinergic urticaria, and symptomatic dermatographism, who responded to treatment with ultraviolet B and oral antihistamines.

Allergic responses that occur as a result of exposure to physical stimuli are discussed. Most of these conditions are mediated by vasoactive substances, resulting in urticaria and/or angioedema. Susceptible individuals who engage in athletic activities may place themselves at particular risk for these problems. The physical allergies include cholinergic urticaria, exercise-induced anaphylaxis, cold urticaria, dermatographism, solar urticaria, and aquagenic urticaria. Management of these conditions includes patient education, selective avoidance, antihistamines, and, in some cases, induction of

Five patients with exercise-induced anaphylactoid reactions are reported. Because of a growing interest in physical exercise and the severity of the symptoms it is important to recognize this condition, even though rare. All of our 5 patients had a history of urticaria and anaphylaxis in association with physical stress, but it seems difficult to induce anaphylactoid reactions under laboratory conditions. Two different clinical patterns could be distinguished in these patients. Three had the anaphylactoid form with signs of alternative complement pathway activation, while 2 patients had the va

Eleven patients with food-dependent, exercise-induced anaphylaxis were studied. Seven patients experienced anaphylactic symptoms only after eating certain foods, such as shellfish, wheat, and grape before exercise. In the remaining four patients, no specific food could be identified, but the act of eating itself predisposed to anaphylaxis. Their anaphylactic symptoms were all clearly distinguished from cholinergic urticaria by history. Patients who developed anaphylactic symptoms before 20 years of age (N = 7) were atopic themselves or had atopic first-degree relatives. Six patients had increa

We report a patient who has an immediate wealing response to pressure in the absence of symptomatic dermographism, which we designate 'immediate-pressure urticaria'. Our patient also suffered from cholinergic urticaria and chronic idiopathic urticaria in the past, demonstrating the clustering of urticarias which frequently occurs.

The levels of tryptase in the suction-blister fluid from patients with chronic urticaria, urticaria pigmentosa, cholinergic urticaria, urticarial dermographism, prurigo of unknown origin, eczema, psoriasis, atopic dermatitis, and from healthy controls were studied. The blister fluid from controls contained up to 15 micrograms/l of tryptase, whereas that from patients with active urticaria contained greater than 50 micrograms/l. This study demonstrates that patients with urticaria have mast cells that readily release tryptase in both the lesional and non-lesional areas of skin.

The clinical features of exercise-induced anaphylaxis in 5 patients are described, with an accurate history, skin tests, IgE determinations and exercise tests. In these 5 patients we were able to distinguish three different forms of exercise-induced disease: exercise-induced anaphylaxis based upon an IgE-mediated allergy to foods, exercise-induced anaphylaxis without food allergy and cholinergic urticaria.

Exercise-induced asthma is a common but frequently undiagnosed problem. The patient may not wheeze, but rather have shortness of breath, chest tightening, and coughing. The coach and the physician must be particularly alert to the signs and symptoms of exercise-induced asthma to recognize this syndrome. Proper conditioning, warming up, inducing refractoriness, participating in sports less likely to provoke exercise-induced asthma, and the aggressive use of appropriate medications allow patients to enjoy sports and compete effectively. A rare but potentially fatal syndrome is exercise-induced a

We report here on 2 patients with adrenergic urticaria and adrenergic pruritus, respectively. The lesions and features developed during phases of stress and during the attacks were associated with an increase in the plasma concentrations of noradrenalin, adrenalin and prolactin. The dopamine plasma level was elevated only in the case of adrenergic urticaria. The symptoms could be reproduced by intradermal injection of adrenalin and noradrenalin and treated successfully with propanolol, a blocker of beta-adrenergic receptors. Adrenergic urticaria is a rare but distinct entity, which has to be s

Muscarinic acetylcholine receptors were studied in slices of unaffected skin and in lymphocyte membranes from 3 patients with cholinergic urticaria and 5 healthy subjects using receptor autoradiography and the radioligand [3H]quinuclidinyl benzilate binding technique. Autoradiography revealed that muscarinic acetylcholine receptors were located in regions with sweat glands. The distribution of receptors did not differ qualitatively between patients and controls. However, the maximum number of muscarinic acetylcholine receptors was reduced in skin from patients with cholinergic urticaria, by 44
