Persisting cholinergic erythema: a variant of cholinergic urticaria.

A new variant of cholinergic urticaria is described. Four patients each had a similar persistent macular skin rash distributed maximally over the upper limbs and upper trunk. Though the rash was persistent, individual macules were of short duration but new macules continually appeared at adjacent sites. Exercise and hot baths exacerbated pruritus and provoked lesions in previously unaffected areas. Topically applied benzoyl scopolamine blocked the appearance of the lesions after challenge. Tests of cholinergic function were normal, apart from an exaggerated pupillary response to arecoline in o

[Chronic urticaria. Treatment with ketotifen].

Ketotifen inhibits the Ca++ pass through the mastocyte and basophil membrane. So inhibit histamine and S. R. S. A. liberation. Results in the treatment with Ketotifen of 36 cases of chronic Urticaria are exposed. Patients between ten and fifty years old, and with symptoms chronicity during from two until ten years, were studied. Chronic cases of unknown etiology, cholinergic urticaria, salicylates urticaria, urticaria by pressure, local heat urticaria and chronic dermographism were treated. Results were hopeful. Especially in patients with cholinergic urticaria, urticaria by heat, salicylates

Reflex heating of the skin and telethermography.

Thermography is a very instructive method of showing the interaction between direct changes in heat radiation of the surface of the skin and its relation to reflex processes. Short reflex arcs, such as the axon reflex, and long reflex arcs, such as the flush phenomenon provoked by visceral cutaneous reflexes, can be illustrated by thermography. Reflex changes in skin temperature by cooling the skin of patients with cold contact urticaria with the short-term development of large areas of warmed skin and also reflex changes in cholinergic urticaria after prior heating of such skin areas where th

Exercise-induced anaphylaxis as a manifestation of cholinergic urticaria.

Two patients presented with a history of exercise-induced hypotension associated with severe pruritus and either generalized urticaria or facial angioedema. Each patient was exercised under controlled conditions with use of bicycle ergometer exerciser (900 KPM/min) for 20 to 30 min at 23 degrees C. Each patients complained of generalized pruritus and then erupted in lesions typical of cholinergic urticaria. In one patient the lesions became confluent about the face and were followed by eyelid edema, lip swelling, and transient hypotension. Plasma histamine levels were elevated in each patient

Cholinergic urticaria with associated angio-oedema.

Three patients are described who developed angio-oedema and urticaria after a hot bath challenge and physical exercise. In one patient this was accompanied by a fall in peak expiratory flow rate. Blood levels of smooth muscle biological activity, measured using a bio-assay, increased in all patients after experimental challenge. The association of angio-oedema with cholinergic urticaria is emphasized and possible mechanisms discussed.

Exercise-induced urticaria, angioedema, and anaphylactoid episodes.

Six subjects with exercise-induced anaphylactoid symptoms were evaluated by exercise challenge. Five of the six patients developed symptoms during free running. One subject developed only periorbital angioedema; another developed giant urticaria, wheeze, and hypotension; and three subjects developed cholinergic urticaria. One of the subjects with cholinergic urticaria also became hypotensive. Both of the subjects who developed hypotension also exhibited elevations in plasma histamine (11.2 and 23.2 ng/ml). Subjects who exhibited only cutaneous or subcutaneous manifestations failed to develop e

Identification of a new physically induced urticaria: cold-induced cholinergic urticaria.

Four patients with symptoms suggestive of either cold urticaria or a combination of cold and cholinergic urticaria were studied. However, all patients were negative to an ice-cube test or cold-immersion test and had no urticaria after exercise in a warm environment. When each patient was seated in a cold room (4 degree C) for 5 to 15 min, generalized urticaria appeared, consisting of puncture wheals and surrounding erythema as seen in cholinergic urticaria. Two patients had weakly positive methacholine skin tests and the other two had completely negative tests. When serial venous blood samples

Evaluation of a patient with both aquagenic and cholinergic urticaria.

An 11-yr-old girl presented with a history of urticaria induced by warm or cool showers, exercise, and emotional stimuli. During evaluation she repeatedly developed generalized punctate urticaria, pruritus, palpitations, and headaches after warm baths or exercise, and she had a positive methacholine skin test. She developed similar lesions and pruritus after local application of sterile water, tap water, ethanol, normal saline, or 3% saline. The diagnosis of combined aquagenic and cholinergic urticaria was made and presented a unique opportunity to study and compare mediator release and clinic

Cholinergic urticaria. A seasonal disease.

Nine young patients with cholinergic urticaria reported that the occurrence of the disease was restricted to winter. They were examined in a climatic room in winter and summer under three climatic conditions in each season (cold, comfort, and severe heat). All subjects experienced severe phenomena of cholinergic urticaria during the winter season when tested under comfortable climate and under severe heat load. No phenomena were elicited in the summer under any of the three climatic conditions or under cold climatic condition in the winter. This finding led to the assumption that symptoms appe

Histamine release and therapy of severe dermatographism.

A case of unusually severe dermatographism is reported in which baseline plasma histamine levels were repeatedly elevated and histamine release into the circulation was demonstrable upon challenge by scratching the skin. Passive transfer was positive when the patient was challenged after a 48-hr interval, and the responsible factor was heat labile consistent with an IgE-dependent mechanism. Five additional patients with severe dermatographism had elevated baseline plasma histamine levels, but an augmented response upon stroking the skin was not evident. The response of this latter group to ant

Mediator release in local heat urticaria.

We described the sixteenth reported case of local heat urticaria, in a 59-yr-old woman with erythema and angioedema upon contact with hot water or outdoor heat exposure. Immersing her hand in 39 degrees to 40 degrees C heated water resulted in an erythematous, angioedematous response sharply demarcated by the line of immersion and was associated with immediate increases in histamine concentration (18 to 135 ng/ml) and high molecular weight neutrophil chemotactic activity (two to five times prechallenge levels) in venous blood draining the challenge site. We suggest that the local heat urticari

Negligible potency of acetylcholine in increasing vascular permeability.

As acetylcholine (ACh) is claimed to be a possible mediator of cholinergic urticaria, the potency of acetylcholine (ACh) in increasing vascular permeability was tested by skin perfusion technique in rats and by dye extraction method from the skin of rabbits. The potency of vascular permeability increase of ACh was much lower than that of histamine (Hist) and bradykinin (BK) in rats and rabbits. The higher dose of ACh (10 mg in 0.1 ml of saline), however, produced slight increase of vascular permeability in rabbits, which was inhibited by the antihistamine, pyrilamine maleate. Furthermore, ACh

Release of mast-cell mediators and alterations in lung function in patients with cholinergic urticaria.

Cholinergic urticaria was elicited in seven subjects by experimental challenge that consisted of running on a treadmill in a plastic occlusive suit. A sensation of generalized warmth of the skin was followed by pruritus, erythema, urticaria, and transient respiratory-tract symptoms consisting of shortness of breath or wheezing or both. Statistically significant falls in one-second forced-expiratory volumes (FEV1), maximal midexpiratory flow rates (MMF), and specific conductance (SGaw) and a rise in residual volume were detected. The serum histamine concentration rose, with an augmentation of e

Protease inhibitor profiles in urticaria and angio-oedema.

Plasma levels of six protease inhibitors have been measured in patients with chronic urticaria, chronic urticaria with angio-oedema, cold and cholinergic urticaria. In chronic urticaria C1 esterase inhibitor activity was increased compared with a reference control population but there was no detectable abnormality of any other protease inhibitor. Patients with chronic urticaria/angio-oedema showed a reduction in inter-alpha trypsin inhibitor. They also manifested a rise in C1 esterase inhibitor. In cold urticaria there was a significant lowering of alpha 1 antichymotrypsin. The reduction in al

Exercise-induced anaphylaxis.

Sixteen patients were seen because of possibly life-threatening exercise-associated symptoms similar to anaphylactic reactions. Asthma attacks, cholinergic urticaria and angioedema, and cardiac arrythmias are recognized as exertion-related phenomena in predisposed patients but are distinct from the syndrome described here. A syndrome characterized by the exertion-related onset of cutaneous pruritus and warmth, the development of generalized urticaria, and the appearance of such additional manifestations as collapse in 12 patients, gastrointestinal tract symptoms in five patients, and upper res

Experimental investigations on the trigger mechanism of the generalized type of heat and cold urticaria by means of a climatic chamber.

The physical conditions of challenge were investigated in a climatic chamber on 8 patients with cholinergic urticaria and 10 patients with generalized cold urticaria. The cholinergic urticaria was induced by passive or active (physical exercise) heating and psychological stimulation; the generalized cold urticaria was induced by general cooling at rest and during physical effort. The ambient temperature was varied, and the mean body temperature was recorded continuously at different measuring points. The experiments revealed that in both types of urticaria the physical trigger mechanism seems

Physical urticaria/angioedema: an experimental model of mast cell activation in humans.

Urticaria and angioedema may occur in skin and mucus membranes when mast cells are activated by various physical stimuli, including trauma, pressure, vibration, light, cold, heat, and (in rare cases) water. Experimental challenge of patients with cold-induced and cholinergic urticaria/angioedema in particular provides an in vivo model of mast cell activation in humans. This model synthesizes observations of the evolution of clinical manifestations, histologic analysis of tissue alterations, measurement of mediators released into the circulation, and assessment of leukocyte motility. The model
