Bridging Resource Gaps in Toxic Epidermal Necrolysis Management: A Case Report Highlighting Innovative Care.

Toxic epidermal necrolysis (TEN) is a rare but potentially life-threatening dermatological condition. It is characterised by diffuse exfoliation of >30% of the skin and mucous membranes due to immune-mediated destruction of the epidermis. It is thought to be due to a hypersensitivity reaction to a variety of medications. The mainstay of treatment involves withdrawal of the potential offending agent, supportive care and use of non-adherent dressings. Pre-packaged dressings are expensive or not readily available in our locality; hence, the need for improvisation, to manage the condition and impr

Ocular outcomes of amniotic membrane transplantation in acute and chronic Stevens-Johnson syndrome and toxic epidermal necrolysis: a systematic review.

PURPOSE: Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) can cause devastating systemic injury and ocular surface destruction. This systematic review aims to synthesize current clinical evidence on ocular outcomes following amniotic membrane transplantation (AMT) in patients affected by SJS/TEN in the acute and chronic phases of disease and to provide clarity on the efficacy of standard and novel surgical techniques. METHODS: We performed a systematic literature search in PubMed, Embase, Cochrane, Scopus, and Web of Science using a standardized search strategy. Screening,

Carbamazepine activates private self and viral reactive TCRs through a drug-permissive HLA-B cleft.

INTRODUCTION: Carbamazepine (CBZ) can cause severe cutaneous adverse reactions, including Stevens-Johnson syndrome/toxic epidermal necrolysis, and is strongly associated with HLA risk alleles. However, HLA genotype alone incompletely predicts clinical risk. Adverse drug reactions depend on drug-peptide-HLA conformation, immuno-regulatory cell interactions and the presence of drug-specific TCR clonotypes in susceptible patients. An unresolved question is whether patients that develop a specific drug reaction harbor the same drug-specific TCR (public TCR) or generate patient unique TCRs (private

The Use of Systemic Immunomodulating Drugs and Management Practices in Stevens-Johnson Syndrome and Toxic Epidermal Necrolysis among Dermatologists in the Philippines: A Cross-sectional Survey Study.

UNLABELLED: Stevens-Johnson syndrome and toxic epidermal necrolysis (SJS/TEN) are rare, life-threatening cutaneous adverse drug reactions with potential morbidity and mortality. Despite increasing use of systemic immunomodulatory therapies, consensus regarding optimal medical management remains lacking, particularly in resource-limited settings. We evaluated management practices and treatment preferences for SJS/TEN among board-certified dermatologists in the Philippines using a clinical vignette-based (CV) survey approach. METHODS: A cross-sectional online survey was conducted among Philippi

Biologics and Small Molecule Inhibitors: Novel Therapeutic Strategies for Cutaneous Adverse Drug Reactions.

Cutaneous adverse drug reactions (cADRs) are unintended and harmful skin responses to medications that impose significant clinical and economic burdens worldwide. The increasing use of novel agents, particularly immune checkpoint inhibitors, has further compounded this challenge. While most cADRs are mild and resolve upon drug discontinuation, approximately 2-6.7% progress to severe, potentially fatal conditions. The most common severe forms include acute generalized exanthematous pustulosis, Stevens-Johnson syndrome/toxic epidermal necrolysis, drug reaction with eosinophilia and systemic symp

Ocular involvement in toxic epidermal necrolysis syndrome at Chelsea and Westminster Hospital: a 10-year retrospective analysis and review of current literature.

BACKGROUND/OBJECTIVES: Toxic Epidermal Necrolysis Syndrome (TENS) is a rare, life-threatening, immune-mediated acute blistering reaction affecting the skin and mucosa. Ocular involvement with inflammatory de-epithelialisation is seen in >60% of patients, with severe cicatrising consequences. Management includes intensive steroid and lubricant eyedrops, Amniotic Membrane Transplantation (AMT) and co-ordinated multidisciplinary care of systemic illness. We assessed the incidence, severity and management of ocular involvement in TENS over a decade at Chelsea and Westminster Hospital. METHODS: Re

Successful Cultivation of Oral Mucosal Epithelial Sheet Following Antifungal Therapy in Cases With Limbal Stem Cell Deficiency.

PURPOSE: The aim of this study was to report four cases of limbal stem cell deficiency in which oral antifungal therapy enabled successful generation of cultured oral mucosal epithelial sheets for ocular surface reconstruction. METHODS: This case series included two patients with Stevens-Johnson syndrome and two with ocular cicatricial pemphigoid, all of whom were scheduled for cultivated oral mucosal epithelial transplantation (COMET). Initial oral mucosal biopsies resulted in fungal contamination during cell culture. Patients subsequently underwent targeted oral management, including dental

Immunological Interplay at the Ocular Surface of Stevens-Johnson Syndrome, Ocular Cicatricial Pemphigoid, and Ocular Graft Versus Host Disease.

Rare ocular surface inflammatory disorders (OSIDs), such as ocular cicatricial pemphigoid (OCP; 1.3-2/million), Stevens-Johnson Syndrome (SJS; 1-5/million), and ocular graft versus host disease (oGVHD; occurring in >50% of chronic GVHD cases) share overlapping clinical phenotypes. Their defining features include conjunctival cicatrization and chronic inflammation. An incomplete understanding of the underlying immunopathogenesis of these conditions has hindered the development of targeted therapies, leaving patients at risk of permanent vision loss and highlighting the need to define distinct o

Severe Cutaneous Adverse Reactions Associated With Newer-Generation Antiseizure Medications: A Real-World Pharmacovigilance Study Based on FAERS and JADER.

BACKGROUND: Severe cutaneous adverse reactions (SCARs) are rare but potentially fatal immune-mediated toxicities associated with antiseizure medications (ASMs). Phenotype-resolved post-marketing safety profiles for newer-generation ASMs have not been comprehensively characterized across pharmacovigilance systems. METHODS: A cross-database disproportionality analysis was conducted using FAERS and JADER (January 2004-September 2025). Twenty-two newer-generation ASMs coded as primary-suspect drugs were included; outcomes were the four SCAR phenotypes (Stevens-Johnson syndrome, SJS; toxic epiderm

Desquamating dermatological conditions: The burns surgeon's guide to diagnosis and management.

Desquamating or blistering skin conditions may be referred to burns and plastic surgeons owing to the morphological similarity to burn injuries, given an increasing shortage of dermatology inpatient beds within the context of NHS care. Desquamating skin conditions, similar to burn injuries, carry the potential to be severe, debilitating and potentially fatal; however, they present more complexity owing to their underlying aetiologies. These conditions may present with similar physiological challenges, dressing requirements and need for extensive multidisciplinary input as burn injuries, and th

Anesthetics and cancer biology: Translational evidence from perioperative modulation to drug repurposing (Review).

BACKGROUND: Reports suggest that children with mobility impairment represent a significant proportion of the population living with a disability. Footwear is considered to be the key extrinsic factor affecting children's gait and footwear modifications have been historically postulated to assist with locomotory difficulty. Although therapeutic footwear has been considered within the literature, there is a lack of consistency on terminology and paucity on the overall understanding. A scoping review was performed to chart the key concepts in children's footwear and to establish the range of stud

A Rare Case of Stevens-Johnson Syndrome in a Young Patient Presenting With Odynophagia Following Antibiotic Use.

BACKGROUND Stevens-Johnson syndrome (SJS) is a rare, immune-mediated mucocutaneous disorder usually triggered by medications or infections. It typically manifests with widespread epidermal detachment and mucosal ulcerations, but atypical cases can present with isolated mucosal involvement, leading to diagnostic uncertainty and delayed treatment. Recognizing such varying presentations is crucial to precent progression to more severe disease and improve outcomes. CASE REPORT We present the case of an 18-year-old male with no significant medical history who presented with severe odynophagia, oral

Mycoplasma-induced Rash and Mucositis: A Distinct Pediatric Entity from India.

Mycoplasma-induced rash and mucositis (MIRM) is a recently recognized mucocutaneous disorder, often misdiagnosed as Stevens-Johnson syndrome (SJS), toxic epidermal necrolysis (TEN), or erythema multiforme (EM). We report a 6-year-old boy presenting with fever, cough, and cold, followed by painful oral ulcerations, conjunctivitis, genital mucositis, and generalized vesiculobullous rash. He was on levetiracetam for a prior unprovoked seizure, raising suspicion of drug reaction. Initial differentials included SJS, TEN, EM, and Drug reaction with eosinophilia and systemic symptoms. Case Report: Bullous erythema multiforme induced by omalizumab.

Bullous erythema multiforme (BEM) is an acute immune-mediated mucocutaneous disease, with drug induction being one of its common causes. Omalizumab, a recombinant humanized IgG1 monoclonal antibody targeting free IgE, is widely used in chronic spontaneous urticaria and asthma due to its favorable safety profile, while severe cutaneous bullous adverse reactions induced by it are extremely rare. We report a 36-year-old female patient with a history of bronchial asthma, allergic rhinitis, allergic conjunctivitis, and chronic spontaneous urticaria who developed generalized wheals. After the first

An update on our understanding of host defense mechanisms at the ocular surface; innate immune dysregulation at the ocular surface: lessons from Stevens-Johnson syndrome and related inflammatory disorders.

INTRODUCTION: The ocular surface is a frontline mucosal immune barrier that protects the eye from pathogens while preserving immune tolerance essential for vision. Breakdown of this innate immune homeostasis can trigger chronic ocular surface inflammation, exemplified by Stevens-Johnson syndrome/toxic epidermal necrolysis with severe ocular complications (SJS/TEN with SOC) and atopic keratoconjunctivitis (AKC). AREAS COVERED: The review summarizes current knowledge of ocular surface innate immunity, focusing on pattern-recognition receptors (TLR3, RIG-I, MDA5) and downstream interferon-stimul

Comparative analysis of microbiota in the ocular surface of patients with atopic keratoconjunctivitis and Stevens-Johnson Syndrome with severe ocular complications.

INTRODUCTION: The interplay between host immunity and the microbiome is increasingly recognized as an important factor in ocular surface diseases. Although cutaneous and intestinal dysbiosis have been implicated in atopic dermatitis (AD), the role of the ocular microbiome in its severe ophthalmic manifestation, atopic keratoconjunctivitis (AKC), remains incompletely understood. METHODS: We performed 16S rRNA gene sequencing to characterize the ocular and fecal microbiota of patients with AD and severe AKC, AD without severe ocular involvement, Stevens-Johnson syndrome (SJS) with severe ocular
