Structural basis for Mg²⁺-glycine synergy in the tandem glycine riboswitch revealed by cryo-EM.

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ

Lamotrigine-Induced Toxic Epidermal Necrolysis (Lyell's Syndrome): A Case Report and Literature Review.

Lyell's syndrome, also known as toxic epidermal necrolysis (TEN), is a rare and severe adverse drug reaction characterized by widespread skin detachment, mucosal involvement, and high mortality. We present the case of a 24-year-old female who developed TEN after the recent initiation of lamotrigine for bipolar disorder. The patient exhibited rapidly progressing skin blistering and detachment affecting over 30% of the body surface area, along with mucosal lesions. Lamotrigine was immediately discontinued, and the patient received intensive supportive treatment in a specialized care unit, result

Drug-Induced Oral Erythema Multiforme: A Report of a Rare Case.

Erythema multiforme (EM) is an acute-onset, self-limiting inflammatory condition affecting the skin and mucous membranes. It exhibits a range of skin lesions, which is why it is referred to as "multiforme." Oral lesions, usually inflammatory and frequently marked by rapidly rupturing vesicles and bullae, often constitute a significant clinical feature. The exact etiology is obscure; however, it may be caused by multiple triggering factors. The most well-established connection is with a previous herpes simplex virus infection, while a smaller percentage of cases (<10%) are linked to drug use. B

Stevens-Johnson syndrome and toxic epidermal necrolysis from pathologist's point of view.

Stevens-Johnson syndrome and toxic epidermal necrolysis (Lyell syndrome) are rare diseases characterized by rapid blistering followed by extensive skin and mucosal exfoliation and constitutional symptoms. In most cases, drugs are the main triggers, but the etiopathogenesis of the diseases is not fully understood. Lyell syndrome is associated with a high mortality rate, reported to be around 35%. Therefore, early diagnosis requiring close interdisciplinary cooperation is essential. The diagnosis based on the clinical picture and a detailed pharmacological history should be confirmed by histopat

Nine years of a single referral center management of Stevens-Johnson syndrome and toxic epidermal necrolysis (Lyell's syndrome).

CONTEXT: Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) corresponds to a rare and acute life-threatening mucocutaneous reactions characterized by extensive necrosis and epidermal detachment. There are no efficacious pharmaceutical interventions proven through large clinical trials. OBJECTIVE: We sought to study clinical cases admitted in our institution in order to determine which drugs and medical comorbidities or treatments impacted the mortality. MATERIAL AND METHODS: In a retrospective study over 9 years we evaluated all patients presenting biopsy-proven SJS or TEN f

[Toxic epidermal necrolysis (Lyell syndrome), a lifethreatening emergency of introgenic origin].

Cutaneous drug eruptions are frequently encoun¬tered. Their putative diagnosis is based on a set of imputability arguments. The histopathological aspect is often suggestive of the dermatosis nature, and varies according to the type of drug reaction. Some drug eruptions follow a benign course, but others including the toxic epidermal necrolysis are severe and life-threatening. It corresponds to a dermatological emer¬gency to be managed in a specialized burn unit.

Death caused by toxic epidermal necrolysis (Lyell syndrome).

Toxic epidermal necrolysis (TEN) is characterized by fever, scalded appearance of the skin, and epidermolysis associated to blister formation and exfoliation, and it is caused by hypersensitivity reaction to a drug. The authors report two cases of death as a result of TEN; both referred to old aged women treated with a polytherapy including allopurinol. Both patients displayed erythematous skin lesions similar to scald burns and epidermolysis at the face, chest, and abdomen and died shortly after hospitalization. Autopsy findings and histological examinations revealed epidermal necrolysis and

Toxic epidermal necrolysis (Lyell's disease).

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are severe adverse drug reactions, characterized by a low incidence but high mortality, initially described as separate entities, but today considered variants of the same pathologic process and differing only for severity. The majority of cases appear to be related to idiosyncratic drug reactions. The drugs most commonly involved are: antibiotics such as sulfonamides, beta-lactam, tetracyclines and quinolones; anticonvulsants such as phenytoin, phenobarbital and carbamazapine; antiretroviral drugs; nonsteroidal anti-inflammat

Toxic epidermal necrolysis related to pemetrexed and carboplatin with vitamin B12 and folic acid supplementation for advanced non-small cell lung cancer.

BACKGROUND: Pemetrexed is a multitargeted antifolate initially approved as a single agent for the second-line treatment of locally advanced or metastatic non-small cell lung cancer and more recently in the first-line setting combined with cisplatin. The combination of pemetrexed with carboplatin has been tested in several phase II clinical trials showing interesting antitumour activity with mild toxicity. Supplementation with folic acid and vitamin B12 during treatment with pemetrexed is recommended to reduce potential haematological and gastrointestinal adverse events. CASE REPORT: A patient

[Stevens-Johnson syndrome in the literature and authors' own studies].

Stevens-Johnson Syndrome and Toxic Epidermal Necrolysis (Lyell Syndrome) are severe, multisystem disease caused mainly by reaction to drugs. The clinical features include: changes on the skin and mucosa as well as lesions in the internal organs. There are no standards of treatment in this diesease group. There are ongoing trials in various groups of drugs. Apart from providing the appriopriate conditions and symptomatic treatment, immunoglobulins, cytostatics and plasmapheresis are used. The authors present the clinical course of a patient with Stevens-Johnsons Syndrome, probably caused by kla

Severe bullous skin diseases: analysis of seven children managed in a burns unit.

Patients suffering from considerable cutaneous loss must be treated under strict aseptic conditions and with positive pressure ventilation, which is available in burns units. The differentiation of severe bullous skin diseases remains a challenge for the clinician. We report seven cases of severe bullous skin diseases in a paediatric age group treated in a burns unit with the cooperation of a paediatric intensive care unit (King Saud Hospital, Unizah, Kingdom of Saudia Arabia) between 2001 and 2005. Toxic epidermal necrolysis (Lyell's syndrome) was encountered in five cases, staphylococcal sca

Novel treatments for drug-induced toxic epidermal necrolysis (Lyell's syndrome).

Drug-induced toxic epidermal necrolysis (TEN) is a life-threatening disease characterized by extensive destruction of the epidermis. It apparently results from the formation of specific toxic drug metabolites by the keratinocytes. The mortality rate which averages 25-30% is mainly due to secondary septicemia, and to ionic and metabolic disturbances following loss of epidermal integrity. Apoptosis is the likely mechanism leading to massive keratinocyte death in TEN. Dysregulations in the tumor necrosis factor-alpha (TNF-alpha) pathway, CD95 system (Fas ligand, CD95L; Fas receptor, CD95R) and ca

Cystic lesion of the parotid following drug-induced toxic epidermal necrolysis (Lyell's syndrome).

A 32-year-old woman developed a unilateral cyst of the duct of parotid gland 4 months after severe oral involvement of drug-induced toxic epidermal necrolysis (TEN). The pathomechanism leading to the TEN epidermal destruction had presumably involved the salivary epithelium as well, leading to the development of the cystic lesion. The patient had low serum lipase levels, but high serum amylase levels at the time of TEN. These serological markers could represent a clue for the risk of developing cystic lesions of the large salivary glands following TEN.

[Toxic Epidermal Necrolysis (Lyell syndrome): a pathology for burn units].

Toxic Epidermal Necrolysis (Lyell's syndrome) is a rare but very serious dermatological lesion, characterized by the sudden onset of high fever, signs of systemic toxicity and intense mucocutaneous exfoliation. Its pathophysiology is not yet well determined, although it is almost consensual the presence of an immunological basis. It appears usually as an answer to the taking of a given drug, and, in spite of being self-limited in the absence of complications, if not well managed it is associated with great morbidity and a high mortality, due, in most cases, to the developing of sepsis. Factor-XIIIa-positive dendrocytes in drug-induced toxic epidermal necrolysis (Lyell's syndrome): paradoxical activation in skin and rarefaction in lymph nodes.

BACKGROUND: Drug-induced toxic epidermal necrolysis (TEN) is a rare life-threatening disease characterized by the extensive destruction of the epidermis contrasting with the discreteness of lymphoid cell infiltration. The precise pathomechanism of the disease remains unclear. METHODS: Skin specimens and peripheral and thoracic lymph nodes (LNs) were collected from 2 TEN patients. They were examined by conventional histology and immunohistochemistry using antibodies directed to factor XIIIa (dermal dendrocytes), CD1a (Langerhans cells), CD15 (granulocytes), CD20 (B lymphocytes), CD45RO (activa
