Outcome of sutureless amniotic membrane dressing ring for acute Stevens-Johnson syndrome with severe ocular involvement: A case report of 5 patients.

RATIONALE: Stevens-Johnson Syndrome (SJS) and toxic epidermal necrolysis can lead to severe ocular damage and long-term vision complications. Our self-made sutureless amniotic membrane dressing (AMD) ring leverages the anti-inflammatory and regenerative properties of the amniotic membrane (AM), promoting healing, reducing scarring, and enhancing patient comfort. PATIENT CONCERNS: Five consecutive patients (3 females, 2 males; aged from 18 to 53 years) with acute SJS/TEN were at immediate risk of significant ocular surface damage, including corneal and conjunctival epithelial defects and eyeli

Clinical Features and Treatment Response in Chronic Recurrent Erythema Multiforme: Difference Based on the Etiology Related to Herpes Simplex Virus.

BACKGROUND: Erythema multiforme (EM) is typically a self-limited, acute hypersensitivity reaction. However, a subset of patients experiences chronic, recurrent episodes, for which clinical features and treatment strategies differ depending on the underlying etiology, especially in herpes simplex virus (HSV)-associated cases. OBJECTIVE: To investigate the clinical and phenotypic features of chronic recurrent EM and assess treatment responses, with a focus on differences based on HSV association. METHODS: This retrospective study included pathology-confirmed cases of suspected EM from 2010 to

A Case of Concurrent Behçet Disease and Erythema Multiforme in a Young Adult.

Behçet disease (BD) and erythema multiforme (EM) are rare, immune-mediated, clinically diagnosed conditions that can present with overlapping mucocutaneous findings and symptoms. We report a diagnostically challenging presentation of concurrent BD and EM characterized by oral ulcers, genital lesions, and targetoid bullous acral lesions in a 23-year-old African American man. This case highlights the risk of premature diagnostic closure and emphasizes that histopathology is supportive rather than determinative in diagnosing either of these conditions. Management required multidisciplinary input

Cutaneous and non-cutaneous diseases due to Mycoplasma pneumoniae in children.

Mycoplasma pneumoniae (MP) is a common pathogen responsible for diverse infections in children and adolescents, primarily affecting the respiratory tract. Besides causing atypical pneumonia, MP can also lead to extrapulmonary manifestations, including mucocutaneous, hematological, neurological, cardiac, and gastrointestinal symptoms. These extrapulmonary manifestations are often overlooked, complicating diagnosis and management. Here, we delineate the spectrum of cutaneous and non-cutaneous diseases caused by MP in children, focusing on the pathophysiology, clinical features, and treatment of

Clinicopathologic and Immunohistochemical Features of Enfortumab Vedotin-Induced Cutaneous Toxicity.

BACKGROUND: Enfortumab vedotin (EV) frequently causes cutaneous eruptions that can mimic erythema multiforme and Stevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN), complicating management decisions. OBJECTIVE: The aim of this study was to define reproducible clinicopathologic patterns and evaluate immunohistochemical markers that distinguish EV-induced cutaneous toxicity (EVICT) eruptions from histologic mimics. METHODS: This retrospective case series included nineteen patients receiving EV (twenty-four biopsies), compared with ten controls (erythema multiforme or SJS/TEN). Blind

A case of Stevens-Johnson syndrome triggered by Mycoplasma infection.

Stevens-Johnson syndrome (SJS) is a rare, potentially life-threatening mucocutaneous disorder, which is often triggered by infections such as Mycoplasma pneumoniae. This report describes a 19-year-old woman who initially presented with oral erosions and painful blisters, along with fever and cough. Oral lesions preceded skin and ocular involvement, and tests confirmed Mycoplasma-associated SJS. She responded to steroid pulse therapy but relapsed at 3 months and was then successfully treated. Oral manifestations may be early SJS indicators; thus, prompt recognition and interdisciplinary care by

Diagnosis and treatment of cutaneous adverse effects of targeted therapy, antibody-drug conjugates, and immunotherapy in cancer patients: a national consensus statement by the Spanish Society of Medical Oncology and the Spanish Academy of Dermatology and Venereology.

Cancer remains a leading cause of mortality in developed countries, with advancements in targeted therapies, antibody-drug conjugates (ADCs), and immunotherapy significantly improving patient survival. However, these treatments often result in cutaneous adverse effects (AEs), impacting the skin, mucosa, hair, and nails. This expert consensus statement provides a comprehensive overview of the mechanisms behind these skin toxicities, their clinical manifestations, and the importance of rapid diagnosis and treatment. The spectrum of skin toxicities ranges from mild reactions such as xerosis and p

The Outcomes of SJS/TEN: A Nationwide Analysis.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) syndrome are potentially fatal skin conditions frequently cared for in burn units. In a national database study, we studied the demographics and outcomes of this patient population. This retrospective cohort study included patient admissions for SJS/TEN. Patient demographics and outcomes were compared and adjusted for age, sex, inhalation injury, and percent total body surface area (TBSA). Logistical regression was used for binary outcomes, and linear regression was used for continuous outcomes. All outcomes were described in

Diagnosis and Management of Stevens-Johnson Syndrome and Toxic Epidermal Necrolysis in Pediatric Patients: A Systematic Review of Clinical Guidelines and Consensus Statements.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are rare, life-threatening mucocutaneous reactions. Pediatric mortality is lower than in adults, but children face higher risks of recurrence and long-term sequelae. Despite disease severity, pediatric-specific guidelines remain scarce and often extrapolated from adult populations. We systematically reviewed and appraised national and international clinical practice guidelines and consensus statements on pediatric SJS/TEN, focusing on supportive care, pharmacologic interventions, mucosal management, and long-term follow-up. A case of delayed-onset Stevens-Johnson syndrome following co-amoxiclav administration in a 98-year-old female.

BackgroundStevens-Johnson Syndrome is a type IV hypersensitivity mucocutaneous reaction characterised by a widespread desquamating rash, and is considered to be an emergency due to its potentially fatal complications. Drugs account for 50-80% of Stevens-Jonson Syndrome/Toxic Epidermal Necrolysis (SJS/TEN) cases, with anticonvulsants, antibiotics, analgesics and allopurinol being the most common causative agents. Co-amoxiclav contains amoxicillin (a penicillin) and clavulanic acid (a beta-lactamase inhibitor) and is widely used in healthcare settings for treatment of common infections due to it

Molecular Insights Into TNF-α and IFN-γ-Induced Epithelial Cell Death in Toxic Epidermal Necrolysis.

Rapid and extensive epithelial cell death in skin and mucous membranes is the hallmark of Toxic epidermal necrolysis (TEN). High levels of TNF-α and IFN-γ have been detected in the blister fluids of skin lesions in TEN patients. Available literature shows that TNF-α synergy with IFN-γ (TNF-α/IFN-γ) elicited cell death in diverse cells. Thus, the cytotoxic effect and underlying mechanism of TNF-α and IFN-γ on HaCaT cells were investigated. Confluent HaCaT cells were subjected to serial doses of either TNF-α alone, IFN-γ alone, or combination TNF-α with IFN-γ (TNF-α/IFN-γ) for indicated time int

Stevens-Johnson syndrome/toxic epidermal necrolysis induced by sintilimab in a patient with advanced non-small cell lung cancer: A case report.

RATIONALE: Sintilimab, an immune checkpoint inhibitor, enhances T-cell responses, leading to robust antitumor activity, and is approved for the treatment of lung cancer. While immune checkpoint inhibitors offer substantial clinical benefits, they are often associated with immune-related adverse events, particularly cutaneous toxicities. These skin reactions typically manifest as mild maculopapular rashes; however, more severe manifestations, such as Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN), can occur. Both SJS and TEN are life-threatening conditions with high mortali

Extracorporeal Membrane Oxygenation Therapy in Toxic Epidermal Necrolysis-Related Respiratory Failure: A Case Report.

Toxic epidermal necrolysis (TEN) is a rare and life-threatening cutaneous adverse reaction characterized by > 30% total body surface area desquamation and mucosal involvement. Although ocular, oral, and urogenital mucosal involvements are well described, pulmonary mucosal injury is uncommon and associated with high mortality. We report a case of a 33-year-old woman with rapidly progressive TEN complicated by respiratory failure. Despite early multidisciplinary management including local wound care, immunomodulatory therapy, systemic corticosteroids, and lung-protective mechanical ventilation,

The effect of different therapies and complete blood cell count-derived inflammatory biomarkers on the prognosis of Stevens-Johnson syndrome and toxic epidermal necrolysis.

BACKGROUND: Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are severe mucocutaneous adverse drug reactions still with controversy in treatment. Complete blood cell count-derived inflammatory biomarkers reflect systemic inflammation, and their value in SJS/TEN has not been fully studied. OBJECTIVES: To compare the therapeutic efficacy of corticosteroid combined with adalimumab (CA), corticosteroid combined with intravenous immunoglobulin (CI) and corticosteroid monotherapy (CM), and explore prognosticators on severity and prognosis of SJS/TEN. METHODS: All inpatients with

Intravenous immunoglobulin as a therapy for severe or steroid-recalcitrant erythema multiforme major: a case series.

Erythema multiforme (EM) is typically self-limiting. However, severe EM major may cause significant morbidity and lacks evidence-based therapeutic guidelines. Intravenous immunoglobulin (IVIg) has primarily been used in recurrent EM. This study evaluates IVIg use in severe first-episode or steroid-recalcitrant EM major. A retrospective analysis reviewed six patients with EM major treated with IVIg at a tertiary Australian hospital. IVIg was administered in eight instances. Disease triggers included Mycoplasma pneumoniae in three admissions, unknown aetiology in three admissions, rhinovirus/inf
