[KEY POINTS FOR DRUG ERUPTIONS IN DAILY PRACTICE].

Drug eruptions are common mucocutaneous adverse reactions encountered across all medical specialties, occurring in approximately 0.1-1% of patients exposed to medications. However, accurate diagnosis and identification of the culprit drug remain challenging because of heterogeneous clinical presentations and a wide range of differential diagnoses, particularly viral exanthems. Comprehensive evaluation of medication history, infection status, latency period, and clinical phenotype is essential for appropriate narrowing of suspected agents. Among drug eruptions, Stevens-Johnson syndrome and toxi

Crosstalk mediators implicated in the Stevens-Johnson Syndrome through gene regulatory network analysis.

Stevens-Johnson syndrome (SJS) is a rare and severe mucocutaneous disorder often triggered by medications or infections. Our previous research identified that four key genes, Ikzf1, Ptger3, Mavs, and Tlr3 are involved in SJS susceptibility and the conjunctival epithelial innate immune response, demonstrating their role in regulating interferon-stimulated genes. However, the interplay among these regulatory factors remains unclear. This study aimed to elucidate the crosstalk mechanisms between the pathways regulated by these four genes in conjunctival epithelial cells. We constructed a comprehe

Stevens-Johnson syndrome and toxic epidermal necrolysis: diagnosis and management in the emergency department.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are rare but potentially fatal mucocutaneous emergencies that exist on a spectrum and are most commonly caused by a medication reaction. Although data guiding diagnosis and management remain limited, early recognition, prompt identification and discontinuation of the causative agent, symptomatic management, and multidisciplinary involvement are essential to improving patient outcomes. Validated prognostic tools can further guide management decisions. This review provides an evidence-based approach to the recognition and manage

Severe cutaneous adverse reactions associated with systemic antibiotics in acne vulgaris: a systematic review.

BACKGROUND: Acne vulgaris (AV) is an inflammatory disorder of the pilosebaceous unit commonly managed with systemic antibiotics. Since antibiotics are known triggers of severe cutaneous adverse drug reaction (SCARs), patients with AV receiving prolonged antibiotics are at potential risk. This study aims to describe reported cases of SCARs occurring among AV patients treated with systemic antibiotics. METHODS: A literature search was conducted in November 2025 using PubMed, Web of Science, ClinicalKey, Ovid and Cochrane library. Eligible studies included reports of AV patients who developed SC

[Steroid-Refractory Cutaneous Graft-versus-Host Disease: a Case Report].

Cutaneous graft-versus-host disease (cGVHD) is a potentially severe complication following allogeneic hematopoietic stem cell transplantation. We present the case of a 40-year-old woman with acute myeloid leukemia who underwent an allogeneic transplant with an HLA-matched unrelated donor. During the post-transplant phase, she developed steroid-refractory cutaneous GVHD, progressing to widespread lesions compatible with toxic epidermal necrolysis (NET-like). Management included second- and third-line therapies, such as JAK inhibitors (ruxolitinib), immunoglobulin, biological agents (infliximab,

[Erythema multiforme : target deciphering].

Erythema multiforme is an immune-mediated skin reaction characterized by target-shaped lesions. These lesions are described here and compared with their clinical mimickers. The condition is mainly secondary to an infection, typically Herpes simplex virus infection, and more rarely to medication, autoimmune disease, hematological neoplasia or radiation. Diagnosis is clinical. Due to its diverse manifestations, a skin biopsy and immunohistochemical analyses can confirm the condition and rule out differential diagnoses. Treatment is symptomatic and combined with management of the underlying cause

Toxic epidermal necrolysis: Management in the ICU.

Epidermal necrolysis (EN) includes Stevens-Johnson syndrome (SJS, detached/detachable skin surface <10% body surface), toxic epidermal necrolysis (EN, detached/detachable skin surface ≥30%), and SJS-EN overlap (detached/detachable skin surface 10-29%). These are rare but severe cutaneous adverse reactions, usually triggered by drugs. Drugs account for 85-90% of cases, with onset typically 4-28 days after exposure. The acute phase is life-threatening, with respiratory and septic complications being the main determinants of prognosis. Mortality averages 15-20%. Supportive care is the cornerstone

Immunological and molecular signatures of carbamazepine-induced maculopapular exanthema.

Cutaneous adverse drug reactions to carbamazepine range from mild maculopapular exanthema to life-threatening syndromes such as Stevens-Johnson syndrome/toxic epidermal necrolysis. While the immunological basis for HLA-B*15:02-restricted carbamazepine-induced SJS/TEN has received greater attention, mechanisms underlying mild maculopapular exanthema remain poorly understood. In this study, we examined circulating T-cell immune recognition and transcriptomic signatures in the blood of patients with carbamazepine-induced maculopapular exanthema during active and resolved disease and compared thes

Successful management of pembrolizumab-associated Stevens-Johnson syndrome/toxic epidermal necrolysis in a patient with uterine serous carcinoma: a case report.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are rare, life-threatening cutaneous adverse reactions associated with immune checkpoint inhibitors (ICIs). Evidence guiding optimal management remains limited. We report the case of a 75-year-old woman with stage IV endometrial carcinoma who developed extensive epidermal necrosis three weeks after the second cycle of maintenance pembrolizumab. Skin biopsy confirmed full-thickness epidermal necrosis, and the SCORTEN score was 3, predicting a high risk of mortality. There was no significant ocular or mucosal involvement. The pa

Expert consensus on oral management in autoimmune bullous diseases, erythema multiforme and SJS/TEN.

BACKGROUND: Autoimmune bullous diseases (AIBD), Stevens-Johnson syndrome and toxic epidermal necrolysis (SJS-TEN) and erythema multiforme (EM) often present with clinically similar features in the oral mucosa: classically widespread mucosal erosions or ulceration. Despite this, there are no formalized guidelines for the specific management of the oral manifestations of these conditions. OBJECTIVES: We sought to establish an consensus on the management of oral involvement of AIBD, SJS-TEN and EM using the Delphi method. METHODS: Participants were sent a survey comprising 62 statements organiz

A Recurrent Case of Reactive Infectious Mucocutaneous Eruption (Rime) in a Young Adult Male: A Case Report.

Reactive infectious mucocutaneous eruption is a new classification as of 2021. It describes a parainfectious eruption that may be difficult to distinguish from Stevens-Johnson syndrome, toxic epidermal necrolysis, and erythema multiforme in its early stages. We present a likely fifth recurrent case in a young adult male, who was treated empirically with his condition improving three days post-admission.

Pustules in Stevens-Johnson Syndrome.

BACKGROUND Stevens-Johnson syndrome (SJS) is characterized by widespread, epidermal necrosis and mucosal involvement mediated by a delayed-type hypersensitivity reaction. Although rapidly progressive epidermal detachment is known to result in blister formation, pustular lesions are rare in SJS. CASE REPORT A 25-year-old male patient with no significant medical history or regular medication presented to the emergency department with a fever and rash. The fever had developed 7 days before the current presentation and was followed 4 days later by lip swelling, conjunctival hyperemia, and sore thr

Diagnostic challenge: Epidermolysis bullosa acquisita of the palms and soles.

Epidermolysis bullosa acquisita (EBA) is a rare autoimmune blistering disorder that can be difficult to diagnose, especially when its presentation mimics other blistering conditions. We present the case of a 56-year-old male who developed widespread blistering on flexural surfaces, palms, soles, and target lesions, with clinical features resembling bullous pemphigoid (BP), erythema multiforme (EM), and infectious aetiologies. A skin biopsy showed subepidermal bullae with linear IgG and C3 deposition at the basement membrane, with additional focal IgM and IgA positivity, and ELISA testing confi

RUCAM-ascertained immunology and autoimmunity specifics of six idiosyncratic drug-induced liver injury types with refined classification: their individual complex molecular interplay.

Idiosyncratic drug-induced liver injury (iDILI) is not a uniform disease but rather includes a variety of types based on immune, autoimmune, and clinical considerations. This review attempts to close information gaps regarding the role of immunity and autoimmunity involved in the different iDILI disease types. The analysis of the current literature focusing on iDILI reveals compelling evidence of a pivotal role of immunity or autoimmunity in various types of iDILI. Among the autoimmune-triggered ones are the drug-induced autoimmune hepatitis (DIAIH) and the idiosyncratic drug-induced anti-CYP

Rare Presentation of TEN-Like Subacute Cutaneous Lupus Erythematosus Occurring in the Setting of Donepezil Exposure and COVID-19 Infection: A Diagnostic Challenge.

Toxic epidermal necrolysis (TEN) is a life-threatening mucocutaneous reaction characterized by rapidly progressive epidermal necrosis within hours to days, extensive skin detachment, and high mortality. Rarely, patients present with TEN-like eruptions that evolve slowly, over days to weeks, and have skin changes mostly confined to photodistributed areas, with possible mucosal involvement. The latter scenario is compatible with TEN-like lupus erythematosus. We report a 65-year-old man without prior lupus history who developed a widespread, photodistributed erythematous patchy eruption within 24
