Patient-attributed lunar periodicity in probable ulcerated erythema nodosum leprosum: A case report and proposed psychoneuroendocrine mechanism.

BACKGROUND: Erythema nodosum leprosum (ENL) is a severe, immune mediated type 2 lepra reaction that occurs predominantly in the lepromatous spectrum of leprosy. It is characterized by the eruption of tender inflammatory nodules. The possible contribution of patient belief and the nocebo effect, on precipitating ENL, has not been examined. We present a patient with lepromatous leprosy and probable ulcerated ENL whose flares the patient reported to coincide with the full moon, raising questions about how cultural explanatory models and cognitive pattern recognition intersect with chronic inflamm

Differential Expression and Correlation of High-mobility Group Box 1 and Vascular Endothelial Growth Factor Messenger Ribonucleic Acid across Leprosy Clinical Groups and Healthy Controls: A Cross-sectional Study.

BACKGROUND: Erythema nodosum leprosum (ENL) is an acute inflammatory complication of multibacillary leprosy. High-mobility group box 1 (HMGB1) mediates inflammatory signaling, whereas vascular endothelial growth factor (VEGF) regulates angiogenesis and vascular permeability. We compared whole-blood HMGB1 and VEGF messenger ribonucleic acid (mRNA) expression across leprosy groups and assessed their correlations. METHODS: This cross-sectional study included 146 participants: 35 with ENL, 36 with multibacillary leprosy without ENL, 38 with paucibacillary leprosy, and 37 healthy controls. HMGB1 a

Methotrexate and Prednisolone compared to placebo and prednisolone in the treatment of severe Erythema Nodosum Leprosum: An international multicentre, double-blind randomised controlled clinical trial-MaPs in ENL.

BACKGROUND: Erythema nodosum leprosum (ENL) is a severe inflammatory complication of leprosy that often requires prolonged corticosteroid therapy which is associated with adverse effects. Methotrexate is an affordable immunomodulatory agent with limited evidence for its use in ENL treatment. We evaluated whether weekly oral methotrexate in addition to prednisolone reduces the need for additional prednisolone in adults with severe ENL. METHODS AND FINDINGS: We performed an international, multicentre, double-blind, randomised, placebo-controlled trial conducted at five leprosy referral centres

Skin Deep: A Case Report of Erythema Nodosum Induced by Dupilumab Therapy.

Dupilumab is a fully human monoclonal antibody that inhibits interleukin (IL)-4 and IL-13 signaling, thereby attenuating type 2 inflammatory pathways. It is currently approved for the treatment of severe eosinophilic asthma, atopic dermatitis, chronic rhinosinusitis with nasal polyps, eosinophilic esophagitis, and other allergic conditions. Erythema nodosum (EN) is the most common form of panniculitis, characterized by tender, erythematous subcutaneous nodules typically distributed over the extensor surfaces of the lower extremities. Drug-induced EN represents a minority of all EN cases and ha

Multiple cerebral venous thrombosis as the first presentation of Behçet syndrome: case report and literature review.

INTRODUCTION: Cerebral venous thrombosis as the first manifestation of Behçet's Syndrome (BS) is very rare. We describe a case of BS in which all four major cerebral venous sinuses, including the bilateral sigmoid sinuses, transverse sinuses, and superior sagittal sinuses, exhibited severe cerebral venous thrombosis. CASE REPORT: An 18-year-old man was brought to our hospital with a three-month-old, wide, throbbing headache in both hemispheres and double, blurred vision while looking to the right for the last week. Magnetic Resonance Imaging (MRI) showed extensive thrombosis involving the bil

Contribution of Ocular Involvement to the Diagnosis of Systemic Sarcoidosis and Prognostic Significance.

PURPOSE: Only a few studies have analyzed the diagnostic utility and prognostic significance of ocular sarcoidosis (OS) in patients with systemic sarcoidosis. Our aim was to analyze the prevalence of OS in sarcoidosis, its possible association with other forms of extrathoracic sarcoidosis, and its prognostic significance. METHODS: Retrospective study including patients diagnosed with sarcoidosis at Bellvitge University Hospital, Barcelona, Spain, between 1980 and 2017. Patients were prospectively followed up at the hospital's sarcoidosis clinic. Clinical data were obtained by reviewing patien

The Development of Sarcoidosis in an Ulcerative Colitis Patient Treated with Vedolizumab: A Case Report and Review of the Literature.

BACKGROUND: Ulcerative colitis (UC) and sarcoidosis are chronic inflammatory diseases that share immunological pathways but rarely coexist. The increasing use of biologic agents in inflammatory bowel disease (IBD) has raised concerns regarding paradoxical inflammatory manifestations, including sarcoidosis-like reactions. CASE PRESENTATION: We report the case of a 63-year-old man with long-standing UC treated with vedolizumab who developed systemic sarcoidosis characterized by bilateral hilar lymphadenopathy, mediastinal and abdominal lymph node enlargement, pulmonary involvement, and erythema

Cutaneous and non-cutaneous diseases due to Mycoplasma pneumoniae in children.

Mycoplasma pneumoniae (MP) is a common pathogen responsible for diverse infections in children and adolescents, primarily affecting the respiratory tract. Besides causing atypical pneumonia, MP can also lead to extrapulmonary manifestations, including mucocutaneous, hematological, neurological, cardiac, and gastrointestinal symptoms. These extrapulmonary manifestations are often overlooked, complicating diagnosis and management. Here, we delineate the spectrum of cutaneous and non-cutaneous diseases caused by MP in children, focusing on the pathophysiology, clinical features, and treatment of

Erythema nodosum as a marker for objective disease activity in inflammatory bowel disease.

BACKGROUND: Erythema nodosum (EN) is an inflammatory condition marked by tender, red nodules, typically on the extensor limbs. Though often linked to IBD activity, its association with objective markers like inflammatory labs or endoscopic findings remains unclear. This study examined the relationship between EN and objective inflammation in IBD patients. METHODS: At a tertiary clinic with over 6500 patients, individuals with active EN were identified. Patients with active EN were identified, and symptomatic disease activity was assessed. Objective assessments included endoscopy, cross-sectio

Tubercular chorioretinitis mimicking sarcoidosis in a patient with celiac disease and erythema nodosum: A case report.

BACKGROUND: Tuberculosis remains an important cause of uveitis in tuberculosis-endemic regions and can closely resemble other granulomatous disorders, such as sarcoidosis, especially in the presence of erythema nodosum. The diagnosis is even more challenging in the presence of an underlying autoimmune condition such as celiac disease. CASE SUMMARY: We report a case of a middle-aged female, a known case of celiac disease and recurrent severe anaemia presenting with progressive, recurring episodes of bilateral diminished vision, ocular pain, photophobia, and floaters over a period of 1.5 years.

Multibacillary Leprosy With Accelerated Progression: A Case Report of an Atypical Clinical Presentation and Therapeutic Approach.

Leprosy, or Hansen's disease, remains a public health concern, especially in endemic regions and among migrant populations. This case involves a patient from Venezuela living in Peru who initially presented with fever, weight loss, and respiratory symptoms without radiological findings. Physical examination revealed multiple erythematous nodules, hypopigmented macules, and thickened peripheral nerves. A skin smear confirmed multibacillary leprosy. The patient began standard multidrug therapy and showed gradual improvement. However, by the third month, he developed erythema nodosum leprosum (ty

Evaluation of Serum Levels of IL-17 and IL-22 in Leprosy: A Case-Control Study.

BACKGROUND: Leprosy exhibits a wide clinical spectrum that is largely determined by host immune responses. Recent evidence suggests an important role of the Th17/Th22 axis in disease activity and lepra reactions. PRIMARY OBJECTIVE: This study aimed to compare serum interleukin (IL)-17 and IL-22 levels between patients with leprosy and age- and sex-matched healthy controls. SECONDARY OBJECTIVES: This study also aimed (i) to evaluate the variation of serum IL-17 and IL-22 levels across the Ridley-Jopling spectrum of leprosy; (ii) to assess their association with lepra reactional states; and (i

Leprosy in Pregnancy: A Systematic Review of Maternal Characteristics, Complications, and Neonatal Outcomes.

BACKGROUND: Nearly 40% of leprosy cases occur in females. Although relatively uncommon in pregnancy, immunological changes during gestation and the puerperium may exacerbate leprosy and influence maternal and neonatal outcomes. To date, no systematic review has synthesized the evidence on leprosy in pregnancy. PURPOSE: To describe maternal clinical and obstetric characteristics, pregnancy complications, neonatal outcomes, and laboratory findings in pregnant women with leprosy. METHODS: We systematically searched electronic databases from inception to August 2025 for studies reporting leprosy

The Multifaceted Legacy of Thalidomide: Chemistry and Biology Driving Modern Drug Design.

Thalidomide represents one of the most instructive case studies in modern medicinal chemistry, embodying both a historic pharmaceutical tragedy and a remarkable example of drug repurposing and molecular reinvention. Initially introduced as a sedative and antiemetic, its catastrophic teratogenic effects reshaped global drug regulatory frameworks. Decades later, renewed investigation uncovered potent immunomodulatory, anti-inflammatory and antiangiogenic activities, leading to its controlled clinical use in erythema nodosum leprosum, multiple myeloma and related disorders. Central to this renais
