Genotypic investigation of Mycobacterium leprae in patients with relapsing leprosy in Mato Grosso, Brazil.

Leprosy is a chronic disease that leads to significant neurological damage and disabilities. Drug resistance is a major challenge in controlling the disease. Mutations in the folP1, rpoB, and gyrA genes can confer resistance to dapsone, rifampicin, and quinolones, respectively. The aim of this study was to identify mutations in these drug-resistance genes in Mycobacterium leprae among patients in the state of Mato Grosso, Brazil, and to investigate potential related clinical characteristics. A total of 60 patients from various cities in Mato Grosso were clinically evaluated. Following skin bio

Two missense mutations in Dystonin lead to epidermolysis bullosa simplex complicated with lepromatous leprosy: a case report.

BACKGROUND: Epidermolysis bullosa (EB) is a severe, single-gene hereditary skin condition characterized by blisters, bullae, and erosion following minor trauma. Epidermolysis bullosa simplex (EBS), one of the four major types of EB, is typically caused by mutations in the KRT5 or KRT14 genes. However, mutations in the Dystonin (DST) gene, which is also involved in maintaining skin integrity, can lead to EBS. This study aims to report a unique case of EBS caused by two novel heterozygous mutations in the DST gene, complicated by lepromatous leprosy (LL), a chronic infectious disease caused by M

Increased expression of histocompatibility leukocyte antigen-G in the tuberculoid form of leprosy.

Background The immunological context of leprosy is influenced by mechanisms closely associated with the human leukocyte antigen (HLA). HLA-G, a non-classical class I (Ib) molecule, plays a key role in regulating immune responses and has been linked to the progression and prognosis of various diseases. Objectives This study aimed to investigate HLA-G expression both in situ and in its soluble form in patients with different forms of leprosy, such as tuberculoid (TT), borderline tuberculoid (BT), borderline lepromatous (BL), and lepromatous leprosy (LL) and leprosy reactions, such as reversal re

Revealing Erythema Nodosum Leprosum in a Leprosy Patient: A Case of Treatment Noncompliance.

Lepra reactions (LR) are acute inflammatory conditions with immune mediators that are highly morbid. Patients with the lepromatous end of the leprosy spectrum (BL-LL) are the only ones who develop Type-2 LR. 90% of the time, it happens during or right after treatment, usually within 2 years. The emergence of Erythema Nodosum Leprosum (ENL) suggests a paradoxical immune reaction and hypersensitivity to leprosy bacteria that results in painful erythematous nodules and systemic symptoms. Here, we present a case of Type 2 LR in a 41-year-old Indian man diagnosed with lepromatous leprosy 2 years ag

From Nerves to Nodules: A Case-Based Review of the Progression of Pure Neuritic Leprosy to Lepromatous Leprosy With Erythema Nodosum Leprosum.

Leprosy, caused by Mycobacterium leprae, affects the skin and peripheral nerves. Pure neuritic leprosy (PNL) is a rare subtype, characterized by nerve involvement without skin lesions. We report a rare case of PNL progressing to lepromatous leprosy (LL) with erythema nodosum leprosum (ENL), with only one such case previously reported in the literature and reviewed herein. A 35-year-old male diagnosed with PNL on sural nerve biopsy showing acid-fast bacilli (3+) completed multidrug therapy for 12 months. Nine months later, he developed painful erythematous nodules and sensory-motor deficits. Sl

Erythema Nodosum Leprosum Triggered by Herpes Simplex Infection: A Case Report.

Leprosy, caused by Mycobacterium leprae, is a chronic granulomatous disease primarily affecting the skin and peripheral nerves. Erythema nodosum leprosum (ENL), or type 2 lepra reaction, is an immunologic complication of lepromatous leprosy, often presenting with erythematous nodules, fever, and systemic inflammation. Early recognition is crucial to prevent chronic complications. We report a 44-year-old man who developed facial erythematous nodules and fever following a sore throat, initially suggestive of a drug eruption or viral infection. Laboratory tests revealed neutrophilia, elevated inf

Erythema Nodosum Leprosum in a Patient with Borderline Lepromatous Leprosy: A Case Report.

BACKGROUND: Leprosy, caused by Mycobacterium leprae, presents on a spectrum ranging from tuberculoid to lepromatous disease. Borderline lepromatous leprosy represents an unstable immunological state that predisposes patients to immune-mediated reactions, including erythema nodosum leprosum (ENL), a severe inflammatory complication. CASE PRESENTATION: We report a case of a 62-year-old female with borderline lepromatous leprosy who presented with recurrent facial cellulitis and later developed disseminated ENL. She was initially diagnosed following a series of facial infections and confirmatory

Estimation of Serum Vitamin D Levels and Helminthic Coinfection in Patients with Type 2 Lepra Reactions and its Correlation with Severity of Type 2 Reactions: A Cross-Sectional Study.

BACKGROUND: Manifestations of leprosy are largely driven by host immune response, with the lepra reactions complicating the indolent course of the disease. Research indicates that the biological processes linked to vitamin deficiencies and helminth infections increase susceptibility to other infections. AIMS AND OBJECTIVES: To estimate serum vitamin D levels and helminthic coinfections in patients with type 2 lepra reactions and their correlation with the severity of type 2 lepra reactions. PATIENTS AND METHODS: A prospective cross-sectional study involving 75 leprosy patients of all ages wi

Clinical, therapeutic, and stigma-related challenges in leprosy and the emerging role of apremilast in managing leprosy reactions.

Leprosy is a chronic granulomatous infection caused by Mycobacterium leprae (or M. lepromatosis) that targets macrophages and Schwann cells. The resulting neuropathy and skin lesions can lead to lifelong deformities and disabilities. Despite being poorly transmissible and readily treatable, leprosy remains a neglected disease. Management is hampered by delayed diagnosis, multidrug resistance, recalcitrant leprosy reactions, and entrenched social stigma. We report a case of imported multidrug-resistant borderline lepromatous leprosy that progressed with recurrent, steroid-dependent erythema nod

A Wolf in Sheep's Clothing: Extensive Musculoskeletal and Cutaneous TB Masquerading as Primary Erythema Nodosum.

INTRODUCTION: Tuberculosis (TB) causes substantial morbidity and mortality globally, with continued endemicity in developing populations. Most cases of primary TB present as pulmonary TB; however, between 10% and 20% include extrapulmonary manifestations. Almost 26% of extrapulmonary manifestations include musculoskeletal TB. The risk of reactivation of latent TB is approximately 10% per year and is most common in those with immunocompromising conditions. Diagnosis of extrapulmonary TB remains elusive because of atypical presentation. CASE: We present a case of a 71-year-old man with a histor

Interaction of CTCF and CTCFL in genome regulation through chromatin architecture during the spermatogenesis and carcinogenesis.

The zinc finger protein CTCF is ubiquitously expressed and is integral to the regulation of chromatin architecture through its interaction with cohesin. Conversely, CTCFL expression is predominantly restricted to the adult male testis but is aberrantly expressed in certain cancers. Despite their distinct expression patterns, the cooperative and competitive mechanisms by which CTCF and CTCFL regulate target gene expression in spermatocytes and cancer cells remain inadequately understood. In this review, we comprehensively examine the literature on the divergent amino acid sequences, target site

Baker's Yeast Might not always be Good for Everyone - A Case of Percutaneous Gastrostomy Tube Induced Saccharomyces Cerevisiae Peritonitis in an Immunocompromised Patient.

Thalidomide is a racemic glutamic acid derivative approved in the US for erythema nodosum leprosum, a complication of leprosy. In addition, its use in various inflammatory and oncologic conditions is being investigated. Thalidomide interconverts between the (R)- and (S)-enantiomers in plasma, with protein binding of 55% and 65%, respectively. More than 90% of the absorbed drug is excreted in the urine and faeces within 48 hours. Thalidomide is minimally metabolised by the liver, but is spontaneously hydrolysed into numerous renally excreted products. After a single oral dose of thalidomide 200

Panniculitis: A Cardinal Sign of Autoinflammation.

Panniculitis was first described in the nineteenth century and is characterized by inflammation of the subcutaneous fat. It may be categorized in septal or lobular subtypes, but other histopathological features (e.g., presence of vasculitis, nature of inflammatory infiltrates, characteristics of fat necrosis) are also important for diagnostic purposes. Clinically, panniculitis is characterized by the presence of subcutaneous nodules, and both ulcerative and nonulcerative clinical subtypes have been proposed. In this review, we aimed to describe the occurrence of panniculitis in autoinflammator

Atypical and Typical Presentation of Erythema Nodosum: Clinical Differences in Treatment and Outcome.

INTRODUCTION: Erythema nodosum (EN) is the most common form of panniculitis that predominantly affects the shins. While EN in atypical sites has been described by many authors, there are currently only case studies published on this topic. This study aimed to evaluate clinical differences between patients suffering from EN on the shins, compared to patients with EN in atypical locations. METHODS: We analyzed 105 patients in a retrospective, single-center study at a university hospital in Switzerland. Typical EN was defined as lesions, found only on the lower legs, while atypical EN as lesions

MicroRNAs correlate with bacillary index and genes associated to cell death processes in leprosy.

Mycobacterium leprae infects skin and peripheral nerves causing a broad of clinical forms. MicroRNAs (miRNAs) control immune mechanisms such as apoptosis, autophagy as well as to target genes leading to abnormal proliferation, metastasis, and invasion of cells. Herein we evaluated miRNAs expression for leprosy phenotypes in biopsies obtained from patients with and without reactions. We also correlated those miRNAs with both, bacillary index (BI) and genes involved in the micobacteria elimination process. Our results show a significant increase in the miR-125a-3p expression in paucibacillary (P
