Lofgren's Syndrome: A Unique Presentation of Sarcoidosis Masquerading as Lower Extremity Cellulitis.

Löfgren's syndrome is an acute, self-limiting variant of sarcoidosis, typically presenting with erythema nodosum, bilateral hilar lymphadenopathy, and acute arthritis or periarthritis-most commonly involving the ankles. Its clinical overlap with other inflammatory conditions can pose a diagnostic challenge. We describe a 34-year-old male with no prior medical history who presented with bilateral ankle and foot pain, erythema, and swelling, initially diagnosed as cellulitis based on imaging findings. He has no response to broad-spectrum antibiotics. During his further hospital stay, he develope

Investigation of potential toxicity associated with long-term amitriptyline exposure: Evidence from genomics.

The increasing environmental detection of antidepressants such as amitriptyline (AT) has raised toxicological concerns, yet its long-term safety profile remains poorly characterized. We applied an integrative strategy combining phenome-wide association studies (PheWAS), Mendelian randomization (MR), network toxicology, and molecular docking to systematically evaluate potential adverse effects of AT. PheWAS analyses were performed across 784 phenotypes using UK Biobank and FinnGen (R10). Among these, loss-of-function mutations in AT targets SLC6A2 and SLC6A4 showed significant associations afte

Assessing disease phenotypes in Behçet's syndrome: insights from a multiple correspondence analysis.

INTRODUCTION: Behçet's syndrome (BS) is a rare systemic vasculitis. Clinical manifestations in BS are frequently clustered rather than discrete, and the concept that distinct clinical phenotypes may exist in BS has recently emerged. The aim of the present work was to identify and analyze the disease phenotypes in a monocentric historical cohort of patients with BS. METHODS: A total of 202 patients with BS diagnosis followed up at the Behçet Clinic of the Azienda Ospedaliero-Universitaria Pisana were identified, and demographics, clinical, and therapeutic data were retrospectively collected. The trends in the prevalence of arthritis and arthralgia and their associations with extra-articular manifestations in patients with Behcet's disease: A systematic review and meta-analysis.

OBJECTIVES: Articular involvement is one of the most common findings of Behcet's disease. This study aimed to systematically evaluate the trends in the prevalence of arthritis and arthralgia in patients with Behcet's disease, as well as their potential associations with extra-articular manifestations of the disease. METHODS: Observational studies that reported the prevalence of arthritis or arthralgia were included. Pooled prevalence with corresponding 95% confidence intervals (CIs) were calculated. Univariate metaregression was performed to evaluate the potential associations of articular ma

Promise of Jak Inhibition in the Management of VEXAS, Case Report with Review of the Literature.

VEXAS syndrome (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) is a recently described adult autoinflammatory disease associated with somatic mutations in the gene encoding ubiquitin-activating enzyme 1 (UBA1) in hematopoietic progenitor cells. Loss of function mutation of UBA1 results in a broad range of inflammatory and hematological conditions. To date, there are no established targeted therapies for VEXAS syndrome, especially in patients who are refractory to conventional immunosuppressive treatments. We report the case of a 75-year-old Hispanic gentleman with hypertension, dysl

Pregnancy and Behçet Syndrome: A Large Retrospective Italian Study.

INTRODUCTION: Behçet syndrome (BS) is a multifactorial disorder with several clinical manifestations that occurs during the childbearing age. The relationship between pregnancy and BS has been previously analysed as the pregnancy immunometabolic reprogramming in case of autoinflammatory and/or autoimmune diseases can influence both the course of the disease and the pregnancy outcomes. We aim to analyse the relationship between BS and pregnancy, investigating: (1) the prevalence and clinical characteristics of disease flares during pregnancy and (2) the pregnancy outcomes in a large cohort of I

Bilateral granulomatous mastitis associated with ductoscopy.

BACKGROUND: Some patients with granulomatous mastitis (GLM) exhibit nipple discharge from the healthy breast. Ductoscopy was conducted on healthy breasts with discharge in these patients, and it was observed that some patients developed contralateral GLM shortly after ductoscopy. This study investigated the association between ductoscopy and GLM formation. METHODS: Forty patients diagnosed with GLM who underwent ductoscopy on the healthy breast were included and divided into bilateral (10 cases) and unilateral (30 cases) groups. General and clinical data of the patients were collected, and si

Idiopathic Mixed Neutrophilic Panniculitis of the Eyelid: An Unusual Finding.

Panniculitis is an inflammatory condition typically presenting with painful, erythematous subcutaneous nodules on the extremities, often linked to systemic diseases such as lupus or erythema nodosum. Eyelid involvement is rare and has not been previously reported as an isolated finding. The authors report the case of a 74-year-old woman referred with presumed preseptal cellulitis secondary to a stye. Clinical signs included conjunctival chemosis, malar edema, and a firm, deep-seated mass in the inferolateral right lower eyelid. Orbital MRI revealed a well-defined lesion with homogeneous enhanc

Extra-intestinal manifestations in inflammatory bowel disease: a comparative review of pediatric and adult-onset disease.

INTRODUCTION: Inflammatory Bowel Diseases (IBD) are chronic immune-mediated disorders diagnosed in both adult and pediatric populations. In recent years, there has been a significant global increase in pediatric-onset IBD, not only in Western countries but also in newly industrialized regions. Age of onset plays a crucial role in disease behavior and prognosis, with pediatric onset often associated with a more extensive and aggressive disease. As a systemic inflammatory condition, IBD can involve multiple organ systems beyond the gastrointestinal tract, thus resulting in extraintestinal manife

VEXAS Syndrome: Genetics, Gender Differences, Clinical Insights, Diagnostic Pitfalls, and Emerging Therapies.

VEXAS syndrome (Vacuoles, E1-enzyme, X-linked, Autoinflammation, and Somatic) is a recently identified late-onset autoinflammatory disorder characterized by a unique interplay between hematological and inflammatory manifestations. It results from somatic mutations in the UBA1 gene, located on the short arm of the X chromosome. Initially, females were considered mere carriers, with the syndrome primarily affecting males over 50. However, recent evidence indicates that heterozygous females can exhibit symptoms as severe as those seen in hemizygous males. The disease manifests as systemic inflamm

Clinical Features and Phenotypic Similarities of Patients with Familial Behçet's Disease.

Background: Behçet's disease (BD) exhibits significant phenotypic diversity. The genetic basis of phenotypic variations in BD has not yet been elucidated. Based on the high frequency of familial BD, we aimed to analyze the familial aggregation of various manifestations of BD in this study. Methods: Patients with BD from 3 Turkish tertiary rheumatology outpatient clinics were evaluated. Demographic and clinical characteristics of the familial group with either a first- or second-degree relative with BD and the non-familial group were compared. Afterward, patients in the familial disease group f

Case report of extranodal NK/T-cell lymphoma mimicking Behcet syndrome.

RATIONALE: Extranodal natural killer (NK)/T-cell lymphoma is an uncommon non-Hodgkin lymphoma, prevalent in Asia. It often involves the nasal and upper airway regions but can disseminate to other sites like skin, soft tissue, testis, and gastrointestinal tract, characterized by Epstein-Barr virus association. PATIENT CONCERNS: This report discusses a 48-year-old male initially diagnosed with Behcet syndrome with dry mouth, uveitis, pruritic macules, and human leukocyte antigen-B51 positivity. Despite steroid treatment, a growing testicular mass and erythema nodosum led to reevaluation. DIAGN

Granulomatous lobular mastitis in pregnancy: report of 29 cases.

Granulomatous lobular mastitis (GLM) is a rare inflammatory breast disease, and there are few reports of GLM in pregnancy (GLMIP). Therefore, this study retrospectively analyzed cases diagnosed with GLMIP from 2011 to 2023 and found that in patients with GLMIP there were varied demographic manifestations such as age, pregnancy weeks, and numbers of pregnancy and delivery, and several associated complications including erythema nodosum, arthritis and lower extremity edema. Among them, 82.8% of the patients received integrated traditional Chinese medicine (TCM) and western medicine treatment, an

[Erythema nodosum: a case report].

BACKGROUND: Erythema nodosum is the most common clinical-pathological variant of panniculitis, presenting with erythematous nodular lesions. Infections are the most common cause. However, other causes cannot be ruled out: immunological, inflammatory, or metabolic. Therefore, the diagnosis must be multidisciplinary and direct treatment toward the underlying cause, as in most cases, it is usually the tip of the iceberg of a significant pathological process. CASE REPORT: An 18-year-old female patient with a history of hepatoblastoma, nephrocalcinosis, and recurrent infections presented one month

Recurrent Erythema Nodosum Associated With Finger Millet-Blended Injera: A Case Report on a Potential Dietary Trigger.

This case report presents a recurrent episode of erythema nodosum (EN) potentially associated with the consumption of finger millet-blended injera. The patient developed EN manifestations within 1 week of initiating intake of injera prepared from teff and finger millet, with symptoms persisting for 6 months. Following dietary elimination of finger millet, complete clinical resolution was observed, and no recurrence was documented during an 8-month follow-up. However, causality cannot be confirmed due to the lack of allergy testing, dietary rechallenge, and histopathological verification. This
