INTRODUCTION: With the increasing use of gemcitabine, more and more rare toxicities are encountered in practice. In general, gemcitabine may cause bone marrow suppression, mild cutaneous rash, and alopecia. We report a case of recurrent cutaneous toxicity during adjuvant chemotherapy with gemcitabine after complete resection of pancreatic adenocarcinoma.
CASE REPORT: The patient developed recurrent toxic erythema on the skin of his lower extremities after each cycle of chemotherapy. Because of the misleading clinical features of the presentation, he was repeatedly treated with unnecessary ant

BACKGROUND: While most patients who present with a rash have no associated systemic illness, many systemic illnesses have skin manifestations at some stage.
OBJECTIVE: This article uses case vignettes to illustrate a problem oriented approach to five commonly presented skin conditions that have common and varied systemic associations. A logical sequence of management for each case is provided.
DISCUSSION: Often the skin manifestations of systemic disease are vague and nonspecific such as the toxic erythema that might follow a viral infection or a drug eruption. Sometimes it is the systemic m

Erythema toxic neonatorum (ETN) is a common, self-limiting condition of term infants, appearing particularly within the first 48 hours of life. ETN has a worldwide distribution, with a frequency variable from 19% to 72%. Erythematous macules, wheals, papules and pustules in varying combination occur, with crops of lesions waxing and waning and spontaneous resolution within hours to days. It's important the differential diagnosis from other pustular neonatal conditions, some of which are severe. The etiology of ETN remains unknown. Recent studies reveal an activation of immune cells in the lesi

Orf is a zoonosis caused by an epitheliotropic DNA parapox virus. Human orf is a generally benign, self-limiting condition that usually regresses in 6-8 weeks without specific treatment. However, it may be accompanied by local symptoms including pain, pruritus, lymphangitis and axillary adenitis, or less frequently by systemic symptoms such as fever or malaise. Furthermore, it may be complicated by erythema multiforme, Stevens-Johnson syndrome, erysipelas, generalized mucocutaneous eruption, toxic erythema, eyelid oedema and giant, persistent or recurrent lesions in immunocompromised patients.

OBJECTIVE: Kawasaki disease (KD) was first reported by Tomisaku Kawasaki in 1967 in Japan. Large-scale nationwide epidemiologic surveys have been conducted continuously by the Japan Kawasaki Disease Research Committee; however, there were very few reports of KD before 1967. This study was performed to clarify when KD appeared in Japan.
DESIGN: We investigated the medical charts of patients who had been hospitalized at Tokyo University Hospital between 1940 and 1965.
RESULTS: We identified 10 patients whose clinical signs fulfilled the criteria for KD. The ages of the patients ranged from 8 m

Captopril, enalapril, and lisinopril are angiotensin-converting enzyme (ACE) inhibitors widely prescribed for hypertension and heart failure. Cutaneous side effects of captopril include angio-edema, anaphylactoid reactions, maculopapular eruptions, pitiryasis rosea-like rash, toxic erythema, and exfoliative dermatitis. Some of the immunological captopril-induced cutaneous adverse reactions have been diagnosed in recent years by patch tests. A case of a cutaneous immune adverse reaction to captopril with tolerance to enalapril and lisinopril demonstrated both by patch tests and double-blind cha

BACKGROUND AND OBJECTIVE: To prospectively survey skin lesions in a large group of newborns, as no such study has as yet been performed in Germany.
PATIENTS/METHODS: In a prospective study, we examined one thousand newborn babies twelve to 120 hours old.
RESULTS: In 59.7%, one or more skin lesions could be detected, some of which were only transient. Noticeable differences from other studies included a higher incidence of congenital melanocytic nevi (6%) and port wine stains (2.8%). Instead of the usual male:female ratio of 1:2 we found a 1:1 ratio for port wine stains. Toxic erythema of the

UNLABELLED: Diagnosis of pustular dermatosis occurring during the first days of life is based on clinical findings. Erythema toxicum neonatorum (ETN) is the more frequent benign self limiting eruption in the newborn.
CASE REPORTS: Three cases of ETN with localized pustules to the genitals and perineal area are described.
COMMENT: When encountering a newborn with a localized pustulosis rash, it is important to separate benign condition as ETN from those that require prompt diagnosis and therapy. Atypical ETN and pustular dermatosis due to bacterial or viral infections or inflammatory diseases

OBJECTIVE: In relatively few cases have perinatal factors been included as risk factors for allergy development. Delivery has not been considered as a possible influential factor in allergy development. To identify risk factors in allergy development, we have included erythema toxicum neonatorum (ETN).
PATIENTS AND METHODS: We have prospectively studied 356 newborns that were followed for a period of two years. Characteristics of the delivery, such as the pregnancy, instrumental delivery, circular cord, ETN, number of vesicles, day of presentation, season of birth, maternal and cord blood IgE

BACKGROUND: Arcanobacterium haemolyticum is a bacterial pharyngeal pathogen that infects adolescents and young adults, frequently causing an exanthem that may mimic a viral exanthem, toxic erythema, or drug eruption. To our knowledge, the cutaneous manifestations of A haemolyticum infection have not previously been reported in the dermatologic literature.
OBSERVATIONS: The severity of this infection can range from mild pharyngitis to a diphtheria-like illness and even septicemia. We report three cases that demonstrate the spectrum of manifestations of this disease, including an unusual case w

INTRODUCTION: Description of a case of fixed drug eruption induced by levamisole.
OBSERVATION: Fixed toxic erythema with pigmentation was observed in a patient taking levamisole as an adjuvant to 5 fluorouracil chemotherapy for cancer of the colon. The imputability of levamisole was highly likely. Epicutaneous skin tests were negative.
DISCUSSION: Several types of skin eruptions have been described after intake of levamisole. It was highly likely that levamisole was implicated here and fixed pigmented erythema should be added to the list.

Pregnant women with respiratory symptoms of pleuritic pain and productive cough should undergo evaluation for coccidioidomycosis. This should include a history of travel or residency in endemic areas and careful assessment for toxic erythema, erythema nodosum, or erythema multiforme. To confirm a diagnosis of this disease, a sputum culture, wet mount, and serological tests should be performed. The risk of dissemination, which is highest in the second and third trimesters, can be estimated by a complement-fixation titer. In disseminated cases aggressive treatment with amphotericin B has improve

Several new findings associated with hyperpigmentation have been reported in the past decade, the most notable being amiodarone-induced hyperpigmentation, which was shown to demonstrate both a lymphocytic dermatitis as well as yellowish brown granules that can be found within several cell types. The nature of this material has not been elucidated, although the drug or one of its metabolites composes at least a portion of the granules. The nature of the clofazimine-induced hyperpigmentation was shown to be caused by the accumulation of a ceroid lipofuscin within lipid-laden macrophages. Several

The authors attempted to increase the cytotoxicity of the renal hilar lymph nodes in ten patients with renal cell carcinoma by administration of recombinant interleukin-2 (IL-2) into the subcutaneous tissue of the instep. A total of 5000 to 21,000 units was given over 3 to 12 preoperative days. Mononuclear cells separated from the lymph nodes excised from the renal hilum at surgery served as the effector cells. Fresh autologous tumor cells and three cultured cell lines, K562, Raji, and ACHN, served as the target cells. Cytotoxicity was measured by a chromium-releasing assay. The results showed

Skin disorders are commonly seen in the neonatal period. A recent survey of neonates demonstrated that almost every neonate had some skin lesion. The most common skin findings are desquamation, Epstein's pearls, sebaceous hyperplasia, milia, toxic erythema, salmon patch, hypertrichosis, and Mongolian spot. In addition to these common cutaneous findings, other disorders may exist and are discussed in this article.

Envenomation by the spider Loxosceles reclusa in five pregnant women proved to have no sustained adverse effects on mother or baby when managed conservatively only with low-dose prednisone. A striking toxic erythema of the skin, common with the bite of the spider, caused the greatest discomfort and concern for the patients but proved to be entirely tractable. No episodes of hemolysis, disorders of coagulation, or renal damage were discovered. There is no clinical evidence of any special risk during pregnancy in the event of loxoscelism.

Three patients are described who developed numerous pinhead sized pustules within areas of a widespread toxic erythema. The eruption was precipitated by food poisoning in one patient, a suspected, but blood-culture-negative septicaemia in another and in the third patient, by a cephalosporin. This self-limiting syndrome consists of fever, a pustular and erythematous eruption, a neutrophil leucocytosis, subcorneal and spongiform pustules but without a history of psoriasis. We believe that this entity of toxic pustuloderma represents a severe form of toxic erythema.

A group of 420 neonates underwent total cutaneous and oral mucosal examinations during the first week of life. Skin lesions were seen in almost every baby (99.3%). The eight most common dermatoses were desquamation (65.0%), Epstein's pearls (56.0%), sebaceous hyperplasia (48.0%), milia (36%), toxic erythema (34.8%), salmon patch (33.8%), hypertrichosis (29.0%), and Mongolian spot (25.5%). Congenital melanocytic nevi were clinically diagnosed in 9 of 420 babies (2.1%); the majority of the lesions were small, that is, less than 1.5 cm in diameter. These neonates had a dark complexion (all had br
