Eight patients had a chronic photodermatitis of five to 19 years' duration. The clinical and photobiological features were consistent with the pattern observed in persistent light reactors. Severity and clinical presentation, however, were quite variable, ranging from a few isolated papules to extensive lichenification involving the entire exposed skin. The Scandinavian photopatch test procedure has been applied to 745 patients with suspected photodermatoses during the years 1980-1981. Our experience has been encouraging with the recording of several relevant reactions. A total of 132 positive photocontact reactions and 120 ordinary contact reactions were seen. Photocontact reactions to musk ambrette (19 cases) and PABA (19 cases) were surprisingly frequent. Patch and photopatch test studies in 23 patients with reactions to sunscreen preparations revealed 17 photopatch and 38 patch test-positive reactions. The offending allergens could be traced in 18 of the 23 patients. Allergy to PABA was demonstrated in 11 patients, 5 of whom had photocontact allergy. Photocontact allergy to oxybenzone was seen in 3 patients, while 8 were sensitive to cinnamates, and 2 of these had a photocontact allergy. The response of a patient with the PD/AR syndrome to patch and photopatch testing with musk ambrette, musk ketone, and musk xylol was investigated. In addition, solutions of these nitro-musks were allowed to photodecompose prior to patch and photopatch testing. Only the photopatch tests with musk ambrette and with the photodecomposed musk ambrette gave positive results. The results do not provide support for the hypothesis that photocontact allergy to musk ambrette is in fact a contact allergy to a photodecomposition product of musk ambrette.

194 patients were standard photopatch tested with Waxtar as is (coal tar 5%) and 161 patients were photopatch tested with para-aminobenzoic acid (PABA) 5% in alcohol, potassium dichromate 0.5% in petrolatum, and a mixture of balsams of Peru as is. 495 patients were standard photopatch tested with Musk Ambrette 5% in petrolatum and in dimethyl phthalate. 4 cases gave positive photopatches; 3 were men and 1 a woman. 2 cases, both men, had current or past relevant reactions to Musk Ambrette presented with typical perfume dermatitis. The other 2 cases had positive reactions to Musk Ambrette of uncertain relevance. Photoallergic reaction to Musk Ambrette is a rare event. Only a few relevant reactions will be missed by excluding it from standard phototesting.

Induction in mice of marked photoallergic contact dermatitis (PCD) to 3,3',4',5-tetrachlorosalicylanilide (TCSA) with UVA (320 to 400 nm) radiation requires pretreatment with cyclophosphamide (CY). Photosensitivity is the broad term used to denote adverse reactions to the sun. These reactions may be phototoxic or photoallergic. Phototoxic reactions are much more common than photoallergic reactions. A farmer specialized in pig-breeding showed acute dermatitis of his hands, forearms and face after using chloropromazine for the sedation of pigs. Epicutaneous and photo-patch tests showed positive reactions to chloropromazine. Further tests with other occupational materials were negative. This observation points out that in cases of contact dermatitis in farmers, besides antibiotics, vitamins and other feeding additives, sedatives of the phenothiazin-type should not be forgotten.

This report describes modifications in the techniques used for both the induction and elicitation of photoallergic contact dermatitis (PACD) in the guinea pig. These changes have improved the reliability of this animal as the model of choice for screening chemicals or products for their tendency to produce PACD. The case of a 42-year-old woman with sunlight-provoked eczematous eruptions recurrent over several years is reported. Photopatch-testing with cosmetic products and sunscreen preparations used by the patient as well as the active ingredient of the sunscreen formulations revealed photosensitivity to the UV-blocking agent 2-hydroxy-4-methoxybenzophenone. The active wavelengths were shown to be in the UV-A range.

We have induced photoallergic contact dermatitis in mice to 3,3',4',5 tetrachlorosalicylanilide (TCSA), chlorpromazine and 6-methylcoumarin. These compounds are known to produce photoallergic contact dermatitis in humans. The photoallergic contact dermatitis reaction in the mouse is immunologically specific viz. mice photosensitized to TCSA react, by photochallenge, to that compound and not to chlorpromazine, and conversely. 4 patients with chronic photocontact dermatitis were sensitive to cobalt salts. They presented as cases of contact dermatitis from cement or pig fodder with persistent lesions on exposed areas. Only 2 of them had standard patch test positive reactions to chromate and cobalt, but all showed positive photo-patch tests to cobalt. Photo-oxidation tests proved that cobalt salts are photosensitizing. Exposure to cobalt salts was responsible for the patients' chronic actinic dermatitis.

This study reports a photoallergic contact dermatitis to musk ambrette in two male patients. Musk ambrette is a widely used perfume ingredient, but it is also included in food and drugs. Both patients reacted in the long ultraviolet light (UV-A) to musk ambrette and to the after shave lotions containing musk ambrette. The histopathologic finding in the photopatch test site revealed acute spongiotic dermatitis.

During a period of 2 1/2 years, 7 of 2000 patients routinely tested at our laboratory revealed contact allergy to oak moss in perfumes. All reacted to a mixture of different lichens and to some specific lichen compounds. The sensitivity was probably induced by cosmetics containing lichen substances. The following 3 compounds caused reactions in all patients tested: atranorin, evernic and usnic acids. 3 patients were photosensitive, but stronger reactions were elicited by prolonged contact during occlusion of the patches and complete protection against light, rather than by irradiation alone. A 78-year-old man experienced pruritus, burning, and redness within minutes of exposure to sunlight. The immediacy as well as the temporary nature of the response was similar to that seen in solar urticaria, but in sharp contrast to the delayed reactions associated with sunburn and photocontact dermatitis. Photo-patch tests revealed a specific photosensitivity to formaldehyde. Despite its ubiquitous presence and its capacity to produce allergic contact dermatitis, formaldehyde, to my knowledge, has not been previously found to be a photosensitizer.

Photoallergic contact dermatitis developed in a patient who had used deodorant soaps containing tribromsalan. He persistently reacted to light, since the dermatitis persisted long after cessation of contact with the causative chemical.
