Fixed solar urticaria (FSU) is an extremely rare type of solar urticaria characterized by urticarial wheals appearing frequently confined to fixed areas of the skin. After a few minutes of exposure to sunlight or other sources of radiation, urticarial lesions can usually be induced exclusively in the same localization. We report a case of delayed onset FSU occurring 6 hours after exposure to ultraviolet A and B light.

Idiopathic solar urticaria (SU) is a rare, debilitating photodermatosis, which may be difficult to treat. First-line treatment with antihistamines is effective in mild cases, but remission after phototherapeutic induction of tolerance is often short-lived. Other treatment options include plasma exchange, photopheresis and cyclosporin. We present two cases of severe, idiopathic SU, which were resistant to conventional treatment. Both patients achieved remission after administration of intravenous immunoglobulin (IVIg) and have remained in remission at 13 months and 4 years, respectively. There

Erythropoietic protoporphyria (EPP) is an inherited disorder of the haem metabolic pathway characterised by accumulation of protoporphyrin in blood, erythrocytes and tissues, and cutaneous manifestations of photosensitivity. EPP has been reported worldwide, with prevalence between 1:75,000 and 1:200,000. It usually manifests in early infancy upon the first sun exposures. EPP is characterised by cutaneous manifestations of acute painful photosensitivity with erythema and oedema, sometimes with petechiae, together with stinging and burning sensations upon exposure to sunlight, without blisters.

A case of solar urticaria is presented, followed by a discussion of the clinical characteristics, pathophysiology, diagnosis, and management of this disease. Special emphasis is given to clinical pearls and pitfalls for the practicing allergist. Solar urticaria is a physical urticaria that can be difficult to diagnose and distinguish from other photodermatoses. There are some characteristic features that are important to remember when evaluating a patient with suspected solar urticaria. Testing can be difficult without the assistance of an experienced dermatologist because there are several di

Physical urticarias are a group of mucocutaneous disorders characterised by the appearance of whealing and/or angioedema and induced by physical stimuli. A 24-year-old Caucasian male affected by ultraviolet A-induced solar urticaria, specific food-dependent exercise-induced anaphylaxis, symptomatic dermographism and allergic rhinoconjunctivitis is described.

BACKGROUND: Polymorphic light eruption and erythropoietic protoporphyria (EPP) have been demonstrated to have a moderate and large impact on the quality of life (QoL) of patients, respectively. However, there is little information available about the impact of other photodermatoses on QoL.
OBJECTIVES: To assess and compare the impact of all forms of photodermatoses on patients' QoL using the standard 1-week Dermatology Life Quality Index (DLQI) questionnaire and a modified questionnaire to assess the impact over the previous year.
METHODS: All patients with photodermatoses seen between 2001

BACKGROUND: It can be difficult to provide patients with idiopathic solar urticaria adequate protection from sunlight. In a nonrandomized controlled trial, we used a standardized phototest procedure to determine the effects of using sunscreen and antihistamine to control idiopathic solar urticaria.
OBSERVATIONS: Three patients with idiopathic solar urticaria underwent phototesting with UV-B and UV-A radiation. The minimal urticarial dose (MUD) was determined 15 minutes after irradiation. The patients were subsequently tested with 5 times the MUD, and the reaction was graded every minute for 1

Solar urticaria is an idiopathic, chronic and rare photodermatosis, characterized by the sudden onset of pruritic urticarial hives and plaques on the exposed areas of the skin, after a brief period of exposure to the natural sunlight or to an artificial light source. A Caucasian 27-year-old man presented with clinical features suggestive of solar urticaria was referred to our photodermatology unit, where phototesting confirmed the diagnosis of solar urticaria induced by visible light. As he was refractory to oral antihistamines and had slight improvement under UVA plus visible phototherapy, hu

Solar urticaria is a relatively rare immunoglobulin E-mediated photodermatosis that is caused by specific, yet diverse wavelengths of light. The history, epidemiology, clinical manifestations, histology, etiology/pathogenesis, differential diagnosis, treatment, course, and prognosis of solar urticaria are reviewed herein.

Photoallergic dermatoses are skin lesions following sun exposure, characterized by activation of immunological mechanisms, involving photosensitizers and photoallergens that can cause photosensibilization in some individuals. In this group of disorders, photoallergic contact dermatitis should be accentuated. It manifests as contact allergic dermatitis on sun-exposed skin areas, following direct contact with photoallergens during UV exposure (predominantly UVA). Under influence of light, photosensitizers get activated, followed by fusion with cutaneous proteins which renders them to complete an

BACKGROUND: Solar urticaria manifests itself immediately after solar or artificial light exposure and disappears a little later. Histopathologic findings of solar urticaria are essentially identical to those of classic urticaria.
CASE REPORT: We report a 41-year-old man who developed urticarial lesions some hours after sunlight exposure, which resolved after approximately 1 week. Histologic examination of the lesions evidenced a leukocytoclastic vasculitis.
CONCLUSION: In a literature review we found one case of solar urticaria with histologic aspects of leukocytoclastic vasculitis.

BACKGROUND: The incidences of idiopathic photodermatoses (IP) are increasing and the available therapeutic methods are often inadequate.
AIM: To evaluate whether, in subjects affected by IP not responding to the usual available therapies, the oral administration of an extract of Polypodium leucotomos (PL) could provide an effective photoprotective activity.
METHODS: 26 patients with polymorphic light eruption and two with solar urticaria were recruited to enter the study. The protocol excluded the use of ultraviolet protection filters or other drugs that could in some way interfere with expo

Chronic urticaria is highly prevalent in the general population, and while there are multiple treatments for the disorder, the results obtained are not completely satisfactory. The second-generation H1 antihistamines remain the symptomatic treatment option of choice. Depending on the different pharmacokinetics and H1 receptor affinity of each drug substance, different concentrations in skin can be expected, together with different efficacy in relation to the histamine-induced wheal inhibition test--though this does not necessarily have repercussions upon clinical response. The antiinflammatory

BACKGROUND: The frequency of photodermatoses in African Americans has not been well characterized.
OBJECTIVE: To evaluate the frequency of photodermatoses in African Americans in an academic medical center during a 7-year period. This was compared with that observed in Caucasians seen during the same period.
METHODS: A retrospective chart review of 2200 dermatology clinic charts from August 1997 to September 2004 was performed. Charts of patients with International Classification of Diseases, Ninth Revision diagnostic codes related to photodermatoses were included.
RESULTS: Two hundred eigh

BACKGROUND: The inhibition spectrum (IS) in solar urticaria was identified mainly in Japanese patients with solar urticaria, although the mechanism of action of the IS has not been elucidated.
METHODS: Because an intradermal injection of action spectrum (AS)-irradiated serum in a case of solar urticaria induced a wheal response, we studied the responsiveness of the intradermal injection after an IS irradiation.
RESULTS: An AS in this patient was composed of visible light shorter than 500 nm, while an IS was composed of visible light longer than 530 nm. When the IS was exposed immediately aft
