Skin tests of a novel nicotine patch, PHK-301p, in healthy male volunteers: phase I, placebo-controlled study.

Major local adverse reactions in the nicotine patches are skin reactions. To assess the skin reaction of PHK-301p, a newly developed nicotine patch, we conducted a phase I study that consisted of 2 parts: a skin irritation test (48-h closed patch test) and a photosensitivity test (24-h closed patch test + Ultraviolet A irradiation). Twenty healthy men were treated with PHK-301p and placebo. Both preparations were punched out to a circle of 6-mm diameter and were applied simultaneously to each participant. Skin irritation and photosensitivity were assessed by a physician who was kept unaware of

[Histopathology of photodermatoses].

Photodermatoses are skin disorders which result from abnormal response to ultraviolet (UV) radiation. Photodermatoses include the following disorders: polymorphic light eruption, hydroa vacciniforme, solar urticaria, actinic prurigo, acute actinic dermatosis, solar dermatosis, phototoxic dermatitis and photoallergic dermatitis. The histopathology of these disorders will be presented and its role in making the diagnosis will be discussed.

Photodermatitis with minimal inflammatory infiltrate: clinical inflammatory conditions with discordant histologic findings.

Dermatoses associated with cutaneous photosensitivity are a group of photodistributed skin eruptions caused or exacerbated by light. Multiple clinical variants of photosensitive dermatoses have been characterized including polymorphous light eruption, chronic actinic dermatitis, solar urticaria, phototoxic and photoallergic dermatitis, reticular erythematous mucinosis, acute cutaneous lupus erythematosus, and dermatomyositis. As there may be significant overlap among the clinical presentation of these conditions, the specific diagnosis of individual photodermatosis relies heavily on characteri

Analysis of photodermatoses seen in a predominantly Asian population at a photodermatology clinic in Singapore.

BACKGROUND: The idiopathic photodermatoses have been reported to be rarer in tropical Singapore than in countries of higher latitude, with photoaggravated dermatoses and systemic phototoxicity making up most of the photodermatoses seen here. This study aims to reassess the spectrum of photodermatoses seen at the National Skin Centre, Singapore, compared with almost a decade ago, and analyse the clinical and photobiological characteristics, as compared with other countries. MATERIALS AND METHODS: We reviewed the clinical data of 141 patients phototested from January 2000 to December 2001, and

Solar urticaria and porphyrias.

The importance of disturbances of porphyrin metabolism in solar urticaria is discussed. 15 cases of porphyrias with sun sensitivity are presented comprising 5 cases of erythropoietic protoporhyria, 8 cases of coproporphyria hereditaria and 2 cases of porphyria variegata, 9 out of these 10 patients presented neuroabdominal symptoms and in 4 cases contraceptive pills have triggered the disease. Due to the risk of severe forms of disease sometimes drugs induced porphyrias must be considered in all cases of solar urticaria and a correct laboratory study done in the suspected cases.

Fixed solar urticaria induced by UVA and visible light: a report of a case.

Fixed solar urticaria (FSU) is a rare and less severe subgroup of solar urticaria. It is characterized by urticarial eruptions, which occurs on the same parts of the body following sun exposure. The lesions are reproducible at the same sites with similar morphology and distribution pattern after repeated sun exposure. The action spectrum of FSU is broad (300-700 nm). We reported a case of FSU induced by UVA and visible light. The patient responded well to cetirizine treatment.

Solar angioedema: an uncommonly recognized condition?

Solar urticaria is a well defined although uncommon photosensitivity disorder, and is said to be the underlying cause of chronic urticaria in approximately 0.5% cases. In contrast, solar angioedema is seldom reported. We describe two patients with postulated solar angioedema, associated with clinical and/or phototest features of solar urticaria. Recognition of solar provocation of angioedema has important consequences for patient management.

[Solar urticaria. Study of 20 cases].

INTRODUCTION: Solar urticaria is an infrequent disorder, but is probably underdiagnosed. It is characterized by the sudden appearance of weals in areas that are not usually photoexposed after exposure to the sun or to artificial sources of visible or ultraviolet light. Few cases have been published in literature, so the information available about this disorder and its natural evolution is limited. MATERIAL AND METHODS: We have compiled data from 20 patients diagnosed with solar urticaria in our department in the last 12 years (1990-2002) in order to try to obtain information about the charac

Solar urticaria in Singapore: an uncommon photodermatosis seen in a tertiary dermatology center over a 10-year period.

BACKGROUND: Solar urticaria is an uncommon photodermatosis, characterized by the appearance of pruritic wheals after sun exposure. In this study, we examine the photobiological characteristics of solar urticaria in the heterogeneous group of Singaporean patients. METHODS: The photobiological features of all patients treated for solar urticaria at a tertiary dermatology center in Singapore over a 10-year period were retrospectively examined. RESULTS: A total of 19 patients were diagnosed to have solar urticaria from 1993 to 2002. The mean age at diagnosis was 26 years, with a racial distribut

[Treatment of solar urticaria by intravenous immunoglobulins and PUVA therapy].

BACKGROUND: Solar urticaria is a rare form of urticaria occurring a few minutes after solar exposure. It is particularly incapaciting because it limits outdoor activities. Antihistamine and phototherapy are sometimes disappointing. CASE REPORT: A 32 Year-old woman had been suffering from severe solar urticaria since November 2000, which was confirmed by photobiological data. High-dose antihistamine treatment (fexofenadine 180 mg twice a day) was inefficient. Despite a first UVA desensitization, PUVAtherapy produced only a partial improvement and short lasting for protection, with an important

Physical urticaria.

Physical urticarias are a unique subgroup of chronic urticaria in which patients develop urticaria secondary to environmental stimuli. Common triggers include exercise, temperature changes, cold, heat, pressure, sunlight, vibration, and water. Systemic symptoms have occurred during severe episodes. Physical urticarias are responsible for approximately 20% to 30% of all cases of chronic urticaria. A basic knowledge of these unusual disorders is important for all healthcare providers. This article covers the following types of physical urticarias: dermatographism, cholinergic urticaria, local he

Detection of a highly toxic clone of Actinobacillus actinomycetemcomitans (JP2) in a Moroccan immigrant family with multiple cases of localized aggressive periodontitis.

The bioactive phyto-components of M. charantia L. demonstrate significant therapeutic potential against the acetylcholinesterase (AChE) enzyme, which is associated with neurodegenerative diseases such as Alzheimer's. In this study, computational tools were used to screen the essential bioactive compounds of M. charantia L. against AChE. Docking results revealed that among the 19 phytocompounds analysed, Ajmalacine, Alkaloid AQC2, Alkaloid SP-K, Steroid U, and Quinine exhibited high binding affinities towards AChE. Ligand-protein binding interactions indicated that these selected compounds show

Classification and evaluation of photodermatoses.

Photodermatoses can be classified into five general categories: 1) idiopathic photodermatoses, including polymorphic light eruption (PMLE), actinic prurigo, hyroa vacciniforme, chronic actinic dermatitis, and solar urticaria; 2) photodermatoses which are secondary to exogenous agents, including phototoxic and photoallergic reactions; 3) photodermatoses secondary to endogenous agents, mainly the porphyrias; 4) photoexacerbated dermatoses, including autoimmune disease, infectious conditions, and nutritional deficiencies; and 5) genodermatoses. The initial step in evaluating a photosensitive pati

Mechanisms of phototherapy and photochemotherapy for photodermatoses.

Most photodermatoses represent indications for preventive ultraviolet (UV) phototherapy and/or psoralen plus ultraviolet A (PUVA) photochemotherapy. The aim of treatment is to prevent the outbreak of disease by increasing the patient's tolerance to sunlight. The mechanisms by which ultraviolet B (UVB) and PUVA induce such tolerance are not completely understood. Pigmentation and skin thickening may be important factors in the protective effect, but they cannot sufficiently explain the degree of protection induced. Other mechanisms that may be of critical importance for the therapeutic efficacy

[Solar urticaria].

Solar urticaria serves as a model to illustrate the photodiagnostic approach in rare photodermatoses and to demonstrate how patients can be effectively helped by prophylactic and therapeutic intervention. To accomplish these tasks, advanced knowledge and experience in photodermatology is required, along with equipment which is often available in specialized centers only. A list is attached listing the dermatology departments with photodiagnostic units in Germany, Austria, and Switzerland.

A case of type IV solar urticaria identified by reverse in vitro serum test.

We had a 17-year-old male patient with solar urticaria diagnosed as type IV of modified Harber's classification. The action spectrum of this case was estimated to be 433-499 nm and the inhibition spectrum was 533 nm. Both in vitro serum test and tentatively designated "reverse in vitro serum test" were positive. The patient was prescribed 20 mg/day epinastine hydrochloride (Alesion) orally for 49 weeks with improvement of the symptoms.
