Solar urticaria in a 1-year-old infant: diagnosis and management.

Solar urticaria is a type of inducible urticaria triggered by sun exposure resulting in hives on exposed skin as well as risk of developing systemic symptoms. Most cases are reported in the adult population. However, cases do occur in children. We present a case of visible light induced urticaria in a 12-month-old girl that demonstrates the need for accurate diagnosis and appropriate management.

Novel pentapeptide activators of mammalian and mushroom tyrosinase.

Melanoma incidence continues to rise due to intentional exposure to ultraviolet radiation (UVR) from sunlight and indoor tanning beds. Eumelanin exhibits photoprotective effects; thus, agents that induce its synthesis offer a means for sunless tanning without UVR damage. Herein, we report the development of two pentapeptides, P9 and P10, capable of enhancing melanin synthesis in B16 melanoma cells by activating mushroom and mouse tyrosinases without any effect on cell viability or proliferation. P9 and P10 significantly increased melanin content in a dose-dependent manner comparable to the pos

Evaluation of patients with photodermatoses.

The systematic evaluation of photosensitive patients involves a comprehensive history, physical examination, phototesting, and, if necessary, photopatch testing and laboratory evaluation. Polymorphous light eruption, chronic actinic dermatitis, solar urticaria, and photosensitivity secondary to systemic medications are the most commonly encountered photodermatoses in dermatology clinics worldwide.

Hydroa vacciniforme and solar urticaria.

Hydroa vacciniforme (HV) and solar urticaria (SU) are uncommon immunologically mediated photodermatoses. HV occurs almost exclusively in children, usually beginning in childhood and remitting spontaneously by adolescence. Association with chronic Epstein-Barr virus infection has been reported in HV, which raises the possibility of lymphoproliferative disorders in these patients. SU is characterized by skin erythema, swelling, and whealing immediately after sun exposure. Although several treatment options are available, the management of both conditions remains a challenge.

Minireview: Peptide analogs and short sequence oligopeptides as modulators of skin pigmentation.

Short sequence amino acids or oligopeptides have recently garnered attention for use as treatments for a myriad of dermatologic disorders due to their ability to effect and modulate various biological processes in the epidermis and dermis, rendering them promising candidates as medical and cosmeceutical therapeutics. Major advantages include their relative ease of synthesis and multitude of modifications that can be applied to enhance potency, affinity, specificity, hydrophilicity or hydrophobicity and cytotoxicity. Given the photoprotective effects of eumelanin on skin, there has been substan

Severe and refractory solar urticaria treated with intravenous immunoglobulins: a phase II multicenter study.

BACKGROUND: Retrospective data have suggested the effectiveness of intravenous immunoglobulins (IVIG) for solar urticaria (SU). OBJECTIVE: We sought to prospectively assess the efficacy of IVIG for SU. METHODS: We conducted a multicentric phase II study to test the efficacy of a single course of IVIG (2 g/kg) in patients with severe and refractory SU. The primary outcome was remission of SU on phototesting at 12 weeks after IVIG treatment. Secondary objectives included clinical remission, improved quality of life, and 50% improvement in disease intensity as measured on a visual analog scale.

Dissociation between history and challenge in patients with physical urticaria.

BACKGROUND: Physical urticaria is a subtype of chronic urticaria induced by a physical stimulus. OBJECTIVE: To evaluate the consistency between a history of physical urticaria and results of challenge testing. METHODS: Seventy-six subjects, ages 3 to 77 years old, were referred with the diagnosis of a physical urticaria and were evaluated by using challenge testing directed toward the presenting diagnosis, yet included other stimuli based on history. The majority of subjects were tested to 3 or more stimuli, thus 294 provocation tests were performed. Fifty-seven subjects were surveyed for th

Retreatment with omalizumab results in rapid remission in chronic spontaneous and inducible urticaria.

IMPORTANCE: Omalizumab has emerged as a novel and effective treatment option for patients with antihistamine-resistant chronic urticaria. It is unclear whether patients with recurrent urticaria symptoms after discontinuation of omalizumab treatment can benefit from retreatment. OBJECTIVE: To assess the response of patients with chronic urticaria who receive omalizumab retreatment. DESIGN, SETTING, AND PARTICIPANTS: Retrospective analyses were conducted of outpatients treated at an urticaria specialist center of a university hospital. Participants included 25 consecutive patients (aged 18-74

Photodermatoses: environmentally induced conditions with high psychological impact.

Photodermatoses are a group of skin disorders caused or exacerbated by ultraviolet and/or visible radiation, which collectively affect a high proportion of the population and substantially affect quality of life (QoL). Our objective was to assess the psychological impact of these conditions. Patients with a range of photodermatoses diagnosed at a specialist investigation centre in the UK completed questionnaires evaluating (i) anxiety and (ii) depression, using the Hospital Anxiety and Depression Scale (HADS), (iii) social anxiety, using the Fear of Negative Evaluation measure (FNE), (iv) copi

Photodermatoses in pigmented skin.

Photodermatoses are a group of skin diseases primarily caused by, or exacerbated by exposure to ultraviolet and or visible radiation. The effect of sunlight on skin depends on a number of factors including skin colour, skin phototype and the content and type of melanin in the skin. There are only a few studies describing photodermatoses in populations with dark skin. A PubMed search was conducted to summarize currently available information on differences in biology of melanin in dark and light skin and photodermatoses in dark skin. Dark skin is characterised by higher content of melanin, high

Solar urticaria: a time-extended retrospective series of 61 patients and review of literature.

BACKGROUND: Solar urticaria is a rare, disabling, chronic disease. Few large series are available. OBJECTIVE: To report the epidemiological, clinical, phototesting, treatment and patient outcome data of a large series. METHODS: Data from 61 patients' files were retrospectively retrieved in a tertiary referral centre. RESULTS: 43 women and 18 men were included (mean age at first symptoms: 34 y). 3 patients had a medical history of chronic urticaria and 29% of atopy. Urticaria occurred before the 15(th) minute of sun-exposure in 95% of the patients and resolved spontaneously after its interru

Energy-saving lamps and their impact on photosensitive and normal individuals.

BACKGROUND: A preliminary investigation showed that ultraviolet radiation (UVR) emissions from compact fluorescent lamps (CFLs) can pose a risk to the skin of photosensitive individuals. OBJECTIVES: To carry out a larger-scale study, in patients with a range of photodermatoses, to assess this risk. To determine a safe alternative light source for photosensitive individuals. To investigate if CFL emissions have the potential to induce skin responses in normal individuals. METHODS: Two hundred patients were directly exposed to a single-envelope CFL as part of their routine management. Irradiat

Influence of narrowband UVB irradiation of serum on autologous serum skin test results.

BACKGROUND: Autologous serum skin test (ASST) is an in vivo test to demonstrate circulating endogenous proinflammatory or wheal-inducing factors in urticaria patients. While ASST was negative in a patient with solar urticaria and in a patient with localized heat urticaria, test results turned to positive when performed with visible light-irradiated and heated serums, respectively. These data suggest that some factors and/or antibodies in serum may be activated or inhibited with different factors. We aimed to investigate whether or not ASST results change when performed with narrowband ultravio

Physical urticarias and cholinergic urticaria.

Physical urticarias are a unique subgroup of chronic urticaria in which urticarial responses can be reproducibly induced by different specific physical stimuli acting on the skin. These conditions include urticaria factitia/symptomatic dermographism, delayed pressure urticaria, cold contact urticaria, heat contact urticaria, solar urticaria, and vibratory urticaria/angioedema. Physical urticarias and cholinergic urticarias are diagnosed based on the patients' history and provocation tests including trigger threshold testing where possible. Treatment is mainly symptomatic. Many patients benefit

Successful and long-lasting treatment of solar urticaria with ultraviolet A rush hardening therapy.

BACKGROUND: Solar urticaria (SU) is a photodermatosis that is thought to be caused through the effects of mast cell mediators released because of an altered chromophore, possibly a photoallergen recognized by IgE. Phototherapy for SU to induce a tolerant state appears to be most effective, but is often time consuming and provides only short-lived remission. Ultraviolet (UV) A rush hardening has been successful and less time consuming in serum factor-negative patients with SU. However, the mechanism of action and long-lasting effects of UVA rush hardening therapy remain unclear. OBJECTIVES: We
