Polymorphous light eruption: PUVA-induced skin hyporeactivity.

Eight patients with polymorphous light eruption (PLE) were treated with the 8-methoxypsoralen (0.6 mg/kg) and UV-A given in an ordinary PUVA cabin. Excellent or good results were observed in 4 and moderate in others remained patients. It is concluded that PUVA treatment is a safe form of therapy which exercises its beneficial effect in the patients with PLE. Various theories concerning the mode of action of photochemotherapy in PLE are discussed in this paper.

Characterization of the dermal infiltrates in Jessner's lymphocytic infiltrate of the skin, polymorphous light eruption and cutaneous lupus erythematosus: differential diagnostic and pathogenetic aspects.

In the present study a comparative immunohistochemical study was performed on skin biopsies from of patients with Jessner's lymphocytic infiltration of the skin (LIS), polymorphous light eruption (PLE), discoid lupus erythematosus (DLE) and subacute cutaneous lupus erythematosus (SCLE) using a large panel of monoclonal antibodies against T cell differentiation antigens (CD3, CD4, CD8), immunoregulatory T cell subsets (CD7, 4B4, 2H4, Leu 8), B cells (CD22), activated cells CD25, OKT9, HLA-DR), Langerhans cells (CD1) and macrophages (Leu-M5). The results showed many similarities between LIS and

Reproduction of the skin lesions of polymorphous light eruption: a case report and a review of the Japanese literature.

A case of polymorphous light eruption with plaque-type lesions is reported. A 55-year-old male had pruritic erythemas and plaques on the head, the face, the nape, the back of the hands, and the left loin within days following sun exposure. The patient's minimal erythema dose (MED) (60 mJ/cm2) of sunlamp irradiation for ultraviolet (UV) B was normal. Three once-daily exposures to 2 MED of UVB (Epstein's repeat phototest technique) produced pruritic papules with a similar microscopic appearance to that of the patient's skin lesion, indicating that the action spectrum causing the lesion seemed to

[Development of a calibrating control unit for a grating monochromator and its clinical application].

For precise measurements of minimal erythema dose (MED) in photosensitive dermatoses, a calibrating control unit was developed for a grating monochromator. Seasonal and sexual differences of wavelength-specific MED and the effects of different half-value wave-width for each wavelength were determined in 64 healthy adults. Wavelength-specific MED was studied using the apparatus in 19 patients with photosensitive dermatoses, including 2 patients with xeroderma pigmentosum, 2 patients with polymorphous light eruption, one patients with actin reticuloid, 2 patients with drug-induced photosensitive

HLA typing in actinic prurigo.

Thirty-two actinic prurigo patients of Cree ancestry underwent human lymphocyte antigen (HLA) typing and were compared with 32 control subjects of Cree ancestry. We found a significantly increased frequency of HLA-A24 and Cw4 antigens and a significant decrease in the frequency of the A3 antigen in actinic prurigo patients. These HLA associations may be helpful in determining whether actinic prurigo is a distinct disease or a variant of polymorphous light eruption.

Repeated exposure to ultraviolet A in polymorphous light eruption patients and normal subjects: a clinical and histopathological study.

The clinical and histopathological responses to repeated exposures of ultraviolet A were studied in patients with polymorphous light eruption and in normal controls. Variable degrees of perivascular and diffuse infiltrates (lymphocytic and neutrophilic) were detected in both groups. These findings indicate that more specific parameters should be used to distinguish reactive from pathological responses.

Common cutaneous disorders in athletes.

Athletic activity may cause or aggravate skin disorders, which in turn may diminish athletic performance. Since many sporting activities necessitate prolonged exposure to the sun, athletes must avoid painful sunburn which will adversely affect their performance. Drugs and chemicals also may cause photoallergic and/or phototoxic reactions, including polymorphous light eruption and athletes should thus avoid photosensitising drugs and chemicals. The effects of chronic ultraviolet exposure include ageing, pigmentation and skin cancers. The most effective protection against excessive exposure to s

Polymorphous light eruption: successful reproduction of skin lesions, including papulovesicular light eruption, with ultraviolet B.

Thirty patients suffering from polymorphous light eruption (PLE), selected by criteria pointing to UVB sensitivity, were phototested for reproduction of skin lesions. Twenty-seven patients (90.0%) had symptoms compatible with or very similar to papulovesicular light eruption (PVLE). Of the 30 patients, 56.7% developed typical lesion of PLE at the test site by provocative phototesting with UVB. Eruptions of the immediate onset type were apt to be reproduced by multiple repeated irradiation, but those of the delayed onset type were reproduced by a single high-dose exposure. In addition, pruritus

Epidemiological studies of the influence of sunlight on the skin.

We examined and interviewed 809 randomly selected individuals (56% women and 44% men, mean age 44 years) from 4 Swedish cities; 1.7% had the sun-reactive skin type I, 26.2% type II, 61.3% type III and 10.7% type IV. Some variations in the distribution of skin types were found between the cities. One hundred subjects (12.4%), mostly women (p less than 0.001), had a history of polymorphous light eruption. People with skin type IV had significantly fewer facial skin problems than the rest of the subjects (p less than 0.05). The majority of subjects with atopic eczema, acne vulgaris or seborrheic

[Characteristics of polymorphous light dermatosis--results of a prospective survey and study of 302 affected patients].

In a prospective study on 302 patients (females = 87%), we registered the historical and clinical data of polymorphous light eruption (PLE) and compared our results to similar studies from Finland, Sweden, and the USA. The mean age of onset of the disease was 24.0 years; the average duration time was 10.1 years. We found the following clinical and historical characteristics of PLE: skin types I and II - 49%; positive family history - 29%; latent period between sun exposure and the first skin eruptions - in 39% less than 2 hours, in 5% 3 days and more. Mostly we observed paular eruptions and st

Preventive effect of an E. coli-filtrate (Colibiogen) in polymorphous light eruption.

During the administration of Colibiogen, a commercially available E. coli-filtrate, there was a significant (P less than 0.01) increase of the test dose of UVA necessary to elicit skin lesions in nine patients with polymorphous light eruptions. Control tests performed in the same manner without the drug indicate that this was due to treatment. Colibiogen appears to be of value in the treatment of polymorphous light eruption (PLE).

Successful treatment of severe polymorphous light eruption with azathioprine.

Two patients with severe, disabling polymorphous light eruption, who were unable to tolerate photochemotherapy and who were unresponsive to alternative recognized therapies, are described. In both cases short-term treatment with azathioprine achieved a marked clinical improvement, confirmed by testing with an irradiation monochromator. This response suggests an immunological basis for polymorphous light eruption. Patients with polymorphous light eruption vary considerably with regard to degree of photosensitivity, and while azathioprine therapy should not be considered in the majority of suffe

Jessner's lymphocytic infiltration of the skin. A clinical study of 100 patients.

Jessner's lymphocytic infiltration of the skin is a well-known but poorly understood disorder. Some doubt still exists about whether it is a single entity or a heterogeneous group that can pass into polymorphous light eruption, discoid lupus erythematosus, or even malignant lymphoma. Therefore, a large number of patients with lymphocytic infiltration of the skin (N = 100; 46 male, 54 female) were examined to elucidate these questions. We conclude that lymphocytic infiltration of the skin is a single entity. Progression into polymorphous light eruption, discoid lupus erythematosus, or lymphoma

Cutaneous and ocular side-effects of PUVA photochemotherapy--a 10-year follow-up study.

To record the potentially serious side-effects of melanoma and non-melanoma skin cancers and ocular damage following long-term PUVA chemotherapy, we re-examined 198 of 242 patients. These comprised: 90 with psoriasis, 27 with parapsoriasis, 19 with cutaneous T-cell lymphoma, 23 with vitiligo, eight with cutaneous mastocytosis, 16 with atopic dermatitis, three with prurigo nodularis, two with polymorphous light eruption and 10 with pruritus of chronic renal failure on dialysis, treated between 1977 and 1987 in our department. During the 10-year period, 11 patients died of unrelated disease. Non

Polymorphous light eruption: eliciting and inhibiting wavelengths.

In 38 patients with polymorphous light eruption (PLE) photoprovocation tests were performed by applying 100 J/cm2 of UV-A to the extensor side of the upper arm on three consecutive days. Test sites were divided into four areas by covering the patients' skin with Schott glass filters WG 320, WG 335, WG 360, and GG 385 (or GG 395). In 17 patients typical skin lesions could be provoked in at least one test field. 10 patients reacted either at all test sites or at shorter wavelengths (simultaneously higher doses) only. In the remaining 7 patients test reactions did not occur underneath filters wit

Eicosanoid release in polymorphous light eruption: selective UV-A-induced LTB4 generation by peripheral blood leukocytes.

The biochemical basis of ultraviolet-induced cutaneous inflammation in polymorphous light eruption (PLE) is not known. We investigated the potential role of eicosanoids--derivatives of arachidonic acid--as mediators of inflammation in this common disorder. 13 patients were investigated, in whom skin lesions could be experimentally reproduced by repeated UV-A but not UV-B irradiation. Peripheral blood leukocytes of these patients were irradiated with UV-A or UV-B, and the release of leukotriene B4 (LTB4), LTC4 and prostaglandin E2 (PGE2) was measured. Cells from PLE patients, but not healthy co
