Genetics versus clinical risk scores for melanoma prediction.

BACKGROUND: Cutaneous melanoma is a common cancer, for which risk stratification has been proposed to aid early detection. OBJECTIVES: To assess the performance of a polygenic risk score (PRS) in predicting the risk of invasive cutaneous melanoma, alone and combined with clinical risk factors. METHODS: The PRS was derived from the most recent genome-wide association study meta-analysis of cutaneous melanoma, which included 28 849 patients with melanoma and 78 922 control participants from 20 studies from the UK, the USA, Australia and Europe. Then, it was tuned in the Canadian Longitudinal S

[Therapeutic options for occupational skin cancer (BK 5102, BK 5103)].

BACKGROUND: Incidence of keratinocyte skin cancer has been continuously rising in recent decades. At the same time, the number of occupational skin cancers has also increased. Treatment depends on clinical manifestation, extent of actinic keratosis, field cancerization and, in case of squamous cell carcinoma, on the tumor stage. OBJECTIVES: This work aims to enhance the understanding of the challenges associated with the treatment of occupational skin cancer. MATERIALS AND METHODS: This article is based on a review of the literature. RESULTS: Different therapeutic options for the treatment

Laser-assisted vs. conventional daylight photodynamic therapies for actinic keratosis: a two-year randomized evaluator-blinded half-side comparative trial.

BACKGROUND: Actinic field cancerization is considered a chronic and relapsing condition. Thus, long-term clearance and prevention of cutaneous squamous cell carcinoma should be the fundamental goal of treatment. OBJECTIVES: This trial aims to compare long-term outcomes of two novel field treatments for actinic keratoses. METHODS: The investigator-initiated intraindividual randomised controlled and evaluator-blinded trial compared fractional laser-assisted artificial- or natural daylight photodynamic therapy to daylight photodynamic therapy alone, as a split-head comparison of 60 patients wit

Actinic keratosis staging in multimodal image data.

BACKGROUND AND OBJECTIVE: Actinic Keratosis (AK) is a common skin condition, usually appearing on sun-exposed areas, whose progression is associated with characteristic dermatoscopic and structural changes. Early detection of AK is crucial, as cancer progression may occur in changed skin. This study aimed to develop a multimodal, machine-learning-based framework combining dermatoscopic and high-frequency ultrasound (HFUS) data to automatically stage AK and identify early lesions. METHODS: A dataset containing 222 pairs of dermatoscopic and HFUS images was clinically evaluated using the 3-poin

External Auditory Canal Eccrine Hidrocystoma and Equivocal Actinic Keratosis: A Rare Combination and Review of the Literature.

Eccrine hidrocystoma is a rare benign cyst arising from the sweat glands, whereas actinic keratosis is a premalignant cutaneous lesion. Scheuermann's kyphosis is a structural spinal disorder typically identified during adolescence. The coexistence of these conditions has not previously been reported. A patient in their early 70s presented with a blue, dome-shaped cyst located in the deeper portion of the external auditory canal. Surgical excision was performed, and histopathological examination revealed an eccrine hidrocystoma with equivocal features of actinic keratosis. The patient also had

Pathway-level insights into the epigenetic ageing of human skin.

Skin ageing emerges from the interplay of intrinsic biological decline, environmental exposures and disease-related remodelling, yet the pathway-level epigenetic programs that integrate these processes remain largely unresolved. Here, we develop a skin-specific PathwayAge clock that aggregates DNA methylation into biologically interpretable gene ontology pathways, enabling both precise quantification of epigenetic age and mechanistic dissection of cutaneous ageing. The model demonstrates high accuracy and strong generalisability across multiple independent cohorts, providing a robust foundatio

A Prebiotic and Panthenol-Containing Repair Balm Improves Tolerability of Topical 5-Fluorouracil Field Therapy for Actinic Keratoses: A Randomised Controlled Trial.

INTRODUCTION: Topical 5-fluorouracil (5-FU) is an effective field therapy for actinic keratoses (AKs) with proven chemopreventive benefit; however, treatment-limiting local skin reactions frequently impair tolerability and adherence in clinical practice. Our objective is to determine whether adjunctive use of a prebiotic and panthenol-containing repair balm (BB5+ repair balm) improves tolerability and treatment adherence to topical 5-FU field therapy for AKs compared with standard of care alone. METHODS: We conducted a single-blind, randomised controlled trial involving adults prescribed topi

Immunohistological Characterization of Actinic Keratoses with Varying Degrees of Proliferation.

Background: Actinic keratoses (AKs) are considered early in situ forms of cutaneous squamous cell carcinoma (cSCC). However reliable histopathological or molecular markers for predicting the risk of progression are still lacking. The aim of this study was to investigate the relationship between immunohistochemical markers and basal proliferation patterns of AKs in order to identify histopathological associations that may be relevant for malignant transformation. Methods: A total of 97 AK samples from facial and scalp areas were retrospectively analyzed and classified according to their basal p

Diagnostic Accuracy of Ex Vivo Confocal Laser Scanning Microscopy for Routine Detection of Cutaneous Squamous Cell Carcinoma and Actinic Keratoses.

Background and Objectives: Invasive cutaneous squamous cell carcinoma (cSCC) and actinic keratosis (AK) are growing burdens on ageing societies and mainly arise from chronic sun exposure. Distinguishing between carcinoma and carcinoma in situ is often clinically challenging but essential for decision-making with respect to targeted therapy. Ex vivo confocal laser scanning microscopy (CLSM) allows histologic examination of native tissue using tissue reflection and nuclear fluorescence staining. The digital staining process almost perfectly mimics conventional haematoxylin and eosin (HE) stainin

Nestin as a Vascular Marker of Angiogenesis in Non-Melanoma Skin Cancer.

BACKGROUND: Angiogenesis is critical for tumor progression. Microvessel density (MVD) is commonly assessed using CD31 and CD34, which detect both mature and newly formed vessels and therefore cannot distinguish active neoangiogenesis from stable, quiescent vasculature. Nestin, an intermediate filament protein expressed preferentially in proliferating endothelial cells, has been proposed as a complementary marker of active angiogenesis and has been investigated in several solid tumor types, including pancreatic, colorectal, and breast carcinomas. However, no studies have quantitatively compared

Photodynamic Therapy in Dermatology.

Photodynamic therapy (PDT) is a minimally invasive treatment choice whose clinical success in dermatology relies on the interaction between a photosensitizer, light of an appropriate wavelength, and tissue oxygen, leading to reactive oxygen species generation and selective cytotoxicity. This narrative review summarizes contemporary mechanisms and clinical evidence supporting PDT across neoplastic, inflammatory, infectious, and esthetic indications. A comprehensive literature search included randomized trials when available, systematic reviews, meta-analyses, and guideline and consensus documen

Superior Induction of Apoptosis by Blue Light Compared to Red Light Photodynamic Therapy in Cutaneous Squamous Cell Carcinoma Keratinocytes.

This study compared apoptosis induction in squamous cell carcinoma keratinocytes following exposure to 5-aminolevulinic acid combined with either blue light (417 ± 5 nm) or red light (635 ± 5 nm) photodynamic therapy, with apoptotic activity quantified via levels of Annexin V expression. Our results demonstrated that blue light induces superior levels of apoptosis in squamous cell carcinoma keratinocytes compared to red light at the parameters tested. Both wavelengths demonstrate additional dose- and temperature-dependent effects across two squamous cell carcinoma cell lines. These conclusions

Skin Cancer: Management of Actinic Keratoses and Keratinocyte Carcinomas.

Actinic keratoses are precancerous skin lesions that are treated to prevent progression to cutaneous squamous cell carcinoma. Keratinocyte carcinomas include basal cell carcinoma and cutaneous squamous cell carcinoma. Cryosurgery is preferred to treat a single or limited number of actinic keratosis lesions, whereas field-directed therapies, such as topical fluorouracil, topical imiquimod, photodynamic therapy, topical tirbanibulin, and off-label topical fluorouracil plus calcipotriene, are preferred treatments for multiple lesions or significant field cancerization. For basal cell carcinoma, l

A U-Net model for epidermal segmentation in optical coherence tomography images of actinic keratosis.

Actinic keratosis (AK) is a pre-cancerous skin lesion typically caused by excessive exposure to ultraviolet light. Optical coherence tomography (OCT) can provide sub-surface information relevant for lesion classification, but manual analysis of images is time-consuming and subject to inter-observer variability; automated segmentation based on deep learning models can provide faster and more consistent results. However, structural changes, such as thickening and hyperkeratosis, complicate epidermal segmentation tasks. For this reason, we aimed to develop and optimize a U-Net model for the autom

Sequential 5-fluorouracil-photodynamic therapy: A mechanism-based approach to actinic keratosis.

BACKGROUND: Actinic keratosis (AK) is a common premalignant condition arising from chronic ultraviolet exposure and frequently occurs within fields of sun-damaged skin that harbour subclinical dysplasia. OBJECTIVE: To summarize the mechanistic rationale, clinical evidence, safety profile, and practical considerations for a sequential regimen of short-course topical 5-fluorouracil (5-FU) pretreatment followed by daylight-mediated photodynamic therapy (dPDT) for field-directed management of AK. METHODS: We performed a narrative synthesis of randomized controlled trials, split-site studies, mec
