Current Perspectives on Photodynamic Therapy for Skin Diseases: A Narrative Review.

Photodynamic therapy (PDT) is a minimally invasive treatment widely used in dermatology for various skin diseases. It works through the interaction of a photosensitizer, light, and oxygen, producing reactive oxygen species that selectively destroy abnormal cells. PDT is well established in treating conditions such as actinic keratosis, Bowen's disease, and superficial basal cell carcinoma, with good cosmetic outcomes. Increasing evidence also supports its use in non-cancerous conditions, including acne, photoaging, and certain infections. Despite its advantages, limitations such as treatment-r

Raman spectroscopy aided evaluation of basal cell carcinoma diagnosis: A case report.

Basal cell carcinoma (BCC) is the most common non-melanoma skin cancer, and primarily diagnosed by invasive and time-consuming histopathological biopsy, highlighting the need for rapid, non-invasive adjunctive tools. We report the case of a 93-year-old patient presenting with a suspicious scalp lesion who underwent dermoscopy, Wood's lamp examination, and in vivo Raman spectroscopy prior to biopsy. Conventional clinical examinations suggested a malignant lesion but were unable to determine the specific tumor type, whereas Raman spectra displayed high similarity to former BCC cases and clear se

A Prediction Model for the Differential Diagnosis of Bowen's Disease and Bowenoid Actinic Keratosis.

Bowen's disease and bowenoid actinic keratosis share overlapping clinical and histopathological features, and there is no clear consensus regarding their differential diagnosis. This study aimed to identify clinicopathological differences between the two diseases and to develop a diagnostic prediction model in a Korean population. We retrospectively analyzed 253 patients pathologically diagnosed with Bowen's disease or bowenoid actinic keratosis at a single institution in South Korea. Clinical and histopathological variables were compared between the two groups, and significant variables were

Iontophoresis-Based Topical Drug Delivery for Dermatologic Conditions: A Systematic Review.

Background/Objectives: The efficacy of topical therapies in dermatology is often limited by the barrier function of the stratum corneum, which restricts drug penetration. Iontophoresis is a non-invasive transdermal delivery technique that uses a low-intensity electrical current to enhance the transport of charged and polar molecules across the skin. It has emerged as a strategy to improve local drug bioavailability while minimizing systemic exposure. We systematically reviewed the clinical evidence on the efficacy, safety, and pharmacologic performance of iontophoresis-assisted topical drug de

Treatment of vitiligo-like actinic keratosis with photodynamic therapy: A case report.

OBJECTIVE: Hypopigmented actinic keratosis is a rare variant that clinically mimics vitiligo, posing significant diagnostic challenges. METHODS: We report a case of a 66-year-old Chinese woman with a long-standing hypopigmented patch on her cheek, complicated by central erythema and scaling. The diagnosis was confirmed by histopathology, and the patient underwent treatment with 5-aminolevulinic acid photodynamic therapy (ALA-PDT). RESULTS: The lesion resolved completely after three sessions of ALA-PDT, with remarkable repigmentation observed in the previously depigmented area. CONCLUSIONS:

Dermatoscopic Features of Hyperpigmented Dermatoses.

In recent years, dermatoscopy has emerged as a valuable adjunctive tool for the diagnosis of various cutaneous disorders. Initially developed for the evaluation of melanocytic lesions, this non-invasive technique has since demonstrated significant utility in the assessment of non-melanocytic hyperpigmented dermatoses. This review provides an up-to-date and practical overview of the dermatoscopic manifestations of common non-melanocytic hyperpigmented conditions, including melasma, nevus of Ota, freckles, lentigines, solar lentigines, seborrheic keratosis, porokeratosis, acquired dermal macular

Editor's Highlights-August 2026.

Noninvasive imaging techniques are playing an increasingly significant role in dermatologic diagnosis, risk evaluation, and the monitoring of therapeutic outcomes. In the August issue of the Journal, we highlight how dermoscopy, onychoscopy, trichoscopy, and line-field confocal optical coherence tomography (LC-OCT) can refine clinical decision-making across acral melanocytic lesions, onycholysis, primary cicatricial alopecia, and hyperkeratotic actinic keratoses. These papers show that imaging is most useful when tailored to anatomical and clinical context, advancing standardized and clinicall

Polygenic prediction of actinic keratosis identifies transplant recipients at risk of extreme lesion burden.

Transplant recipients have an elevated risk of actinic keratosis (AK), a precursor of squamous cell carcinoma. We assessed whether an AK polygenic risk score (PRS) can identify transplant-naive individuals and transplant recipients at risk of multiple AKs who may benefit from early prevention. Using genome-wide association data (140 339 cases; 1 430 776 controls), we developed an AK PRS and evaluated it in cohort studies of European descent individuals in the QSkin Sun and Health Study (QSkin) (N = 12 843), Skin Tumors in Allograft Recipients Study (STAR) (N = 357), UK Biobank (N = 963), and A

Predictors of outcomes and satisfaction with tirbanibulin in actinic keratosis: a multicenter real-world study.

BACKGROUND: Tirbanibulin 1% ointment is a short-course field therapy for actinic keratosis (AK). Real-world data on predictors of response, tolerability, and satisfaction may support patient selection and counselling. The aim was to evaluate effectiveness and tolerability in a large multicenter cohort, identify predictors of high clinical clearance and local skin reactions (LSRs), and describe patient and physician satisfaction. METHODS: This retrospective observational real-life study (33 Italian dermatology centers) included 679 patients. Clinical response was classified as complete (100%),

The Role of General Practitioners in Actinic Keratosis Management in Europe: A Dermato-Oncologists Initiative.

Background/Objectives: Actinic keratosis (AK) is the key precursor of invasive cutaneous squamous cell carcinoma (cSCC). Rising incidence, ageing populations, and limited dermatologists' availability, particularly in rural and underserved regions, underscore the need for more inclusive care pathways. As in other common diseases such as diabetes or hypertension, general practitioners (GPs) could play a pivotal role in improving access, early recognition, and management of AK, provided that systemic barriers are addressed. Methods: This article synthesises current evidence and expert insights fr

Clinical Clearance of Actinic Keratoses with Tirbanibulin 1% Ointment: Evidence from a Meta-Analysis.

Actinic keratoses (AKs) are skin conditions that arise from the proliferation of atypical keratinocytes and are considered precursors to cutaneous squamous cell carcinoma. There are several treatment options available, which can be categorized into two main groups: lesion-directed therapies and field-directed therapies. Tirbanibulin 1% ointment has emerged as a new treatment for AKs. We conducted a meta-analysis to evaluate the effectiveness of tirbanibulin by analyzing the rates of complete clearance (CC) and partial clearance (PC) in randomized controlled trials (RCTs) and real-world studies

Efficacy, tolerability, and patient preference of indoor, simulated, or artificial daylight photodynamic therapy versus conventional lamp-based photodynamic therapy for actinic keratosis: A systematic review and meta-analysis.

BACKGROUND: Actinic keratosis (AK) is a common ultraviolet-induced keratinocyte lesion. Conventional photodynamic therapy (C-PDT) is effective as a field-directed treatment, but pain and treatment burden may limit acceptability. Indoor, simulated, or artificial daylight PDT (Indoor/SDL-PDT) may offer a more tolerable and controllable alternative. This systematic review and meta-analysis compared the efficacy, tolerability, and patient preference of Indoor/SDL-PDT versus C-PDT for AK. METHODS: Four major databases were searched from their establishment to 30 June 2026. Eligible studies enrolle

4% 5-Fluorouracil Cream for Actinic Keratoses: First Report of Efficacy and Tolerability in a Person Living With Human Immunodeficiency Virus Infection.

Actinic keratosis (AK) is a common premalignant skin condition associated with chronic ultraviolet exposure and an increased risk of progression to cutaneous squamous cell carcinoma (cSCC). Its prevalence rises with age and is influenced by multiple factors, including fair skin phototype and immunosuppression. People living with human immunodeficiency virus (HIV) are at increased risk of cSCC due to impaired immune surveillance, highlighting the importance of early diagnosis and effective management of AK in this population. However, data on optimal treatment strategies in individuals with HIV

Real-time LC-OCT monitoring of scalp hypertrophic actinic keratosis clearance following cryotherapy, laser, and 5-fluorouracil therapy.

INTRODUCTION: Assessment of treatment response in hypertrophic actinic keratoses (hAKs) remains largely dependent on clinical examination and dermoscopy, although apparent clearance may not reflect complete resolution of imaging features associated with residual disease. Line-field confocal optical coherence tomography (LC-OCT) enables non-invasive, longitudinal visualization of epidermal, dermal-epidermal junction, and superficial dermal changes at near-cellular resolution. OBJECTIVE: To characterize LC-OCT response patterns in scalp hAKs treated with ablative CO2 laser, cryotherapy, or topi

Photodynamic Diagnosis of Actinic Keratosis: A Systematic Review.

Actinic keratosis (AK) is a precancerous condition typically diagnosed by hyperkeratotic papules. The concept of field cancerization suggests subclinical lesions surround visible actinic keratoses. Photodynamic diagnosis (PDD)is a non-invasive imaging modality that can im-prove actinic keratosis diagnosis and treatment assessment. This systematic review describes current evidence regarding photodynamic diagnosis modalities for actinic keratosis. We searched MEDLINE and Embase through 28 February, 2025. Data synthesis was descriptive due to methodological heterogeneity across studies. Thirty-se
