ON101 counteracts oxidative stress and inflammation in radiation-induced dermatitis through Keap1/Nrf2 signaling.

Radiation-induced dermatitis (RID) remains a significant and challenging side effect of radiotherapy, with few effective topical interventions. This study investigates ON101 as a therapeutic candidate for RID, focusing on its modulation of the Keap1/Nrf2 signaling pathway. Using a murine model of radiation-induced skin injury, topical application of ON101 substantially reduced skin damage, as evidenced by decreased wound severity scores, preserved body weight, and improved histopathological outcomes. Histological analyses using H&E and Masson's trichrome staining revealed that ON101 and its ac

Bao Yuan Gao vs. Calendula cream for radiotherapy-induced skin toxicity in head and neck cancer: A randomized controlled trial.

BACKGROUND AND PURPOSE: Radiation Dermatitis (RD) is a frequent side effect in head and neck cancer (HNC) patients undergoing radiotherapy, negatively affecting their quality of life. Topical treatments are used for RD management, but many lack robust evidence of efficacy. METHODS: This randomized controlled trial compared the effectiveness of Bao Yuan Gao, a traditional Chinese herbal ointment, with Calendula cream, a cortisone-free topical agent, in preventing severe acute RD in HNC patients. Fifty-five patients receiving radiotherapy (60-70 Gy) with or without concurrent chemotherapy were

Skin Microbiome and Radiation-Induced Skin Injury: Unraveling the Relationship, Mechanisms, and Therapeutic Implications.

Radiotherapy (RT) is a treatment method commonly used in oncology. A vast majority of patients undergoing RT suffer from radiation-induced skin injury (RISI), which results from complex biochemical reactions in the irradiated skin. Current strategies for preventing and managing RISI are insufficient for achieving full skin regeneration. Multiple studies have shown that alterations in the skin microbiome correlate with the development and severity of RISI. These studies suggest that dysbiosis is a crucial factor in promoting radiation-associated dermatitis. Targeting the skin microbiota present

Chronic radiation dermatitis in breast cancer patients: pathophysiology, prevention and management strategies, and clinical impact.

Radiation therapy (RT) is a common component of breast cancer treatment. Chronic radiation dermatitis (CRD), defined as occurring after or extending beyond 90 days following the completion of RT, can be progressive and irreversible and manifest as changes in skin pigmentation, fibrosis, telangiectasia, ulceration, necrosis, and contribute to the development of cutaneous malignancy. There is limited existing research on the incidence, management approaches, and prevention strategies for CRD. The literature that does exist reports widely varying rates of CRD, reports the various manifestations i

Integrating genetic polymorphisms and clinical data to develop predictive models for skin toxicity in breast cancer radiation therapy.

BACKGROUND: We aim to develop and validate predictive models for acute and late skin toxicity in breast cancer (BC) patients undergoing radiation therapy (RT). Models incorporate a genetic profile-comprising candidate single nucleotide polymorphisms (SNPs) in non-coding RNAs and previously reported toxicity-associated variants-combined with clinical variables. METHODS: The study involved 1979 BC patients monitored for two to eight years post-RT in a multi-centre study. We assessed acute (oedema/erythema) and late (atrophy/fibrosis) toxicity using logistic regression and Cox proportional hazar

Do Breast Cancer Patients With Chemotherapy-induced Peripheral Neuropathy Have an Increased Risk of Radiation Dermatitis?

BACKGROUND/AIM: Patients with breast cancer scheduled for irradiation may present with peripheral neuropathy (PNP) caused by preceding taxane-based chemotherapy. Chemotherapy-related toxicity was suggested to be associated with a lower individual DNA repair capacity. A similar association was described for radiation-related toxicity. We investigated whether patients with taxane-induced PNP have an increased risk of radiation dermatitis. PATIENTS AND METHODS: In a retrospective cohort of 129 patients irradiated for breast cancer following taxane-based chemotherapy, PNP plus 14 characteristics

Engineered Microalgal Extracellular Vesicles for Enhancing Mitochondrial Homeostasis in Radiodermatitis Prevention.

Radiodermatitis, one of the most prevalent side effects of cancer radiotherapy, is characterized by cellular oxidative stress, mitochondrial damage, and inflammatory responses. In this study, we isolated extracellular vesicles (EVs) from the natural microalgae Spirulina platensis (SP) and engineered them by loading astaxanthin (AST) into SP-EVs, resulting in the formation of SP-EVs@AST. This engineered system significantly enhanced the solubility and stability of AST while preserving the structural integrity and biological activity of SP-EVs, thereby enabling the complementary and synergistic

K-Edge-Inspired Selenomelanin Hydrogel: An X-Ray Filtering Biomaterial for Skin Protection in Radiotherapy.

Radiotherapy is widely employed in cancer treatment, yet its inevitable side effects, particularly radiation dermatitis (RD), remain a significant concern. Numerous studies focus on alleviating RD symptoms, but approaches to minimize the radiation dose to the skin are underexplored. Shielding skin-damaging photons (10-20 keV) holds promise for skin dose reduction; however, this strategy is hindered by inadequate shielding efficiency of current RD biomaterials and overshielding by conventional high-Z materials which attenuate therapeutic high-energy photons. Here, inspired by the strong absorpt

Real-world Safety of Concurrent Pembrolizumab and Radiotherapy in Triple-negative Breast Cancer.

OBJECTIVES: Triple-negative breast cancer (TNBC) accounts for ∼15% of invasive breast cancers and is associated with a poor prognosis. The introduction of pembrolizumab in both neoadjuvant and adjuvant settings, as established by the KEYNOTE-522 trial, has improved event-free survival and is now considered standard of care. Postoperative adjuvant radiotherapy remains essential in reducing recurrence and mortality. However, combining radiotherapy with pembrolizumab may increase the risk of toxicities, particularly cardiac, and its long-term safety profile remains poorly characterized. This stud

Development of a smartphone-based multi-spectral imaging system and its performance validation through radiodermatitis assessment.

Cancerous skin lesions (i.e., skin cancers) are the most common types of cancer. Currently, there exists an unmet clinical need to develop advanced imaging tools for skin diagnosis and associated treatment monitoring. In this work, we reported the development of a smartphone-based multi-spectral imaging (MSI) system to fulfill the need and validated its performance through radiodermatitis assessment: one of the severe side effects of skin cancer radiotherapy (RT) treatment that needs to be objectively graded to avoid the possible RT treatment cessation thereof. In combination with a graphics p

Advances in stem cell-derived exosome therapy for radiation-induced skin injury.

Radiation-induced skin injury (RISI) represents a significant clinical challenge, affecting up to 95% of radiotherapy patients and often resulting in both acute and chronic complications that can severely impact quality of life. While conventional treatments show limited efficacy, stem cell-derived exosomes have emerged as a promising cell-free therapeutic approach. These nanoscale vesicles, which harbor bioactive chemicals derived from their progenitor stem cells, exhibit significant potential in facilitating tissue healing while mitigating the risks associated with cell-based therapies. This

Risk factors for early-onset radiodermatitis in patients with locally advanced breast cancer: a comparative analysis of volumetric modulated arc therapy and intensity-modulated radiotherapy.

BACKGROUND: Radiodermatitis is the predominant acute toxicity in locally advanced breast cancer (BC) radiotherapy. Early-onset radiodermatitis substantially increases the risk of high-grade injury and potential treatment interruption. This study aimed to identify risk factors for early-onset radiodermatitis by analyzing patient characteristics and dose-distribution profiles from step-and-shoot intensity-modulated radiotherapy (IMRT) and volumetric-modulated arc therapy (VMAT). METHODS: This retrospective analysis included 128 women with locally advanced unilateral BC who received postoperativ

Nanofat promotes wound healing in skin following exposure to ionizing radiation.

Radiotherapy, while effective in cancer treatment, can lead to side effects, such as radiodermatitis with potential long-term consequences including telangiectasias, ulceration and fibrosis of the skin, eventually resulting in impaired wound healing. In this study, we analyzed whether the healing of such challenging wounds can be improved by nanofat (NF). NF is generated by mechanical emulsification and filtration of fat samples and, thus, is a random mixture of adipose-derived stem cells, microvascular fragments, extracellular matrix components and growth factors. Two months after localized i

Radiation Recall Dermatitis Following Capivasertib Administration.

Radiation recall dermatitis is a known but rare adverse effect that is characterized by the development of dermatitis in the region of prior irradiated tissue triggered by exposure to a systemic agent. Capivasertib is a small-molecule inhibitor targeting the phosphatidylinositol 3-kinase/protein kinase B pathway recently approved in locally advanced and metastatic breast cancer; however, the safety of its use in the setting of palliative radiation is currently unclear. Here, we report a case of radiation recall dermatitis in a patient with metastatic breast cancer on capivasertib with history
