Breast cancer, the most common cancer worldwide, is treated with surgery, chemotherapy, and radiotherapy. Radiotherapy reduces recurrence and mortality but causes radiodermatitis in over 90% of patients. Current preventive and topical treatments have limited efficacy. Fluoroscopy-induced radiodermatitis, whether acute or chronic, is a complication of fluoroscopy-guided cardiac diagnostic and interventional procedures. Diagnosis of fluoroscopy-induced radiodermatitis remains challenging for dermatologists. We present a case report where a patient had predictable risks of developing chronic radiodermatitis, yet the diagnostic was delayed for over a year and a half. In the current absence of appropriate follow-ups in some institutions, dermatologists should keep high suspicion for chronic fluoroscopy-induced radiodermatitis when encountering lesions characteristic of radiation dermatitis without a history of radiotherapy, prompting the necessity to inquire about antecedents of fluoroscopy-guided interventions.

Atypical vascular proliferations (AVP) are a late complication after radiotherapy. Most cases have been reported in female breast cancer patients on the chest wall. These lesions are mostly of the lymphatic type. Herein, we report a blood vascular-type AVP in a male on the neck 60 years after radiotherapy for a benign hemangioma, which makes this case exceptional. We removed the whole chronic radiodermatitis surgically. Histopathology excluded vascular malignancies but confirmed AVP. We discuss the differential diagnoses and treatment.

Chronic radiodermatitis is a delayed response of skin and underlying soft tissues after exposure to the ionizing radiation. It bears a risk of secondary tumors, in particular non-melanoma skin cancer (NMSC). We present 3 case reports of the patients with the development of BCC's ≥40 years after radiation of either childhood hemangioma or basal cell carcinoma. Patients with chronic radiodermatitis need a life-long dermatologic follow-up for early detection of NMSC and its consequent removal by Mohs surgery.
