Comparison of phenol and sodium hydroxide chemical matricectomies for the treatment of ingrowing toenails.

OBJECTIVE: Chemical matricectomy is performed mainly by two agents: phenol and sodium hydroxide. Both agents have excellent cure rates, but there are no data about the comparison of postoperative healing periods. This study was designed to compare the postoperative morbidity rates of sodium hydroxide and phenol matricectomies. MATERIALS AND METHODS: Forty-six patients with 154 ingrowing nail sides were treated with either sodium hydroxide or phenol matricectomy. In the postoperative period, the patients were evaluated for the duration and severity of pain, drainage, and peripheral tissue dest

A new anatomical repair method for the treatment of ingrown nail: prospective comparison of wedge resection of the matrix and partial matricectomy followed by lateral fold advancement flap.

Currently, there are various surgical treatment modalities for ingrowing nail. None of these procedures are perfect to achieve esthetic results with low cost, recurrence, and complication rates. Eighty-seven toenails of 77 patients were operated in our clinic; 49 wedge matrix resections (WMR) and 38 partial matricectomy followed by lateral fold advancement flap (LFAF) were applied. Average follow-up period of the patients was 13 months. The recurrence rates, spicule formation, immobilization periods, and patient satisfaction for cosmetic result and discomforting symptoms were investigated. Nai

Formable acrylic treatment for ingrowing nail with gutter splint and sculptured nail.

BACKGROUND: The treatment of choice for an ingrowing nail has been surgical rather than nonsurgical. Yet, surgical treatments are far from successful, cause pain and patient apprehension, and leave disfigurement. Further, there is misunderstanding about the disease pathophysiology. OBJECTIVE: To demonstrate the benefits of a noninvasive method of treatment for an ingrowing nail using gutter splint and formable acrylics and to present a current understanding of the disease pathophysiology. METHODS: From a total of 541 cases of ingrowing nails treated, full follow-up data were obtained between

Analysis of the factors influencing the therapeutic effects of onychomycosis.

In order to improve the curative effect of onychomycosis, the factors influencing the therapeutic effects were investigated. 545 cases including 245 males and 300 females, who were diagnosed both clinically and mycologically, were treated by Intraconazole with intermittent pulse therapy. The therapeutic effects were judged by the following observations regularly and analyzed from the factors as follows: age; growing speed of nails; accompanied diseases; family history; trauma of nails; infection ways of the pathogens; manifestation of the injury; pathogens; duration of the treatment. The resul

The Caribbean experience with the diabetic foot management of the diabetic foot.

The foot complications of diabetic patients are one of the commonest and most devastating of medical problems that occurs in the Caribbean. The scale of the problem is reflected in the fact that, on average, 75% of the beds in the general surgical wards of the Queen Elizabeth Hospital in Barbados are occupied by patients with this problem. Of the patients admitted, a third lose a limb by amputation and another third of the patients lose toes or part of their feet and remain in hospital an average of two months as doctors struggle to prevent them losing their limbs. Half of the patients are in

Nail surgery.

Nail surgery is not particularly difficult but often delicate. Profound knowledge of the anatomy, physiology and pathology of the nail organ as well as surgical skills and atraumatic instruments are prerequisites for successful nail operations. Careful preoperative preparation of the patient as well as consistent post-operative care are necessary. The following article will give information about the essentials of nail surgery, such as adequate anaesthesia, instrumentation, and complications. The most frequently performed nail operations are described focussing on techniques proven to be safe

Congenital hypertrophy of the lateral nail folds of the hallux: clinical features and follow-up of seven cases.

Congenital hypertrophy of the nail folds of the hallux describes an abnormality of the periungual soft tissues of the big toe characterized by hypertrophy of the nail fold which partially covers the nail plate and is frequently associated with inflammation and pain due to an ingrowing nail. We describe the clinical picture and follow-up of seven patients with this abnormality. In three patients the affected toe showed an asymptomatic, dome-shaped, hypertrophic lip that partially covered the nail plate. Four patients had acute inflammatory changes due to toenail ingrowth, with considerable swel

[Some aspects of work at the city laser surgery center of Ashgabad].

During 7-year work 2,500 operations with a laser were performed for 52 diseases at the center. The authors studied the medico-economical data on the use of the laser techniques in the treatment of persisting wounds and trophic ulcers, cutaneous leishmaniasis, chronic anal fissure, purulent diseases of the soft tissues, epithelial coccygeal passage, ingrowing nail, and localized hyperkeratosis. A comparison group consisted of patients who received traditional treatment. With the use of the laser methods the clinical results of treatment were appreciably improved--the number of poor results redu

The surgical treatment of ingrowing toenails.

Two prospective studies of ingrowing toenail management were conducted. In the first, 163 patients (204 ingrowing nail edges) who had not had previous surgery were randomised and treated by total nail avulsion, nail edge excision, or nail edge excision with phenolisation of the germinal matrix; recurrence rates one year postoperatively were 73%, 73% and 9% respectively. In the second study, 63 ingrowing nail edges which had recurred after previous operations underwent nail edge excision and phenolisation. There was a 5% recurrence rate and 5% incidence of dystrophy of the nail one year after o

Recurrent blistering distal dactylitis of the great toe associated with an ingrowing toenail.

We report a case of recurrent blistering distal dactylitis in a child. Although the skin returned to normal following antibiotic therapy, blistering later recurred. This recurrence was probably related to an associated ingrowing nail affecting the same digit.

Conservative outpatient management of ingrowing toenails.

Twenty-five patients with ingrowing toenails were treated conservatively by inserting cotton wool under the ingrowing nail edge. Seventy-nine percent were relieved of their symptoms after follow up for a mean of 23.7 weeks. Thirty-six percent had a history of previous surgery to the nail, 75% of whom had a good or excellent result. Although conservative management was first described in the eighteenth century and has been reported sporadically since, only a few doctors treat their patients in this way. There is, however, a distinct place for this highly effective, low cost method as the initia

Partial matricectomy with electrodesiccation for permanent repair of ingrown nail borders.

A large number of chemical and surgical procedures have been described for the management and correction of onychocryptosis (1-15). Many of these procedures share the problem of prolonged postoperative drainage, which may continue from 4 to 6 weeks. This extended time period can cause the patient inconvenience and discomfort, and continuous follow-up care by the physician is necessary. Electrodesiccation combined with surgical excision of the nail matrix to correct an ingrowing nail border permanently is simple, fast, has a low incidence of infection, and will usually have a dry bed of granula
