Treatment of Isolated Nail Psoriasis with JAK Inhibitor Tofacitinib: A Case Report.

BACKGROUND: Isolated nail psoriasis is clinically rare, characterized by nail involvement in the absence of cutaneous lesions, and is often misdiagnosed as onychomycosis. Although tofacitinib, a JAK1/JAK3 inhibitor, is approved for psoriatic arthritis, evidence regarding its efficacy and safety in isolated nail psoriasis remains limited. CASE PRESENTATION: We present a 39-year-old female with a 6-year history of refractory isolated nail psoriasis, previously unresponsive to itraconazole, acitretin, topical antifungals, and corticosteroids. The patient received oral tofacitinib 5 mg twice dail

Onychoscopy in Nail Psoriasis: A Narrative Review of Diagnostic Utility, Disease Severity Assessment, Musculoskeletal Associations, and Treatment Monitoring.

Nail psoriasis is a common and clinically significant manifestation of psoriasis that may be difficult to recognize, quantify, and differentiate from other nail disorders. Onychoscopy has emerged as a non-invasive imaging technique that enables detailed visualization of nail matrix, nail-bed, and vascular abnormalities. This narrative review critically synthesized the current evidence on the diagnostic and clinical applications of onychoscopy in nail psoriasis. Literature search of PubMed/MEDLINE, Scopus, and Google Scholar was conducted, and the available evidence indicates that pitting, onyc

Multidisciplinary Management of Severe Taxane-Induced Nail Toxicity in a Breast Cancer Patient.

INTRODUCTION: Taxane-induced nail toxicity (TINT) is a relatively common but often underreported adverse effect of docetaxel-based chemotherapy that may significantly impair quality of life and functional status. Severe cases can require structured supportive care involving multiple clinical disciplines. We present a case of high-grade TINT in a patient with breast cancer successfully managed with a multidisciplinary supportive approach. CASE PRESENTATION: A 70-year-old woman with HER2-positive breast cancer received neoadjuvant TCH chemotherapy (docetaxel, cyclophosphamide, trastuzumab), fol

Case Report: Near-complete resolution of nail psoriasis with upadacitinib in a pediatric patient.

Nail psoriasis (NP) is refractory in children, causing punctate pits, onycholysis, subungual hyperkeratosis and discoloration, impairing quality of life. We report a 14-year-old boy with 4-year NP, all fingernails involved with no cutaneous lesion, and unresponsive to traditional therapy. After 20-week oral upadacitinib, all nail lesions achieved near-complete resolution with only minimal residual nail changes, and no treatment-related adverse events. This suggests that upadacitinib demonstrates favorable efficacy and safety for pediatric refractory NP, large-scale prospective clinical investi

Subungual Myiasis Presenting to the Emergency Department: A Case Report.

INTRODUCTION: Subungual myiasis is an infection under the fingernail or toenail caused by an infestation of fly larvae. It is rarely reported internationally, with only one previously reported case in the United States. CASE REPORT: An 80-year-old female with a history of polyneuropathy, peripheral artery disease, peripheral venous insufficiency, and chronic bilateral lower extremity edema presented to the emergency department (ED) after staff at her skilled nursing facility noted what appeared to be several whitish-colored maggots moving under the nail of the patient's left great toe. Acrylate Allergic Contact Dermatitis Among Manicure Consumers and Professionals.

Acrylate allergic contact dermatitis is a growing concern, particularly among women exposed to nail cosmetics as consumers or professionals. Not all nail products carry equal risk. Repeated exposure to acrylates or improper curing can lead to symptoms resembling other forms of allergic contact dermatitis. Onycholysis is one common symptom that can be painful and slow to heal. Treatment of acrylate allergic contact dermatitis centers on acrylate avoidance and selection of alternative nail beautification methods. Nurses are positioned to educate and advocate for nail professionals, who are often

Comparison of clinical and dermoscopical nail findings of rheumatoid arthritis, ankylosing spondylitis, psoriatic arthritis and psoriasis: A prospective study.

Psoriasis (PsO), psoriatic arthritis (PsA), rheumatoid arthritis (RA), and ankylosing spondylitis (AS) are chronic inflammatory disorders classified as immune-mediated inflammatory diseases (IMIDs). Although these diseases share common pathogenic mechanisms, they can affect different parts of the body. Nail involvement is important in PsO. However, nail findings in RA and AS have not been extensively investigated despite their inflammatory presentation, similar to PsO. This study aimed to investigate clinical and onychoscopic nail findings in patients with RA and AS and compare their distribut

Cutaneous adverse events with antibody-drug conjugates: a FAERS-based pharmacovigilance study.

INTRODUCTION: Antibody-drug conjugates (ADCs) have revolutionized oncology by integrating the target specificity of monoclonal antibodies with the potency of cytotoxic payloads. However, despite a subset of clinical trials and case reports documenting cutaneous adverse events (CAEs) associated with these agents, comprehensive systematic characterization of ADC-related CAEs remains elusive. METHODS: Data extracted from the FDA Adverse Event Reporting System (FAERS) spanning Q1 2004 to Q2 2025 included patient baseline characteristics. Disproportionality analyses-reporting odds ratio (ROR) and

Nail Bed Sporotrichosis in a Patient with Psoriasis Arthritis Treated with a TNF Blocker: A Case Report.

INTRODUCTION: Sporotrichosis is a granulomatous infection caused by Sporothrix species, typically following traumatic inoculation. Brazil is an endemic area, and zoonotic transmission from cats has become the predominant route of infection. Nail bed involvement is extremely uncommon, likely due to the protective barrier of the nail plate. CASE PRESENTATION: We report a 51-year-old female, who cared for domestic and stray cats, with cutaneous and arthropathic psoriasis under treatment with methotrexate and adalimumab, who developed a painful, ulcerated lesion on the nail bed and folds of the r

Onychomadesis: A Rare Manifestation of Coxsackievirus A6 Infection in an Adult Patient.

Hand, foot, and mouth disease (HFMD) is a self-limiting viral illness predominantly affecting children under 10 years of age, with peak incidence in those aged one to four years. Recent outbreaks of Coxsackievirus A6 (CVA6) have increased its incidence across all age groups, including adults. Onychomadesis, a proximal nail plate detachment due to transient matrix arrest, is a recognized but underreported delayed complication of HFMD. We report a case of a 60-year-old man who developed onychomadesis three weeks after clinical recovery from CVA6-induced HFMD. Initial symptoms included fever, sor

Effect of Microcurrent Stimulation on Cartilage Regeneration in Engineered Chondrocyte Spheroids.

Microcurrent stimulation has emerged as a promising biophysical cue for cartilage regeneration, but its dose-dependent effects in 3-dimensional engineered cartilage constructs remain unclear. Here, we investigated the effects of continuous direct microcurrent stimulation on auricular chondrocytes and engineered cartilage spheroids using 0-, 10-, 25-, 50-, 100-, and 200-μA stimulation for 30 min daily. In 2-dimensional culture, microcurrent stimulation did not markedly increase the DNA content or Cell Counting Kit-8 value, indicating that it did not markedly promote cell expansion or overall me

Onychoscopic Alterations in Newborns with Early Congenital Syphilis: A Single-Center Cross-Sectional Study of 10 Cases.

INTRODUCTION: Early congenital syphilis (ECS) is a vertically transmitted infection that remains difficult to diagnose in the neonatal period, as many newborns are asymptomatic at birth. Although mucocutaneous manifestations are well recognized, nail involvement has been rarely described. METHODS: We conducted a prospective, single-center study including neonates with ECS fulfilling scenario 1 or 2 criteria established by the Centers for Disease Control and Prevention and the American Academy of Pediatrics. Ten neonates admitted to the neonatal intensive care unit between April and July 2025

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Erector Spinae Plane versus Pectoralis Nerve Block for Mastectomy in Cancer Patients: A Retrospective, Multicenter Cohort Study.

BACKGROUND: Mastectomy patients require effective acute postoperative pain management. Regional anesthesia techniques such as fascial plane blocks are important pain treatment modalities to address pain control. This study compared the effects of the erector spinae plane (ESP) block versus the pectoralis nerve (PECS) block in providing postoperative analgesia for mastectomy patients. METHODS: This retrospective study included 517 breast cancer patients undergoing mastectomy from 2017 to 2020, with 428 receiving a PECS block and 89 an ESP block. Nail toxicity associated with anticancer agents.

In recent decades, cancer mortality rates have declined due to development of systemic antineoplastic agents targeting specific cancer pathways. Nail toxicities associated with these therapies are underrecognized by many and contribute significantly to patient morbidity. Nail changes from chemotherapy occur due to toxicity to continuously dividing nail matrix keratinocytes. Preventive measures, including avoiding repeated trauma from manicuring and artificial nails, regular nail trimming, and applying emollients to cuticles are essential in mitigating nail toxicities. Frozen gloves or socks ha
