Successful treatment of nail psoriasis with topical roflumilast: A case report.

Nail psoriasis occurs in approximately half of all cases of plaque psoriasis and manifests with onychodystrophy, which includes morphological features of onycholysis, subungual hyperkeratosis, oil drop sign, pitting, splinter hemorrhages, leukonychia, and crumbling of the nails. Nail psoriasis can have a significant adverse impact on quality of life. However, nail psoriasis is often refractory to both local and systemic therapies, making it challenging to treat. Topical and oral phosphodiesterase-4 inhibitors have been successfully used to treat multiple different subtypes of psoriasis. Topica

[Onychopapilloma : Diagnosis and surgical treatment of a rare nail tumor].

We report on two cases of nail tumors associated with onychorrhexis, distal V‑shaped onycholysis, and subungual keratosis. Surgical exposure was performed in each case to confirm the diagnosis. Histopathological examination revealed skin changes consistent with onychopapilloma. We describe two common surgical techniques in the treatment of onychopapilloma: classical longitudinal excision and alternative tangential excision with corresponding follow-up.

Case Report: Alpha6 Integrin Disorder Presenting in Childhood with Nail Dysplasia and Onycholysis But No History of Fragile or Bullous Skin Changes.

We report a 7-year-old girl born with pyloric atresia but without congenital epidermolysis bullosa or skin fragility. Nail dysplasia developed at age 8 months and throughout childhood she suffered from onycholysis and mild nail hypertrophy. Whole-exome sequencing demonstrated biallelic mutations in alpha6 integrin (ITGA6): p. Q139* and R153W. ITGA6 normally forms a protein heterodimer with beta4 integrin (ITGB4), and this dimer participates in anchoring the basal skin cells to the extracellular matrix. Biallelic mutations in each gene are well known to cause epidermolysis bullosa and pyloric a

Celebrating Diversity: Unveiling the Characteristics of Nail Psoriasis and Nail Lichen Planus in 30 Patients With Skin of Color.

INTRODUCTION: Dermatological conditions affecting the nails can manifest differently in individuals with distinct skin tones. This often leads to difficulty in the recognition of nail diseases, especially in people with skin of color (SoC), who are not well represented in the literature. OBJECTIVES: Our aim was to provide dermatologists with useful clues for prompt recognition and diagnosis of nail psoriasis (NPso) and nail lichen planus (NLP) in people with SoC. METHODS: We described the ungual manifestations of NPso and NLP in a population of 30 patients with SoC. Diagnosis was primarily b

Acantholytic Dyskeratotic Acanthoma of the Nail Revealed by a Longitudinal Erythronychia.

INTRODUCTION: Focal acantholytic dyskeratosis is a distinctive histological pattern first described by Ackerman in 1972, consisting of focal suprabasal clefts in the epidermis and dyskeratotic cells at all levels of the epidermis with hyperkeratosis and parakeratosis. The first case of subungual acantholytic dyskeratosis acanthoma (ADA) was reported in 1990. This subungual variant is a very rare entity. CASE PRESENTATION: A 63-year-old female patient consulted for a longitudinal erythronychia (LE) associated with distal onycholysis. Dermoscopy showed LE with a few splinter hemorrhages, with l

Efficacy of Nonablative Bipolar Radiofrequency in the Treatment of Fingernail Psoriasis.

BACKGROUND: Psoriasis is a common chronic systemic disease affecting the skin, nails, and joints. Nails are commonly associated with a greater severity of the disease. Radiofrequency (RF) is a nonionizing radiation that provides energy originating from electric current to generate heat inside the dermis with anti-inflammatory effects. OBJECTIVE: To assess the efficacy of nonablative bipolar radiofrequency in treating fingernail psoriasis. METHODS: Forty-three affected fingernails were treated with nonablative bipolar RF. Sessions were performed every 2 weeks for 2 months, with a maximum of 5

A Case of Subungual Exostosis Mimicking Verruca Vulgaris.

Subungual exostosis is a benign bone tumor causing nail deformities and possibly pain for the patient. Etiology includes trauma, infection, and activation of a cartilaginous cyst, more commonly seen in adult patients. Here, we present a case of subungual exostosis in a pediatric patient that initially mimicked subungual verruca vulgaris. After failed cryotherapy, surgical management allowed for symptom resolution without recurrence. This case highlights the need to consider subungual exostosis in the differential when managing suspected verruca vulgaris associated with the nail unit, a common
