Aplasia Cutis Congenita in Bart's Syndrome: A Case Report and Literature Review.

Aplasia cutis congenita (ACC) is a partial or complete absence of skin layers which can be a part of a syndromic disease. Bart's syndrome is a combination of clinical manifestations including ACC, epidermolysis bullosa, blistering in the oral mucosa, nail dystrophy, or congenital absence of nails. Additional anomalies, such as pyloric atresia, a broad nasal root, hypertelorism, and ear abnormalities, may also be present. We aimed to raise awareness about concomitant anomalies with ACC. A 23-year-old female with gravid 3 para 2 gave birth to a male fetus of 36 weeks and 3 days who had multiple

Cronkhite-Canada Syndrome: A Diagnostic Challenge and Management With Vedolizumab.

BACKGROUND Cronkhite-Canada syndrome (CCS) is a rare, nonhereditary gastrointestinal polyposis syndrome characterized by diffuse polyps, diarrhea, malnutrition, and ectodermal changes. It can mimic inflammatory bowel disease (IBD), leading to misdiagnosis and delayed treatment. Here, we present a case of CCS that was initially thought to be Crohn's disease, managed with vedolizumab, and ultimately required total parenteral nutrition (TPN) due to severe malnutrition. CASE REPORT A 65-year-old man presented with a history of 2 months of watery diarrhea, weight loss, postprandial abdominal pain,

Mucocutaneous Manifestations of Primary Systemic Amyloidosis: A Descriptive Study from Eastern India.

INTRODUCTION: Primary systemic amyloidosis (PSA), also referred to as amyloid light-chain (AL) amyloidosis, is characterized by the deposition of insoluble monoclonal immunoglobulin light chains or L-chain fragments in a variety of tissues and organs of the body. There is a dearth of literature regarding the mucocutaneous manifestations of PSA in the Indian population, which prompted us to undertake the present study. AIM AND OBJECTIVE: We aimed to study the clinical characteristics of mucocutaneous manifestations in patients with PSA at a tertiary care center in Kolkata. PATIENTS AND METHOD

Effectiveness of Abrocitinib in Atopic Dermatitis Presenting with Hand-Foot Eczema and Nail Dystrophy: A Case Report.

Eczema-associated nail dystrophy is an uncommon but clinically relevant manifestation of atopic dermatitis (AD), which can lead to substantial structural and functional impairment and impose significant therapeutic challenges. However, evidence regarding systemic treatment for AD-related nail involvement remains limited. We report a case of AD presenting predominantly with refractory hand and foot eczema and marked nail dystrophy who achieved notable improvement following treatment with the selective Janus kinase 1 (JAK1) inhibitor abrocitinib, with cutaneous symptoms improving within several

Epidermolysis bullosa accompanied by long-segment gastric outlet obstruction in a newborn: a report of a rare case.

Epidermolysis bullosa (EB) is a rare congenital disorder caused by mutations affecting structural proteins of the epidermis and dermo-epidermal junction, resulting in extreme skin and mucosal fragility, blistering with minimal trauma, and impaired wound healing. Gastric outlet obstruction (GOO) in newborns arises from disruption of normal gastric emptying and may cause poor feeding, weight loss, severe vomiting, dehydration, and electrolyte imbalance. Long-segment gastroduodenal atresia is a rare subtype of GOO in which the blockage spans a continuous region from the distal stomach into the pr

Clinical and Onychoscopic Evaluation of Nail Changes in Patients with Erythroderma.

INTRODUCTION: Erythroderma is a severe inflammatory dermatosis involving over 90% of the body surface. Nail changes are common and reflect underlying dermatoses, systemic influences, and disease chronicity. However, their detailed characterization, particularly on onychoscopy, remains insufficiently studied. METHODS: This cross-sectional study was conducted at a tertiary referral hospital in India over 18 months. Consecutive clinically diagnosed, untreated erythroderma cases (≥4 weeks off therapy) were enrolled. Comprehensive clinical and onychoscopic evaluation of all 20 nails was performed,

Systemic Amyloid Light-Chain (AL) Amyloidosis Revealed by Progressive Pan-Ungual Dystrophy: A Case Report.

Systemic amyloid light-chain (AL) amyloidosis is a multisystem disorder caused by extracellular deposition of monoclonal immunoglobulin light chains. Cutaneous manifestations are diverse, but nail involvement is uncommon and may lead to diagnostic delay. We report the case of a 53-year-old man presenting with progressive dystrophy affecting all 20 nails for 18 months. Clinical examination revealed marked nail plate atrophy and onychorrhexis. Dermoscopy demonstrated nail plate atrophy, subungual hyperkeratosis, and splinter hemorrhages. A longitudinal lateral nail-bed biopsy revealed abundant a

Suspected parental gonadal/gonadosomatic mosaicism for a TINF2 mutation in two sisters with dyskeratosis congenita.

BACKGROUND: Dyskeratosis congenita (DC; OMIM: 127550) is a rare inherited bone marrow failure syndrome. TINF2 mutations are the second most common genetic cause of DC, and most cases arise from de novo mutations. Although the TINF2 p.Thr284Pro variant has been reported in isolated cases, its pathogenic role has not been functionally validated, and its potential association with suspected parental gonadal/gonadosomatic mosaicism has not been previously described. OBJECTIVE: To evaluate the functional impact of the TINF2 p.Thr284Pro variant and explore its association with suspected parental go

Case Report: Successful topical simvastatin therapy in a 2-year-old girl with keratin 16-associated palmoplantar epidermal differentiation disorder.

Palmoplantar epidermal differentiation disorder associated with pachyonychia congenita is a rare autosomal dominant genodermatosis characterized by painful palmoplantar hyperkeratosis and nail dystrophy. Treatment options remain limited, particularly in young children, in whom systemic therapies carry safety concerns. Herein, we report a 2-year-old girl who presented with palmoplantar hyperkeratosis since 1 month of age, with progressive nail dystrophy involving all 20 nails. Whole-exome sequencing identified a hemizygous multi-exon deletion in keratin 16 (ClinVar: SUB16259296; OMIM: #167200),

Risk factors for nail dystrophy following distal phalangeal fractures of the hand.

INTRODUCTION: Distal phalanx fractures are commonly associated with nail unit injuries and may cause long-term nail dystrophy. However, the risk factors associated with such deformities are poorly understood. In this study, we aimed to assess the potential risk factors for nail dystrophy after distal phalanx fractures. METHODS: We conducted a retrospective review of adult patients with distal phalanx fractures treated at a single hand surgery centre between January 2015 and December 2019. The patients were followed up clinically and radiographically for at least 1 year after injury. RESULTS:

AI-assisted diagnosis of nail unit melanoma and melanonychia using a clinical deep learning model.

BACKGROUND AND OBJECTIVES: Nail unit melanoma (NUM) is a rare but potentially fatal malignancy often misdiagnosed as melanonychia. Because biopsy may cause permanent nail dystrophy, accurate noninvasive diagnosis is essential. This study aimed to develop and validate an artificial intelligence model to distinguish NUM from benign melanonychia using clinical images and to assess its diagnostic utility through human comparison and external validation. PATIENTS AND METHODS: Clinical images from 172 patients with melanonychia and 122 patients with NUM were retrospectively collected. Pediatric mel

Epidemiological Features and Pathogenic Mechanisms of Steatocystoma Multiplex: A Systematic Review.

BACKGROUND: Steatocystoma multiplex (SM) is a rare hereditary skin disorder arising from the pilosebaceous unit, clinically characterized by multiple asymptomatic sebum-containing cysts predominantly distributed on the chest, axillae, and upper arms. However, the epidemiological features and pathogenic mechanisms of SM remain poorly characterized. METHODS: A systematic search of PubMed, Embase, and CNKI was conducted for studies published between January 2003 and December 2023. Case reports and observational studies reporting individual-level clinical data on confirmed SM patients were eligib

Atypical fingernail onychomycosis due to Aspergillus niger in a healthy adult: an unusual presentation of a non-dermatophyte mold infection.

BACKGROUND: Onychomycosis is commonly caused by dermatophytes, but nondermatophytic molds (NDMs) are increasingly recognized, even in immunocompetent individuals. These infections often present atypically, leading to delayed diagnosis. NDMs require accurate identification, as they may not respond to standard antifungal treatments used for dermatophyte infections. CASE REPORT: An immunocompetent man in his 40s presented with discoloration of a single right index fingernail. He reported no trauma, systemic illness, or immunosuppressive therapy. Examination revealed brown-black nail discoloration

Psoriasis associated with stress following a through-andthrough gunshot wound in a military service member: A case report.

The paper presents a clinical case of psoriasis that developed in a 23-year-old Ukrainian service member after a combat-related through-and-through gunshot wound to the lower leg sustained in 2022. The patient had no previous dermatological diseases or family history of psoriasis. Two weeks after the injury, irregular pink plaques appeared around the damaged skin areas and gradually spread to the lower limbs, trunk, and upper extremities, becoming generalised within two months. The disease subsequently progressed to severe plaque psoriasis, complicated by nail dystrophy and psoriatic arthritis

Intussusception in Cronkhite-Canada Syndrome.

Cronkhite-Canada syndrome is a rare, non-hereditary polyposis syndrome characterized by non-specific gastrointestinal symptoms accompanied by alopecia, cutaneous hyperpigmentation, and nail dystrophy. Characteristic endoscopic findings are diffuse sessile polypoid lesions with edematous mucosa. We report a case of a 44-year-old woman in whom intestinal ultrasound and CT revealed diffuse mucosal thickening and an ileocecal intussusception-findings that may not be pathognomonic but are highly unusual in adults. These imaging features served as critical red flags that directed the clinical suspic

Nail dystrophy successfully treated with dupilumab in pediatric atopic dermatitis: case series and literature review.

Atopic dermatitis (AD) is a common chronic, recurrent inflammatory disease, yet its accompanying nail abnormalities have long received insufficient attention. The clinical characteristics, underlying mechanisms, and assessment systems for nail dystrophy in AD remain unclear. AD-associated nail dystrophy can manifest in various forms, including Beau's lines, nail pitting, koilongchia, trachyonychia, leukonychia, brachyonychia, melanoychia, onychomadesis, onychoschizia, onycholysis, and paronychia. Current treatments face limitations such as slow onset of action and uncertain efficacy with tradi
