The various viral and other microbial infections represent only a small proportion (5-10%) of the noxae which threaten pregnancy and the fetus. Predictive indices for the outcome of measles were sought from a clinical and immunological study. A poor outcome was defined as death or persistence of bronchopulmonary disease 6 weeks after onset. The 47 Black children studied were well nourished and lymphopenic (absolute lymphocyte count less than 2 000/mm3) in the first 2 days of the exanthem. Chest radiographs of 335 Nigerian children presenting with pulmonary and cardiac complications of measles were studied systematically. Perihilar (88%) and diffuse (69%) pulmonary infiltrates were the commonest findings. Other abnormalities included segmental consolidation (34%), emphysema (20%), cardiomegaly (12%) and atelectasis (10%). Some 800 profoundly deaf children from 4 Yoruba states in Nigeria were assessed and examined in an attempt to identify the causes of deafness in this part of West Africa. It was found that measles, meningitis and rubella were the three major preventible diseases causing deafness. Intrathecal production of oligoclonal mumps-specific IgG was demonstrated in nine out of 10 children with mumps meningitis by imprint immunofixation (IIF) of sera and cerebrospinal fluids (CSF) separated by agarose electropheresis and by thin-layer electrofocusing. The purpose of the present study was to investigate whether risk of multiple sclerosis (MS) is associated with the occurrence of viral or bacterial diseases, with vaccinations, or with operative events during childhood. Eighty Nigerian children (mean age = 8.8 years) with rheumatic heart disease (RHD) were studied prospectively. The initial acute rheumatic fever (ARF) was recognized in 34 of them (42%) while 18 patients (23%) had recurrent ARF. 76 patients (95%) had mitral incompetence, either alone or in combination with other lesions. Thirty-six cases of interstitial pneumonia in acquired immunodeficient states were treated with transfer factor (Dialysable Leukocyte Extract) and studied retrospectively. The criteria of efficacity of this treatment were: rapidity of immediate improvement, improvement after failure of other immunostimulant therapy and demonstration of a dose-related effect. The mechanism of the therapeutic action is unclear. There is no evidence in favour of "transfer" of cell mediated immunity. A non-specific mode of action (adjuvant effect, interferon synergy, proinflammatory action) seems much more likely.

The occurrence of viral antibodies in relation to IgG separated by thin-layer polyacrylamide gel isoelectric focusing was studied in CSF and serum from 24 patients with mumps meningitis by immunofixation with viral antigens and autoradiography. Moustached marmosets (Saguinus mystax) were infected intranasally with either of two low-passaged, wildlike strains of measles virus, strain Edmonston or strain JM. Necrotizing lymphadenitis with Warthin-Finkeldey type giant cells was found in the inguinal lymph nodes of a 14-month-old boy following measles vaccination. Electron microscopy displayed (mostly in the endothelial cells) numerous intracytoplasmic inclusions composed of fine, dense granular and fibrillar material, which (among other structures) are known to occur within various cells in connection with measles paramyxovirus infection. Subsequent investigation disclosed familial immunodeficiency of a predominantly cellular type.

A four-year old girl had acute lymphoblastic leukemia in remission with immunosupressive chemotherapy and complicated giant cell pneumonia. Investigations by light and electron microscopy and immunoperoxidase study of autopsy material disclosed measles virus nature of the penumonia, although there was no typical rash of the disease. Similar giant cells were seen in the gastric and colon mucosa, and the other of Warthin-Finkeldey type in the spleen, and lymph nodes of pulmonary hiluses.
