Gangrene of the extremities in measles.

Gangrene of an extremity is very uncommon in measles, but we have encountered 3 cases. One patient had a peripheral variety with gangrene of the toes, while the other 2 had major vessel thrombosis. Evidence from the literature is advanced to support the hypothesis that disseminated intravascular coagulation on an immunological basis may be an important precipitating factor. Treatment is briefly discussed.

Cytomegalovirus complement fixation antibody in Guillain-Barré syndrome.

Cytomegalovirus, measles, and adenovirus antibodies were measured in the sera of 92 Guillain-Barré patients and 120 controls. Thirty patients (33 percent) had markedly elevated levels of complement-fixing antibody to cytomegalovirus and in 21, a fourfold or more alteration in titer was demonstrated. Diagnostic falls in titer were seen in most instances and no significant elevation to the other viral agents was found. The serologic findings reported here suggest that cytomegalovirus may be a common agent involved in the pathogenesis of the Guillain-Barré syndrome.

[Trendobservation of beneficial effects of vaccinations (author's transl)].

The beneficial effects of vaccinations may be calculated by means of special formulas: (formulas: see text). In these formulas R means the risks of the disease with regard to special complications, r the comparable risk of vaccination, p the protection rate and t the duration of protection. A vaccination is beneficial, if N and Q greater than 1 and D greater than 0. Vaccinations which show low levels for N, Q and D must be reevaluated constantly by means of the above mentioned formulas. This control may be performed also by calculation of the border - risk Rlim = r/p.t and the epidemiologic tr

Slow virus infections.

Slow viruses produce diseases whose incubation periods range from several months to many years. Because of this long latency period, the lack of inflammation produced by these diseases and the lack of recoverable virus particles, it is only recently that the association has been made between the viruses and the diseases they cause. The detailed study of kuru, a neurologic affliction of a remote tribe of cannibals in New Guinea, was responsible for the synthesis of new and previously gathered information into a unified framework to explain not only kuru but other diseases as well. Since then, a

[Non-specific fetal infections: possibilities and value of early diagnosis].

Notes of 4,000 obstetric and neo-natal cases have been analysed. The role of non-specific fetal infection (which means excluding syphilis, toxoplasmosis and German measles) has been defined with the backing of histopathological, bacteriological and clinical findings. Its role in perinatal mortality and neo-natal morbidity has been outlined. Possibilities for prevention of the condition are suggested, dealing with premature rupture of the membranes and clear-cut maternal infection. "At risk of infection" is the term that has been defined for neonates by studying carefully obstetrical clinical p

Multiple sclerosis and measles: a study of cellular sensitization.

Lymphocyte sensitization to measles has been studied in comparable groups of multiple sclerosis, other neurologic disease and normal subjects. The macrophage electrophoretic mobility test has been used. No difference between the three groups has been found, though there is a small group of children with obscure neurological disease which might well be multiple sclerosis and which fails to mount an adequate response to measles. The role of measles (and other banal infections) in aetiopathogenesis of multiple sclerosis is briefly discussed.

Multiple sclerosis and childhood infections.

There is evidence that some event in childhood may determine risk of multiple sclerosis: Elevated titers to measles and other childhood infections suggest a childhood infection. Therefore, childhood infections reported by 30 patients with multiple sclerosis and matched controls were compared. Patients reported a childhood infection between 5 and 9 years (not simply exposure to an infection) more often than controls. The mean age of measles peaked somewhat later (age 7) in patients than in controls (age 4); this differnce approached statistical significance (p less than 0.1). Evidence that host

Necropsy findings in childhood leukaemia, emphasizing neutropenic enterocolitis and cerebral calcification.

In 50 necropsies on leukaemic children, the major cause of death was infection. In patients dying during therapy for induction or reinduction of remission, the most frequent infection was a distinctive neutropenic enterocolitis or typhlitis. This was seen in 46% of the whole series and was a major factor in the death in 38%. Other infections were predominantly bacterial pneumonia in patients in relapse, and viral disease, e.g. measles pneumonia, in those in remission. One patient treated for meningeal leukaemia showed an unusual linear calcification of the cortical grey matter.

[Studies on the immune status of patients with operated and irradiated cervical and breast cancer. III. Comparative investigation of the immune status of patients with breast cancer treated by operation and irradiation and of patients with cervical cancer treated either by operation or irradiation (author's transl)].

The immunological reactivity of 19 operated cases of breast cancer (14 T1 2, 5T3) was investigated before and at 3-weekly intervals up to 12 weeks after radiotherapy by means of a large panel of techniques. The results were compared with two groups of identically-investigated patients, one consisting of 11 cases of cervical cancer (stage III), treated solely by irradiation and the other of 12 cases of cervical cancer (Stages I a and II b), treated by operation only. Before irradiation and in the non-irradiated group cellular immunity appeared to be more disturbed in cases of breast cancer than

[Interstitial pneumopathies in children treated for malignant diseases].

The authors report 21 cases of severe interstitial pneumonitis observed in 1974 in Paris, in children with malignancies, either solid tumours or leukemias. Twelve patients died. The incidence of these complications in children with intensive chemotherapy and the clinical symptoms which may reveal them are reviewed; the bad prognosis of measles in these patients is stressed. Radiological findings are described and the possible wrong diagnoses are listed. Among viral infections, measles is the severest and its diagnosis is often difficult. Pneumocystis carinii infection is frequent. The use of s

[Strategy of persistence (facts, hypotheses)].

The universal phenomenon of virus persistence is considered to be a regular phenomenon primarily meeting the interests of the host. Examinations of 1112 patients with various diseases of the nervous system revealed a certain dependence of macroglobulin measles antibody upon the character of the disease and the intensity of demyelinization process. These data do not suggest the etiological role of the persisting measles virus in the genesis of multiple sclerosis and a large group of other nervous system diseases (SSPE). Similar results were obtained in analogous examinations of patients for mac

Measles mortality. Analysis of the primary cause of death.

Four hundred fifty-four death certificates showing measles as the cause of death were analyzed. These represented 35.4% of the recorded deaths due to measles from 1964 through 1971. Respiratory or neurologic complications of measles or both were noted as the primary cause of death on nearly 90% of the certificates reviewed. In younger children, death was most frequently attributed to respiratory problems, while encephalitis and other neurologic sequelae of measles accounted for a larger percentage of deaths in the 10- to 14-year-olds. Nearly 17% of the persons who died had some underlying dise

Multiple sclerosis: relation to scrapie and slow infection; ageing and measles.

Multiple Sclerosis (MS) is shown to have no relation to scrapie and the evidence for a "slow infective" aetiology is very meagre. It is not related to an ageing process during which scrapie-like antigens make their appearance as they do in young animal with the disease. The emergence of scrapie-like antigens in ageing tissues is discussed. The increased sensitization of MS lymphocytes to scrapie, as opposed to normal brain (or spleen), is not characteristic of the disease, but occurs in other conditions with astroglial overgrowth. On the other hand, special lymphocyte sensitivity to inhibition

Measles pneumonia. Bacterial suprainfection as a complicating factor.

During a 3 1/2-month period, 32 previously healthy young men were first seen typical prodromal symptoms and signs of measles. On admission or within 48 hours of admission, 16 patients (50%) manifested physical signs and roentgenographic evidence of pneumonia. Of these 16 patients, ten (63%) were found to have bacterial suprainfection or colonization confirmed by culture of transtracheal aspirates. From six of these ten patients, Neisseria meningitidis serogroup Y was isolated as the sole organism responsible for suprainfection. In the other patients, Haemophilus species, Neisseria species (not

Rubella-virus infection in juvenile rheumatoid arthritis.

Antibody activity against mumps, measles, polio, and rubella viruses was determined in patients with juvenile rheumatoid arthritis (J.R.A.), rubella-vaccine associated arthritis, adult rheumatoid arthritis, other chronic systemic disorders (e.g., systemic lupus and dermatomyositis), and in a matched population of normal, non-rheumatoid (control) children. The antibody levels against mumps, measles, and poliovirus were similar in all patients. Rubella-antibody levels in rheumatoid arthritis and other systemic disorders were similar to those observed in controls. The mean rubella-antibody levels

Otolaryngological complications of measles in West Africa.

In Nigeria as in West Africa generally measles is a serious condition with a high morbidity and mortality rate. Complications involve every section of Otolaryngology. Some of these are amenable to treatment by the otolaryngologist alone, while others need the skill of colleagues in other specialties. The irreversible neuro-audiological complications cause greatest concern to physician and parent and will continue to do so until the variety of available therapeutic, preventive and rehabilitative services are increased and improved.

Pancreatitis in children. A study in the North of England.

Cases of pancreatitis in children in the Newcastle Regional Board area were studied over approximately 6 years. There were 9 cases aged between 6 weeks and 15 years. In 5 children (all under 7 years of age) likely aetiological factors were found (2 cases of mumps and 1 case wach due to trauma, measles, and hereditary pancreatitis). In 4 children all over 13 years of age no likely aetiological factor was found. It is speculated that they may represent one end of the age spectrum of idiopathic pancreatitis as it occurs in adults. Hereditary pancreatis was definitely found in only one child and p

[The acute visual hallucinosis in infancy. Clinical, neurophysiological and psychodevelopmental aspects and differential typology (author's transl)].

By introducing the definition "hallucinosis" (Wernicke) it has become possible to confine the psychoses of organic origin more closely. Therefore, this term should also be used in pediatry and pedopsychiatry in order to designate cases with corresponding clinical aspects. Thus, accordance to the phenomenological characteristics of such syndromes as described in this paper, it is justified to emphasize that the acute hallucinosis in children is a special type of disease as compared to other psychoses caused by exogenic influences in this age group. The 10 case reports deal with visual hallucino
